About this file

The document is an Addendum No. 02 to an Invitation to Bid (ITB) issued by the Florida Department of Children and Families (DCF) for the Florida State Hospital Waterline Replacement project. The project involves the construction of approximately 1,010 linear feet of 12-inch polyvinyl chloride (PVC) waterline to replace a portion of an existing water line along US Highway 90 that serves the Florida State Hospital. The project must comply with all technical documents provided and state and local regulations. The timeline for the solicitation has been extended, with written questions due by April 24, 2023, and electronic bids due by May 15, 2023. The anticipated contract start date is June 1, 2023, with an estimated 150-day duration for the project.

The Addendum includes changes to the bid sheet, allowing vendors to provide an estimated work begin date and indicating that the low bid will be determined based on the lowest overall price for the base bid and accepted alternates. Vendors are required to provide a bid bond of not less than 5% of the bid amount if the base bid and sum of all additive alternates exceeds $100,000. The successful vendor will be required to furnish a satisfactory contract public construction bond in the amount of 100% of the accepted bid.
