Addendum No. 1 Invitation for Price Quote (IFPQ) #02A2338

NOTICE TO ALL PROSPECTIVE BIDDERS:

Addendum No. 1 is being issued for the above-referenced IFPQ. Quotes for this work shall be with the full understanding of this Addendum.

The following represents changes for this IFPQ:

Price Quote Proposal, Attachment 1, has been revised. Please refer to the revised Price Quote Proposal, Attachment 1, at the end of this Addendum for all changes.

IFPQ Attachment 4, Exhibit A, item 7., I., c), has been revised as follows:

c) Contractor shall begin the morning shift at 7:00 a.m. to 8:30 a.m. and the afternoon shift at 4:30 p.m. to 6:00 p.m. each day.

The following represents questions submitted for this IFPQ and the Department’s responses:

Question 1: Since this bid is going by a Monthly Unit price, will this Monthly Unit price (or the total amount of bid divided by 19) be the amount we will be paid per month if we are awarded the contract? And the number 19 is the number of months we're bidding on? We're asking because it's always been based on hours before, so this is different.

Answer 1: Please see changes to Price Quote Proposal, Attachment 1. This contract is for 18 months. Contractor shall be paid the monthly rate per Price Quote Proposal, Attachment 1.

Question 2: Exhibit A Section #3 says Contractor must maintain accurate daily log for hours worked for all employees. We are an owner/operator Contractor business with NO employees. We have no hourly employees; we perform all services. So, this must not apply to us, correct?

Answer 2: Daily log is to track total hours that the contractor was on site.

All other terms and conditions set forth in the IFPQ remain in full force and effect.

Sincerely, Wendy Kwong Wendy Kwong Contract Analyst
