STATEMENT OF WORK
Aneth Community School
Purchase and Install Playground Equipment
A. SCOPE OF WORK:
The Contractor shall furnish labor, materials, equipment and supervision necessary to Purchase and Install Playground Equipment at Aneth Community School.
B. PROJECT LOCATION:
East Highway 162, San Juan County Road 466 Montezuma Creek, UT 84534
C. SPECIFIC REQUIREMENTS:
Overview:
The Playground Installation Project at Aneth Community School shall follow the scope of work and shall be submitted through Bureau of Indian Education (BIE) contracting office.
The proposed new playground is located on the Navajo Reservation at the address above.
This equipment will support the Family and Child Education program. students in grades K- 8.
General Information:
A school site visit is required to verify conditions and measurements. A site visit is scheduled for:
July 20, 2021, at 10:00am local
The contractor shall purchase and install playground equipment at Aneth Community School.
The playground equipment will be Landscape Structures Smart Play Playground System (210183A) or equal. Equipment will be installed per manufacturer drawings and specifications. Contractor shall provide drawings, submittals, and all other pertinent information to properly size and install playground equipment system. Contractor is responsible to locate utility and overhead lines before any construction is to begin.
Contractor shall provide an itemized cost breakdown for this request for quote.
Scope of Work The contractor shall provide all equipment, materials, labor, supervision, and performance of work as shown and specified in the submittals, specifications and provision of this Contract.
A substantial walk-through inspection shall be conducted with the contractor at the end of the project to finalize the contract.
The playground equipment shall meet or exceed Landscape Structures Smart Play Playground System and meet the following:
• Integrated shade structures,
• Benches/Play Tables with seats,
• Protective surfacing with borders,
• Cut-outs in roofs to create shadow play on decks below,
• Minimum of 16 play activities,
• Designed for ages 2-5, and
• ADA compliant.
Playground equipment colors will be selected by the school location.
The vendor shall be responsible for the following:
• Mobilization cost, such as unloading and storage of materials,
• Site preparation and orientation per drawings,
• Construction of new playground equipment shall be performed by a Factory Certified Installer and also have a certified safety inspector on site at all times,
• Installation of new play system per manufacturer's guidelines and specifications,
• Installation of a new border to contain engineered wood fiber surfacing,
• Installation of 12" of engineered wood fiber surfacing,
• Provide a written audit report, including a drop test by a certified playground safety inspector when construction is complete, and
• Clean up of all materials on site when construction is complete.
D. GENERAL REQUIREMENTS:
BIA/BIE Regulations The contractor shall adhere to all BIA/BIE adopted codes and the contractor shall abide by all OSHA Safety regulations while on school grounds and shall review all applicable safety regulations before beginning the project. The contractor shall provide signs, barriers, and or caution tape as a safety precaution around the work site. The contractor shall have a fall protection plan if workers are to be preforming duties above six (6) feet from the ground or finished floor. The contractor shall bear sole responsibility of taking all appropriate actions necessary to prevent accidents, injuries to individuals and damage to property in and around the work area.
Playground Equipment Applicable Codes All playground equipment shall meet ASTM Fi487-l 7 and be IPEMA certified.
Manufacturer should maintain ISO 14001 and 900 I for environmental and quality control standards. All playground equipment shall follow and meet American with Disabilities Act
(ADA).
Playground Equipment Debris Removal:
Contractor shall remove all debris generated by or caused by the demolition work in this contract. The Contractor bears all responsibility and all associated or related costs with the removal and disposal of all debris from properties of the United States Government.
The Contractor is required to comply with applicable Environmental Protection Agency (EPA), Federal, State, and local Government laws, policies and regulations while engaged in the removal and disposal debris.
The Contractor shall transport the debris off the boundaries of the Navajo Reservation to an approved EPA landfill. The Government is not responsible in any way, financially or otherwise, for the success or failure of arrangements for disposal of debris between the Contractor and other parties.
Warranty:
Contractor shall provide manufacturer's warranty information to the school at the end of the project. The contractor shall also provide a one (1) year warranty for all workmanship from date of acceptance by the Government.
Shop Drawing Distribution 100 percent drawings shall be distributed to the following individuals:
Contracting Office - email copy
New Mexico Navajo North – 2 full sets COR, Facility Management Office
Principal – 2 full sets Aneth Community School
Division of Safety & Risk Management – 2 full sets
Working Hours Work on this project shall be performed during normal duty hours of 8:00 am to 4:00 pm each day unless other arrangements are made between the contractor and the COR.
(Exception: Workdays may be interrupted by unannounced closures due to Tribal Traditional Activities).
Background Investigations The contractor shall provide a designated individual(s) from their company who will be subject to a BIA Security Background Investigations in accordance with Section 231 of the Crime Control Act of 1990, Public Law 101-647. Background Investigation forms will be provided at the pre-construction meeting. Please note background investigations will not be required if no students are present.
Navajo Nation and State Taxes Contractor is responsible and required to pay Navajo Nation Tribal Tax, which is equal to 6 percent of their total bid amount. In additions, the contractor shall also pay local and state gross receipt taxes.
Performance Performance time for this estimated project is approximately 120 calendar days from Notice to Proceed. All work shall be completed in a professional manner and without delay once the project has begun.
Cleaning The Contractor shall keep the work area reasonably clean at all times. The Contractor shall be responsible for properly disposing of all debris.
E. PRODUCTS:
Materials All materials for use on the project will be in new and in an undamaged condition. Also, a statement indicating all materials, supplies and equipment are asbestos free.
