☐ (b) FAR 13.106-1(b)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item; OR
☐ (c) FAR 8.405-6(a)(1)(i)(A): An urgent and compelling need exists, and following the procedures would result in unacceptable delays; OR
☐ (d) FAR 8.405-6(a)(1)(i)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; OR
☐ (e) FAR 8.405-6(a)(1)(i)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order provided that the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order or BPA must not have been previously issued under sole-source or limited-sources procedures.
5. Reason for Authority Cited: In August 2021 Request for Quotation (RFQ) W91YTZ21Q0204 was issued as an Unrestricted requirement for Scleral Lenses was issued. As a result of the RFQ, two vendors responded, and two Blanket Purchase Agreements W91YTZ21A0016 and W91YTZ21A0017 were awarded. The stock of Scleral Lenses has been vastly depleted, and it is imperative that the Military Treatment Facility (MTF) re-stock the lens in the event patients requiring lens adjustments can be taken care of. Alden Optical provides unique specialized Scleral Contact lenses that are customized to each individual patient’s eye care and vision needs. Alden Optical is also very user friendly as they warrantee their contact lenses and in frequent cases provide adjusted lens proscriptions at no cost to the government. As in frequent cases the patient eye care perceptions change or lenses need additional adjustments, requiring an additional custom lens be manufactured to adjusted specifications within the prescription. These contact lenses also support our Primary Care Residency Program, allowing our Active Duty one-year Residents the experience of providing a full eye exam and contact lens fitting for Soldiers and authorized DOD beneficiaries in need of this specialized contact lenses. A sole source letter has been provided from Alden Optical which states they are the sole source provider of these lenses; PCF location A.02.
6. Actions to Increase Competition: There are no practical alternatives available to increase competition for this requirement, based on the reasons stated in paragraph 5 above.
Sources sought will be posted on the SAM.gov website to increase competition for future requirements.
7. Market Survey: Market Research conducted in accordance with FAR Part 10. I researched by using the General NAICS code 339115 and found no small business contractors having the capability to provide these customized contact lenses or that the listed materials under this code specifically addressed individual customization. Only stated “contact lens manufacturing” nor did it specify Scleral lenses that we are currently using. On 31 January 2024 an on-line search was conducted on
