COMBINED SYNOPSIS/SOLICITATION FOR BELKIN MODULAR DP DUAL HEAD HOST
CABLE
Solicitation Number: 75D301-24-Q-0016
PART 1 – INTRODUCTION
The Centers for Disease Control and Prevention (CDC), Office of Science, Safety and Asset Management has a requirement for journal subscriptions.
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
(ii) The solicitation number is 75D301-24-Q-0016. This solicitation is issued as a Request for Quotation (RFQ), using the Simplified Acquisition Procedures of FAR 13 and FAR Part 12.
(iii) The solicitation document/incorporated provisions and clauses are those in effect through the current Federal Acquisition Circular: FAC 2023-04, dated 06/02/2023.
(iv) The associated North American Industry Classification System (NAICS) Code is 334118 – Computer Peripherals; Small Business Size Standard is 1000 employees.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If
Solicitation Number: 75D301-24-Q-0016 this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. HHSAR 352.232-71 Electronic Submission of Payment Requests
(a) Definitions. As used in this clause -
Payment request means a bill, voucher, invoice, or request for contract financing payment with associated supporting documentation. The payment request must comply with the requirements identified in FAR 32.905(b), “Content of Invoices” and the applicable Payment clause included in this contract.
(b) Except as provided in paragraph (c) of this clause, the Contractor shall submit payment requests electronically using the Department of Treasury Invoice Processing Platform (IPP) or successor system. Information regarding IPP, including IPP Customer Support contact information, is available at www.ipp.gov or any successor site.

2. QUOTE FORMAT
2.1. Quote Preparation - The technical proposal shall include a detailed description of the requirement.
A successful technical proposal shall include the affirmation of the offer price (s); a Point Of Contact (name, title, email address and telephone number); a statement from the offeror verifying that they are SAM Registered under NAICS code 334118; delivery terms; business size; and payment terms.
2.2. Technical Capability Contents - The offeror shall provide documentation (e.g., technical brochure, explanation, or any other supporting documentation) that they are an authorized manufacturer distributer/reseller of Belkin products. Volume 1 – Technical shall not exceed 3 pages including cover letters, table of contents, appendices and attachments. Offerors who submit more than 3 pages will be considered as being nonresponsive and the proposals will not be considered for award.
2.3. Price Contents- The offeror shall submit a complete pricing sheet/list. Pricing should include an individual item list based on the brand name description and a completed Section B for each proposed line item.
PART 4- EVALUATION FACTORS/CRITERIA FOR AWARD
THE GOVERNMENT INTENDS TO AWARD A SINGLE FFP PURCHASE ORDER TO THE ELIGIBLE AND RESPONSIBLE
OFFEROR ON A LOWEST PRICE, TECHNICALLY ACCEPTABLE BASIS. IN ORDER TO BE DETERMINED TECHNICALLY
ACCEPTABLE:
(1) THE OFFEROR MUST QUOTE CABLES IN NEW CONDITION THAT MEET ALL OF THE MINIMUM REQUIREMENTS, IN THE REQUIRED QUANTITIES SPECIFIED ABOVE; THE OFFEROR MUST PROVIDE SUBSTANTIATION TO
DEMONSTRATE THAT THE QUOTED CABLES MEETS THE GOVERNMENT’S REQUIREMENT.
(2) PROVIDE PROOF THAT ALL ITEMS ARE SHIPPED FROM THE US; DROP SHIPPING FROM OUTSIDE THE US WILL
NOT BE ACCEPTABLE.
