STATEMENT OF WORK LIBRARY Online Web Based Software- VISUAL DX

DWIGHT DAVID EISENHOWER ARMY MEDICAL CENTER

FORT GORDON, GEORGIA 30905-5650

1.1 General. This requirement is for an Institutional Site License to VisualDx for the Dwight D. Eisenhower Army Medical Center (DDEAMC), Fort Gordon, GA.

1.2 Background: By providing detailed comparative images relevant to a given patient’s differential diagnosis, VisualDx has been shown to increase diagnostic accuracy of skin conditions by as much as 34 percent when used by primary care physicians. Statistical data of its usage gathered over the 2022 Fiscal Year calendar year (8,515 diagnoses reviewed, 72,278 images retrieved, 2015 clinical inquiries) indicate that DDEAMC clinicians, instructors, and students greatly value it as a useful diagnostic and learning resource.

1.3 Objectives: The procurement of VisualDx will increase the probability of initial diagnostic accuracy and appropriate treatment for hundreds of conditions, which in turn will help improve patient outcomes and reduce healthcare costs due to misdiagnoses. VisualDx will also serve as a helpful education resource for various GME curricula, and it will provide an opportunity for users to earn AMA PRA Category 1 CME credits.

1.4 Scope: The VisualDx Institutional License Agreement shall provide 24/7 institution-wide access to its web-based, point-of-care visual diagnostic, clinical support system via IP authentication and intranet portals, and via mobile access for registered users via Apple and Android devices. Content additions, updates, and other system enhancements will be activated as they become available

1.5 Period of Performance: The period of performance shall be for one (1) Base Year of 12 months and four (4) 12-month option years. The Period of Performance reads as follows:

Option Year 229 September 2025 - 28 September 2026
Option Year 329 September 2026 - 28 September 2027 Option Year 429 September 2027 - 28 September 2028

1.6 GENERAL INFORMATION

1.6.1 Training: Unlimited online training sessions will be provided. Train-the-Trainer sessions will be provided upon request. One on-site training session per licensed year shall be provided upon request.

1.6.2 Usage Statistics: Statistics measuring both Web and Mobile monthly usage will be provided upon request. Statistics must provide the following kinds of usage information: Number of images viewed per month, Diagnoses count by month, Clinical inquiries per month, A listing of the most commonly viewed diagnosis per month and the Percentage of access coming each month from Web access versus mobile access.

1.6.3 Support: Due to the 24-hour health-care mission of DDEAMC, online technical support shall be available 24 hours a day, 7 days a week. Toll-free telephone technical service and customer service report shall be available Monday through Friday from 0830 to 0430 Eastern Time.

1.6.4 Performance Measurements

How measured: The DDEAMC Health Sciences Library will track responsiveness to its requests for technical support by tracking the turnaround time between both: the initial request for support and the acknowledgement of the request by VisualDx staff, and the time of the VisualDx acknowledgement and the resolution of the issue that generated the request.

1.6.5 Quality Assurance: The government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rates.

1.6.6 Security Requirements: Contractor personnel performing work under this contract must become familiar with all security requirements at time of the proposal submission and must maintain the level of security required for the life of the contract.

1.6.6.1 All contractor employees with access to a government information system must be registered in the ATCTS (Army Training Certification Tracking System) at commencement of services and must successfully complete the DoD Information Assurance Awareness training prior to access to the information system and then annually thereafter.

1.6.6.2 All contractors performing work on site shall report to the Security office and obtain vendor credentials for the duration of the work. Contractors shall wear their badge credentials at all times while on site. The contractor shall comply with applicable installation, facility, and area commander installation and facility access and local security policies and procedures (provided by the Government representative). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by the installation Provost Marshal Office, Director of Emergency Services, or Security Office. The contractor workforce must comply with all personal identity verification requirements as directed by DoD, HQDA, and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

1.6.6.3 The contractor shall become familiar with the local iWATCH program (training standards provided by the requiring activity ATO). This training will be used to inform the contractor of the types of behavior to watch for and to report suspicious activity to the COR.

2. DEFINITIONS:

2.1. Contractor. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

2.1.1. Contracting Officer. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

2.1.2. Contracting Officer’s Representative (COR). An employee of the U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

2.1.3. Quality Assurance. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.

2.1.4. Quality Assurance Surveillance Plan (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.

2.1.5. Quality Control. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.

5. SPECIFIC TASKS:

5.1. Training. The contractor shall provide unlimited online training sessions as requested.

5.1.1 Contractor shall be available to conduct one onsite training session per year throughout the duration of the contract upon request.

5.2. Usage Report. Upon request, the contractor shall provide a usage report measuring web and mobile monthly usage as outlined in 1.6.2 under general information.

5.3. Technical Support. Technical support and assistance shall be made available by the contractor as stated in 1.6.3 under general information
