1.0 Introduction: This Performance Work Statement (PWS) is for a services contract to repair and maintain five (5) elevators (one (1) hydraulic and four (4) traction) located at Melvin Price Lock and Dam, East Alton, IL, 62024.

1.1 Description of Services: The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and services necessary to perform elevator repair and maintenance so that the elevators are working 24 hours a day, 7 days a week and 365 days a year as defined in this Performance Work Statement. The Contractor shall perform to the standards in this contract.

1.2 Background: Melvin Price Lock and Dam is located in Madison County at #1 Lock and Dam Way, East Alton, IL on the Mississippi River, approximately 1.5 miles southeast of the Clark Bridge. It contains five (5) elevators:

Abutment (0) and Piers 1, 3, 4 and 11. Abutment, Pier 1, Pier3, Pier 4, and Pier 11 was last inspected in April 2017.

The inspection reports will be made available to the selected Contractor.

1.3 Definitions:

Acceptable Quality Level (AQL): The AQL is the maximum percent defective that, for purposes of sampling, inspection can be considered satisfactory.

Contractor: The Prime Contractor and all Contractor employees and personnel. The Prime Contractor shall be responsible for ensuring all employees and subcontractors comply with the provisions of this contract.

Contracting Officer (KO): A person duly appointed with the authority to enter into and administer contracts on behalf of the Government.

Contractor’s Representative: A foreman or superintendent assigned to represent the interests of the Contractor with regards to all matters included in this contract.

Critical Element: A critical element is a task, procedure, or operation that has been determined to be mandatory to fulfill the condition, value, or measure comprising a standard.

Customer Complaint: A Government program that is explained to every organization that receives service under this contract which is used to evaluate a Contractor’s performance, and a means of documenting certain kinds of contract service problems.

Defect: An instance of noncompliance with a service requirement. A defect may be caused by either non-performance or unsatisfactory performance.

Defective Service: A service output that does not meet the standard of performance associated with it in the Performance Requirements Summary (PRS).

Deliverable: Anything that can be physically delivered but may include non-physical things such as safety meeting minutes.

Government Property: All property owned by or leased to the Government or acquired by the Government under the terms of the contract.

Key Personnel: Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.

Major Defect: A major defect reduces the usability of the unit of service for its intended purpose. A major defect is a defect that judgment and experience indicate would result in hazardous or unsafe conditions for individuals using, maintaining, or depending on the service delivery require corrective action to achieve substantial compliance with an established standard. A major defect is also classified as a collection of two or more minor defects in the same general area.

Minor Defect: A minor defect is a defect not likely to reduce the usability of the unit of service for its intended purpose, or is a departure from the established standards having little or no bearing on the effectiveness of the unit of service. A minor defect is a defect that judgment and experience indicates is not a substantial deviation from requirements and generally does not require re-performance for overall performance to be considered satisfactory.

Non-Critical Element: A non-critical element is a task, procedure, or operation that has been determined to be essential to the overall condition, value, or measure comprising a standard, but will not constitute a substantial deviation (major-defect) from the standard unless viewed in the context of repeated unsatisfactory performance, or non-performance.

Performance Requirement: The point that divides acceptable and unacceptable performance. In the case of surveillance by random sampling, the performance requirement is the maximum number of defectives in a random sample chosen that may occur before the Government will affect the price computation system in accordance with the Performance Requirements Summary and the Inspection of Services clause. When the method of surveillance is other than random sampling, the performance requirement is the number of defectives or maximum percent defective in the lot before the Government will affect the price computation system in accordance with the Performance Requirements Summary.

Performance Requirements Summary (PRS): Identifies the key service outputs of the contract that will be evaluated by the Government to assure contract performance standards are met by the Contractor.

Performance Standard: A desired condition, value, or measure by which a service or product is judged to meet a specification or requirement.

It accurately describes the contract service requirements, including the standards to be used to determine whether the requirements have been met.

Physical Security: Actions that prevent the loss or damage of Government property.

Planned Sampling: A sampling method in which work outputs are selected in accordance with subjective criteria established in the surveillance plan.

Factors that may contribute to removal of a contractor employee include not following site safety and security requirements, accessing secured areas without an escort and working while under the influence of alcohol and/or drugs. Due to the presence of the general public for daily tours of the facility, professional behavior is expected at all times.
