CERTIFICATION

In accordance with the Environmental Protection Agency’s (EPA) National Emission

Standards for Hazardous Air Pollutants (NESHAP), an inspection was conducted at

1111 Indiahoma Road located in Indiahoma, Oklahoma.

The survey was performed by Ms. Maci Wiley and Mr. Jason Wiley, Environmental

Protection Agency/Asbestos Hazard Emergency Response Act (EPA/AHERA) accredited, and Oklahoma licensed asbestos inspectors. The scope of this survey included minimally destructive methods and did not include evaluation of architectural plans, or the quantification of materials for abatement purposes, or removal cost estimating.

The inspection followed good industrial practice without prejudice toward any of the parties (occupants, owners, insurers, builders, contractors, or others) who may have a direct or indirect interest in the property or outcome of this evaluation. James R. Gaylor, CHMM or any staff of Environmental Measurements, LLC has no interest in the property that would persuade or influence the outcome of this evaluation. This report has been developed with the purpose and intent to disclose any potentially significant asbestos containing material which might pose a health threat or other environmental concern.

1.0 EXECUTIVE SUMMARY

1.1 Background

EPA regulation 40 CFR 61.140, the National Emission Standards for Hazardous Air

Pollutants (NESHAP), prohibits the release of asbestos fibers to the atmosphere during any renovation or demolition activity. The regulation further requires that potentially regulated asbestos containing building materials be identified and quantified before any planned renovation or demolition activity.

Regulated Asbestos-Containing Material (RACM) is defined as (a) Friable asbestos materials, (b) Category I non-friable ACM that has become friable, (c) Category I non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading, or (d) Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition or renovation operations.

Any friable or regulated asbestos material that is found is under the authority of the

Oklahoma Department of Labor and is required to be removed before any demolition occurs under both state and federal law. The facility owner and/or demolition -renovation contractor will be required to file a NESHAP notification with the

Oklahoma Department of Environmental Quality (ODEQ) 10 days prior to the demolition. (Note: A NESHAP notification is required on all demolition projects.)

1.2 Executive Summary

In pursuant of NESHAP requirements, the purpose of this Demolition Asbestos Survey performed at 1111 Indiahoma Road located in Indiahoma, Oklahoma, was to identify accessible asbestos-containing materials (ACMs) within structures on the site prior to scheduled demolition plans which might disturb or cause asbestos containing material to become friable during such activity. This inspection was completed between the dates of May 19 through May 22, 2021, by Ms. Maci Wiley and Mr. Jason Wiley, Oklahoma Department of Labor (ODOL) licensed Asbestos Inspectors, in accordance with the published EPA/ Asbestos Hazard Emergency Response Act (AHERA) protocols.

2.0 SAMPLING & ANALYTICAL PROTOCOLS

2.1 General Procedures

An inspection to determine the presence and condition of any asbestos containing building material (ACBM) is required. The inspection was performed by visually observing accessible areas within the scope of work to identify potential suspect asbestos containing building materials. After the overall visual survey was completed, representative sampling areas were determined. The surveyor delineated homogeneous areas (HA) of suspect materials and samples of each material were obtained, in general accordance with regulations as established by the Occupational

Safety and Health Administration (OSHA) and NESHAP. Both friable and non-friable materials were sampled. A friable material is one that can be crushed when dry by normal hand pressure.

This survey did not include the demolition of building components to access suspect material. The field surveyor determined sample locations based on previous experience. Suspect materials included sheetrock, joint compound, wall texture, plaster, ceiling tile, vinyl floor tile and associated mastic, cementitious paneling, sheet vinyl and associated adhesive.

The asbestos inspection was conducted according to Asbestos Hazard Emergency

Response Act (AHERA) guidelines using a minimum number of samples collected from each HA, which meets the sampling criteria found in 29 CFR 1926.1101. Quality

Assurance (QA) samples were submitted every twentieth (20th) sample collected.

These samples were independently marked in a manner to conceal the presence of QA samples with exception to reference of this report.

Upon completion of the fieldwork, samples were delivered into the custody of

QuanTem Laboratories (QuanTem), located in Oklahoma City, Oklahoma. QuanTem is a National Voluntary Laboratory Accreditation Program (NVLAP) accredited laboratory. The independently marked QA samples were delivered to the custody of a separate NVLAP accredited lab, Crisp Analytical Labs (CA Labs), located in

Carrollton, Texas. Both sample sets were analyzed by polarized light microscopy

(PLM) following the current published EPA method. Summaries detailing the homogeneous areas for each building are included below. Copies of all laboratory results are attached for your permanent records. The laboratory results from the suspect homogeneous areas show regulated and/ or non-regulated asbestos containing building materials to be present in six (6) of the twenty-six (26) sampled structures across the site.

2.2 Sampling Protocol

Identification of Sample Locations:

Samples of asbestos materials were collected in a randomly distributed manner sufficient to determine whether the materials were asbestos containing. Under the inspectors’ professional experience and discretion, homogeneous areas that were determined as non-asbestos containing (such as fiberglass, foam glass, or rubber) was not sampled.

Sampling Methodology:

Suspect asbestos samples were collected in a manner as to not create excessive dust or damage. Adequate wetting and debris cleaning took place throughout the inspection. Representative samples were taken of layers to the substrate of materials to reveal materials that may be otherwise concealed underneath the surface layer.

3.0 SITE DESCRIPTION

3.1 Summary of Site

An original 26 buildings, totaling approximately 109,000 square feet, was determined to be tested by Environmental Measurements. Construction dates of the buildings located at Former Treasure Lake Job Corps located at 1111 Indiahoma Road in

Indiahoma, Oklahoma varied from 1934-1995. During the inspection, Environmental

Measurements identified a total of 30 structures, with four (4) of these structures being determined to contain no identified suspect asbestos containing building materials at the time of the inspection. These non-suspect structures, as well as those that were sampled for asbestos containing building materials are summarized in

Table 3.2 below. A representative view of the site is contained in Appendix D.

Under EPA guidelines, asbestos containing material is any material with greater than 1% asbestos by weight. Results of this asbestos inspection indicate thirteen

(13) homogeneous areas with regulated (friable) asbestos containing materials across six (6) of the thirty (30) surveyed structures as identified in Table 3.2-

Structure Master List.

Damaged regulated (friable) asbestos containing materials were identified were identified in structures 12 and 27. These materials require

We recommend that the identified RACM be maintained as part of an O&M

Program and periodically monitored for any changes in condition. Additionally, we recommend that a licensed asbestos abatement contractor properly remove and dispose of the identified materials prior to conducting renovation or demolition activities that might disturb the ACM.

5.2 Identified Non-Friable ACM

• Structure #12: Vacated Cafeteria o CT-02: Black/ Brown Mastic Puck (5% Chrysotile)

▪ Condition: Good

• Structure #26: Carpenter Shop o FT-01: 12” White Vinyl Floor Tile (5% Chrysotile)

▪ Condition: Good

Category I non-friable ACM means asbestos-containing packings, gaskets, resilient floor covering, asphalt roofing products, and pliable sealants and mastics that are in good condition and not friable, containing more than one percent asbestos, as determined using the method specified in Appendix E, Subpart E, 40 CFR Part 763, Section 1, PLM. Results of this asbestos inspection indicate two (2) homogenous areas with Category I nonfriable asbestos containing materials across two (2) of the thirty (30) surveyed structures identified in Table 3.2- Structure Master List.

These materials may remain within the facilities during demolition, provided they remain non-friable. However, there is no guarantee that they will remain non-friable.

If a material becomes friable, then it is classified as RACM. RACM, as defined by the EPA, must be removed prior to renovation or demolition activities that may disturb the materials. Additionally, we recommend that a qualified contractor properly perform such removal of the ACM prior to such disturbances. Also, OSHA has additional requirements that may supersede the EPA rules. These materials do not appear to present a significant issue, as observed at the time of the survey. The identified ACM is recommended to be maintained as a part of an O&M Program and periodically monitored for any changes in condition.

5.3 Guidance Summary

Point Counting

Two (2) homogenous areas of suspect RACM were determined to contain less than

10 percent asbestos by PLM analysis. According to the NESHAP, when the asbestos content of a bulk sample of suspect RACM is determined to be less than 10 percent by PLM visual estimation, you may:

1. Assume the amount to be greater than one percent and treat the material as asbestos-containing; or

2. Conduct confirmatory verification by point-counting. Note, the results obtained by point-counting are considered the definitive analytical result.

Environmental Measurements should be procured for the following applicable events;

I. Planned Renovation: Prepare specifications for abatement of friable and non-friable asbestos materials that would be disturbed during renovations activities.

II. Planned Demolition: Prepare specifications for abatement of friable materials and nonfriable asbestos materials that may become friable in the demolition activities.

III. Continued Operation of Facility without Abatement: Prepare and implement an Asbestos Management Plan to manage the asbestos in place. Abatement of identified damaged materials are to be abated and cleaned by a licensed asbestos contractor to allow for public occupancy of these structures.

6.0 Conclusions

Asbestos has been commonly used in building materials for decades. Due, in part, to its physical properties, asbestos fibers may be found in insulation, acoustical ceiling materials, fireproofing and various floor coverings. The Environmental Protection

Agency banned the use of asbestos in most building materials by the late 1970’s, however it is still possible to find asbestos containing material in materials such as roofing tars, floor mastics, etc. Fibrous forms of asbestos are known carcinogens.

Inhalation of these fibers may lead to the development of various related diseases such as asbestosis, mesothelioma, or lung cancer.

Based on the on-site inspection, and sample analysis of the suspect materials in this facility, both non-regulated and regulated asbestos containing materials were identified. This survey has been conducted in a manner consistent with that level of care and skill ordinarily exercised by members of the profession currently practicing under similar conditions in the same locale. The results, findings and conclusions, and recommendations expressed in this report are based on conditions observed during our survey of readily accessible areas of the building. It was not within the scope of this survey to remove surface materials to observe hidden or otherwise inaccessible materials.

7.0 Limitations

As a result of previous renovations, there may be hidden materials, such as floor tile, sheet vinyl flooring, insulation, etc. These materials may be found in various areas hidden under existing flooring materials or in wall cavities. Any materials found during construction activities, either not addressed in this survey report, or similar to the ACM identified in this survey report should be assumed to be ACM until sampling and analysis documents otherwise.

Because of the hidden nature of many building components (i.e., within mechanical chases), it may be impossible to determine if all the suspect building materials have been located and subsequently tested. Destructive testing in some instances is not a viable option. We cannot, therefore, guarantee that all potential ACM has been located.

For the same reasons, estimates of quantities and/or conditions are subject to readily apparent situations, and our findings reflect this condition. We do warrant, however, that the investigations and methodology reflect our best efforts based upon the prevailing standard of care in the environmental industry.
