About this file
This document is a Justification and Approval (J&A) for a sole source contract with Transformcare, Inc. for the Defense Health Agency (DHA). The contract is for Accelerating Care Transformation (ACT) support, valued at $5,947,960.00, with a 9-month base period from September 30, 2025 to June 29, 2026, using Operations and Maintenance (O&M) funding. The services include diagnostics, future state modeling, implementation, and solution management across multiple venture sites, with the goal of continuing the integration of healthcare transformation information into clinical areas to enable a patient-centered, digitally enabled care delivery system.
The sole source justification is based on Transformcare's ongoing support of the DHA's Healthcare Prototype Transformation (HPT) accelerator, which began in September 2023. The company has demonstrated deep understanding of DHA and Military Health System operations, and has been continuously supporting the work since its inception. Market research confirmed that introducing a new vendor would result in unacceptably high risk due to the steep learning curve required to understand the Department of Defense application. A competitive Source Selection for a full contract is scheduled to occur in FY26, making this a bridge contract to ensure continuity of critical healthcare transformation efforts.
