QUALITY ASSURANCE SURVEILLANCE PLAN

1. Objective. This Quality Assurance Surveillance Plan (QASP) serves as the principal basis for assessing overall performance quality. This document will be used by the Government to assess the effectiveness of the Contractor’s technical services. This QASP provides the methodology by which the Contractor's performance will be monitored to determine compliance with established performance objectives and to establish performance benchmarks that ensure a quantifiable basis for measuring effectiveness. The plan is designed so that surveillance is limited to that which is necessary to verify the Contractor is performing technical services satisfactorily and relates directly to performance objectives delineated in the SOW.

2. Government Surveillance. The Contracting Officer will appoint a Contracting Officer’s Representative (COR) and may appoint a Technical Monitor to function as the technical representative for the Contracting Officer on this contract; and her/his authority will be limited to administering specific technical aspects of the contract. The COR and/or Technical Monitor will not provide direction that is outside the scope of responsibilities delineated under this contract and will defer any conditional interpretations to the Contracting Officer. The COR/Technical Monitor will:

0. Maintain a detailed knowledge of the technical requirements of the contract;

0. Document Contractor performance in accordance with the QASP;

0. Identify and immediately forward notifications of deficient, or non-compliant performance to the Contracting Officer;

0. Approve priorities of support, resources, and associated schedules.

3. Surveillance Methods. Surveillance of Contractor performance is the method used by the Government to determine whether the contractor is effectively and efficiently complying with all terms and conditions of the contract. The functional expertise of the COR/Technical Monitor plays a critical role in adequately evaluating contractor performance.

4. Performance Objectives. This performance-based service contract incorporates the following performance objectives: (1) delivery of a full-time experienced technician to support day to day operations of the Combat Feeding Division’s (CFD) Food Pilot Plant and Navy Laboratory. Research and development of fresh, frozen, and shelf-stable ration components requires a clean, sanitary workplace to effectively and safely conduct mission critical work and reliable operation of mission-unique equipment is essential to meeting program objectives. These objectives will be used to measure contractor’s employee’s performance in the General and Specific Requirements, as set forth in the SOW. (2) adherence to contract schedule, milestone, and delivery requirements, and (3) efficient and effective cost control.

General Requirements. The contractor's primary focus shall be to:

(1) Ensure food processing assets in CFD’s Pilot Plant and Navy Lab production areas are clean and sanitary and in top working condition on a daily basis.

(2) Assist food technologists with setup and breakdown of food processing equipment and product development workstations.

(3) Breakdown, clean and reassemble food processing equipment in accordance with equipment manuals and standard operating procedures to ensure proper sanitation, functionality and aesthetics are maintained.

Specific Requirements: The contractor shall:

(1) Assist equipment specialists with daily and/or weekly maintenance schedule for mission critical equipment assets: environmental chambers, refrigeration, freezer, HVAC systems, high pressure steam and compressed air lines.

(2) Compile/generate inventory lists for pilot plant chemicals, paper goods and various consumables and maintain compliance with safety data sheet (SDS) requirements and supply inventory lists to lab manager in a timely manner.

(3) Monitor temperature-controlled storage boxes on a daily basis, troubleshoot problems and report all issues/concerns to identified equipment specialist and lab manager in a timely manner.

(4) Check eye wash and hazard waste stations once per week. Report deficiencies to lab manager.

(5) Assist with setup, operation and breakdown of food processing and packaging equipment and workstation preparation in the batching, filling and sealing operations of ration prototypes and food items in flexible pre-formed pouches (ambient and vacuum packaging) and trays.

(6) Have experience operating food production equipment to include (but not limiting) single and co-extruding bar formers, piston and auger fillers, transport pumps, steam-jacketed kettles, mixers, dough sheeters, grinders, comminutors and well versed in set-up/break-down and sanitation/maintenance of all equipment.

(7) Have a working knowledge of pipe fitting and basic electrical wiring.

(8) Assist as needed with maintenance and sanitation needs within production, processing and storage areas.

(9) Maintain a valid food handler’s certification from an accredited institution for the length of service (i.e., Servsafe).

It is the contractor’s responsibility to employ the necessary resources to ensure accomplishment of these objectives. The Government’s assessment of the contractor’s performance in achieving these objectives will utilize the standards, acceptable quality levels, and surveillance methods described in the Performance Requirements Summary matrix set forth below.

The contractor will be notified, in writing, of the Government’s determination of its performance level for each performance objective, including all instances where the contractor failed to meet the acceptable quality level.

Adherence to Schedule

Contract requirements met with few minor and no significant problems encountered.

Performance meets all technical and functional requirements, and is highly responsive to any changes in technical focus of the project.

Personnel qualifications, assessments, evaluations, analyses, recommendations, and related input are thorough, reliable, highly relevant to contract requirements, and consist of substantial depth and breadth of subject matter expertise.

Contractor technical performance meets all contract requirements.

Problems that are encountered are minor and resolved in a satisfactory manner.

Add specifics as they apply to deliverable (e.g. Technical report(s) will provide details of both the theoretical input and the resulting data; establishing a clear link between the two.)

Routine Assessment of Workforce Qualifications and Availability of Resources

Insert specifics as they apply to this deliverable: (e.g., Frequent (weekly? Bi-weekly? Ad hoc basis?) conference calls to discuss progress

Quarterly meetings to discuss progress

Frequent, Ad hoc review of work by lab manager.

Assignment of performance rating for QUALITY criteria:

Excellent Performance meets all and exceeds standard compliance of many CONTRACT requirements. Insert specifics as they apply to this deliverable: (e.g., The contractor is able to perform more simulations than initially indicated. Reporting of data to US Army NSRDEC is frequent and with sufficient explanation.)] Performance delivered with no required re-performance/rework at least 95% of time. Problems that are encountered are minor and resolved in a highly effective manner.

Acceptable Performance and deliverables meet all contract requirements. Insert specifics as they apply to this deliverable: (e.g., The contractors are able to perform the amount of simulations indicated in the proposal. Reporting of data to US Army NSRDEC is frequent and with sufficient explanation.)] Performance delivered with no reperformance/rework at least 90% of Time. Problems that are encountered are minor and resolved in a satisfactory manner.

Unacceptable Many contract requirements not met.

Insert specifics as they apply to this deliverable: (e.g., The contractors are not able to perform the amount of simulations indicated in the proposal. Reporting of data to US Army NSRDEC is infrequent and without sufficient explanation.)] Numerous re-performances/reworks required. Substantial problems were encountered and were resolved in a less than satisfactory manner.

Adherence to Milestone Schedule
