FORT APACHE AGENCY
SOLID WASTE/REFUSE COLLECTION
GENERAL: This is a non-personnel service s contract to provide refuse and recycling collection services at Fort Apache Agency. The Government shall not exercise any supervision or control over the contract service provides performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the
Government.
DESCRIPTION OF SERVICES: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personnel services necessary to perform refuse and recycling collection services as defined in this Scope of
Work (SOW) except for those items specified as government furnished property and services.
The contractor shall perform to the standards in this contract.
BACKGROUND: Fort Apache Agency is a Federal Government establishment responsible for the
Operations and Maintenance of its federally owned and operated facilities. Facilities includes;
offices buildings, shops, warehouses and Government Furnish Quarters. Many routine maintenance items are contracted to private sectors. Refuse collection are one of the activities that are performed by private contractors at Fort Apache Agency.
SCOPE: The contractor shall provide all manpower, labor, equipment, materials/supplies, transportation, fuel, tools, supervision and other items and services necessary to provide refuse collection and disposal services for designed within Fort Apache Agency, White River, Arizona as described in the attach SOW. The contractor shall perform to the standard in the contract.
PERIOD OF PERFORMANCE: The period of performance shall be for one (1) Year.
QUALITY CONTROL: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this SOW. The contractor shall develop and implement procedures to identify, prevent and ensure non-recurrence of defective services. The contractor's quality control program is the means by which he assures himself that his work complies with the requirement of the contract. Contractor shall provide QC Plan within thirty {30) days after contract aware, one copy of a comprehensive written Quality
Control Plan {QCP) shall be submitted to the CO and COTR within ten {10} working days when changes are made thereafter. After acceptance of the QCP the contractor shall receive the contracting officer's acceptance in writing of any proposed changes to his QC system.

BIA Cedar Creek, Government Furnish Quarters (#407, 408, 409, and 413.)
Department of Correction (DOC) Bldg #95.
DUMPSTER PLACEMENT: The areas subject to refuse collection under this procurement and the approximate number and sizes of receptacles to be serviced, are shown as follows;
a. BIA Fort Apache Agency, Warehouse one {1) - 6 yard bin.
b. BIA Forestry, Rick Lupe Center one (1) - 6 yard bin.
c. BIA Fort Apache Detention, WMAT Department of Correction one (1) - 6 yard bin and one (1) - 3 yard bin.
d. BIA Fort Apache Agency Government Housing four (4) 90-gallon trash containers.
e. BIA Cedar Creek Government Housing four (4) 90-gallon trash containers.
f. BIA McNary Government Housing two (2) 90-gallon trash containers.
REMOVAL AND DISPOSAL: Contractor shall empty all bins and trash containers once a week including on all holidays. Contractor may service bins/containers one day prior or one after
Federal Holidays . Contractor shall pick up all refuse within 10 feet of bin/containers. Contractor shall clean up immediately any spillage of refuse in course of removal and disposal operation.
POST AWARD CONFERENCE/PERIODIC PROGRESS MEETINGS: The contractor agrees to attend any post award conference convened by the contracting activities or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The contracting officer
(CO), Contracting Officer's Technical Representative (COTR) and other government personnel as appropriate may meet periodically with the contractor to review the contractor's performance.
At these meetings the CO will apprise the Government of problems, if any is being experienced.
CONTRACTING OFFICER TECHNICAL REPRESENTATIVE (COTR): The COTR will be identified by separate letter. The COTR monitors all technical aspects of the contract and assist in contract -administration. The COTR is authorized to perform the following functions: assure that the
Contractor performs the technical requirement of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the
Contractor concerning technical aspects of the contract: issues written interpretations of technical requirements, including Government drawings, designs, specifications: monitor
Contractor's performance and notifies both the CO and Contractor of any deficiencies:
coordinate availability of government furnished property, and provide site entry of Contractor
