1. Background

This contract requires the contractor to provide all necessary services and materials for the completion of work to install exposed aggregate accessible sidewalk at Abraham Lincoln Birthplace NHP. This work requires the installing of 135 linear feet x 6 feet wide of exposed aggregate sidewalk along the ellipse in the V.C. Parking area. Competing contractors are encouraged to visit site and self-determine project needs.

2. Objectives:

Install new exposed aggregate sidewalk along the ellipse in the Visitor Center Parking Area to match existing sidewalks. All new sidewalks must meet accessible standards.

3. Place of Performance Visitor Center

2995 Lincoln Farm Road, Hodgenville KY Permitted Work Hours: Monday through Friday, 7:00am – 3:30pm

4. Period of Performance Work shall be completed within 90 days after the notice to proceed (NTP)

5. Scope:

This project consists of installation of the aforementioned items. The duration of this project shall be 90 days from the Notice to Proceed (NTP)

6. Tasks:

Under this contract, the contractor shall complete the following tasks and requirements:

· Installation of the following items and associated tasks as identified

· Base Bid Item #1 Installation of an accessible sidewalk constructed out of exposed aggregate to match the existing sidewalks in the area.

Requirements

· Removal of soil along the ellipse next to the parking area road to ensure depth will allow for a new 135 linear feet x 6 foot wide x 5-inch-thick exposed concrete sidewalk to be constructed.

· All sidewalks will be in conformance of ABAAS standards and have a maximum of 5% running slope and a maximum of 2 % cross slope.

· Concrete mix for approval shall meet the following:

a. 4,500 psi @ 28 days

b. 3/8 aggregate size- color/texture to match existing

c. Air Entrained- 6.0 1/- 1.0% conforming to ASTM C94

d. Maximum water/Cement ratio .45

e. Slump 5.00 +/- 1.00 inch

f. Slump after SP: 6.00 +/- 2.00

· Exposed aggregate finish shall match existing sidewalks. Contractor shall provide a 6’ x 5’ sample panel for approval prior to installation of all sidewalks.

· New sidewalks must meet exiting sidewalks flush and have a install ½” expansion joint (Non-Bituminous) Seal with Elastomeric Sealant at both ends of new sidewalk where it meets existing sidewalks.

· Contractor shall spray retardant on surface according to manufacturer’s recommendations and at the correct time to achieve proper finish to match existing sidewalks.

· Contractor shall apply two coats of pavement sealer for use with exposed aggregate as approved by the Contracting Officer.

· Concrete shall not be placed if temperatures are below 40 degrees during a 24 hour period.

· Contractor shall install ½” expansion joint at 25’ O.C. (Non-Bituminous) Seal with Elastomeric Sealant and tooled ½” contraction joints with a radial edge at 5’ O.C.

· Contractor shall compact subgrade within a 98% density

· Contractor shall install 6x6-W1.4xW1.4 (10x10), 21 lb/CSF wire mesh for reinforcement the entire length and width of new concrete.

· Contractor shall not remove forms until 24 hours after concrete has been placed.

· Contractor is responsible for removal of all debris from the work site and off park grounds.

· Contractor shall confirm all measurements and quantities.

· Final inspection shall include confirming all maximum slopes are met.

7. Submittals/ Deliverables:

· Concrete Mix

· Mesh Reinforcement material

· Retardant material

· Sealant material

· Expansion joint material

· 6’ x 5’ Sample panel of concrete

· Project Scheule

· Quality Control Plan

· Safety Plan

· Designated Superintendent Letter

· Certificate of Liability Insurance

· SF1413 (For any Subcontractors)

8. Government-Furnished Property, Material, Equipment, or Information (GFP, GFM, GFE, or GFI):

No Government Materials Furnished.

9. SAFETY:

a. This contractor is to follow at a minimum all OHSA rules and procedures. Contractor shall identify any hazards related to the work and provide measures to mitigate risk to include Personal Protective Equipment (PPE) and all other safety related issues dealing with this type of work. Contractor shall provide a safety plan to the Contracting Officer for Approval

b. Contractor will be responsible for providing traffic control in work area since it is adjacent to parking area road.

10. Security:

Contractor shall be responsible for the security of all his equipment and supplies.

11. Travel:

12. Special Material Requirements:

· Concrete mix for approval shall meet the following:

g. 4,500 psi @ 28 days

13. Other Unique Requirements:

· Contractor shall follow a work schedule from 7:00 a.m. to 4:30 p.m. Eastern Time unless otherwise agreed to in writing by the COR or designated representative.

· Contractor or designated representative shall be available to meet with the COR or designated representative as often as necessary to ensure that work is progressing satisfactorily. These meetings will be held at the discretion of the Contracting Officer and a mutual effort will be made to discuss and resolve any problems identified.

14. Quality ASSURANCE Requirement:

The contractor shall develop and maintain a quality program to ensure services are performed in accordance with commonly accepted commercial practices. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. As a minimum the contractor shall develop quality control procedures addressing the critical areas identified in this statement of work. The government will periodically evaluate the contractor’s performance in accordance with the statement of work. All new equipment must conform to the manufacturer’s instructions and guidelines for installation and start-up.

While performing work, the contractor shall ensure all work complies with the latest applicable versions of the following:

· Manufacture recommended installation instructions

15. Acceptance Acceptance check list will include:

· Completion of all work as described in SOW

· Meeting ABAAS

· Workmanship

· Site Cleanliness
