About this file
This redacted justification and approval document outlines a sole source award for the development of an immersive virtual reality training platform. The U.S. Army Medical Research Acquisition Activity intends to award a firm-fixed-price contract to Exonicus, Inc. in September 2020, using fiscal year 2019 Research, Development, Test and Evaluation funds. Exonicus developed the initial trauma simulator for the Department of Clinical Investigation at Madigan Army Medical Center under a previous award, and holds proprietary data and code necessary to further enhance the platform for this follow-on phase. No other sources were found during market research and an RFI that could provide the required consistency with the existing platform and proprietary data.

FAR Subpart 13.5 Simplified Procedures for Certain Commercial Items – Sole Source Justification and Approval
1. Contracting Activity: US Army Medical Research Acquisition Activity (USAMRAA), Service Branch 1,820 Chandler Street, Fort Detrick, MD 21702
2. Description of Action: Department of Clinical Investigation, Madigan Army Medical Center requests a firm-fixed-price contract with Exonicus, Inc. The Government projects award for September 2020 and Fiscal Year 2019, Research, Development, Test & Evaluation (RDT&E) funds will be used.
An acquisition plan is not required because this procurement does not meet the threshold at Defense Federal Acquisition Regulation Supplement 207.103(d)(i)(B).
3. Description of Supplies/Services: The purpose of this effort is the development of a high-fidelity, immersive environment replicating Virtual Reality (VR) training platform to assess the impact on individual clinician and team trauma management performance in a simulated deployed treatment environment. The Madigan Army Medical Center, Emergency Medicine Department team has worked with Exonicus in developing the initial trauma simulator, thus providing unique product knowledge to allow more refined enhancements to meet the objective of this project. Exonicus will create and validate VR replicas of ROLE I, ROLE II environments, ROLE III medical evacuation platforms, and fully integrate complex autonomous trauma management simulation cases into these virtual environments.
a. ROLE I - Environment will include the following elements: field environment construction, victim, immediate lifesaving measure scenarios, disease and non-battle injury prevention and care scenarios, patient location and acquisition (collection) scenarios. These measures include maintaining the airway, stopping bleeding, preventing shock, protecting wounds, immobilizing fractures, and emergency measures.
b. ROLE II - Environment will include the following elements: a large tent environment construction, victim, packed blood products, limited x-ray, and laboratory functions. All objects will match the pre-determined scanned layout.
c. ROLE III - Medivac environment will include the following elements: military vehicle construction, patient, and life-saving scenario development leveraging 3D video captured by the Madigan team.

4. Authority Cited: Simplified Procedures for Certain Commercial Items, 41 U.S.C. 1901 as implemented by FAR Subpart 13.501:
Unusual and compelling urgency Exclusive licensing agreements Industrial mobilization
Other: One responsible source per FAR 13.501(a)(1)(i)
5. Reason for Authority Cited:
(a) MAMC requires a Phase Two for the development of a high-fidelity, immersive environment-replicating VR training platform to assess the impact on individual clinician and team trauma management performance in a simulated deployed treatment environment.
Exonicus developed the phase one simulation baseline. As such holds the propriety data code for further expansion for Phase Two.
Additionally, the company developed a technology platform that provides real-life simulated training which are exact matches to the desires outcomes of this research project much of which was based on the phase one production model. They have already built simulated models for the four most common preventable lethal battlefield injuries.
No other source in the commercial market place has access to this code for further development.
If a new source would be sought the project would have to restart at Phase One creating a duplication of cost, the Government additional cost of $241,359.16. Exonicus is the only contractor that can fulfill this requirement.
(b) Alternatives: The Government looked at three alternatives: suspended operations, in-house personnel/expertise or replace rather than upgrade an existing system and none of them are options to meeting the requirement. The Program Office needs the phase 2 of this project to be conducted and suspended operations is not a possibility.
Madigan does not have any in-house technical personnel with experience to develop the second phase of this project. The only known source to meet the requirement is Exonicus, Inc. Replacing rather than upgrading an existing system is not an option for the second phase of the project.
6. Efforts to Obtain Competition: Contracting Officer shall publish the notices required by FAR 5.201. A Request for Information (RFI) was posted on “Contracting Opportunities” on www.beta.SAM.gov on 2020. responses were received to this RFI by the time it expired on 10 July 2020. A review of the capabilities statements found
Immersive Environment-Replicating VR Training Platform
Version – March 2017 FOR OFFICIAL USE ONLY that none of the responders was acceptable as they did not have the proprietary and the phase one platform developed by Exonicus, Inc.
7. Actions to Increase Competition: Competition of the requirement is not possible since the propose contractor, Exonicus, Inc., who is the only source that can provide consistency with platform further development from the first phase and holds the proprietary data gives the Government confidence that the final product will meet the requirement.
8. Market Research:
Market research was conducted from 24 November 2019 through 26 November 2019 by the requiring activity. Market research included online internet searches to include Ability One, CHESS, One Acquisition Solution for Integrated Services (OASIS) and GSA Acquisition Gateway. The sources in the market research demonstrated that these sources could not provide the services required for this effort in support of MAMC.
USAMRAA did a RFI on beta.SAM for sources sought in support of this requirement.
The results of this RFI was twelve responses were received from industry. The Program Office analyzed each capability statement submitted and found them not to be acceptable as they did not demonstrate the capability with developing the second phase using proprietary and licensed to develop the first phase with Exonicus, Inc. A Notice of Intent was posted beta.SAM.gov on 22 July 2020 and no responses were received. Based on the market research only one source can provide the services, Exonicus, Inc.
9. Interested Sources: See paragraph 6 for the synopsis information to date, no other sources have written to express an interest in this acquisition.
10. Other Facts:
a. Procurement History: The first phase was developed by Exonicus, Anatomy Next through another Government award awarded on 20 August 2018: contract#
a. Procurement History:
(1) Contract numbers and dates of the last several contracts for these same requirements. W911QY18P0189, 20 August 2018 (Phase 1)
(2) The competitive status of these actions. Other than Full and Open
(3) Authority previously cited if less than full and open competition was used. -
(4) If a justification was prepared to support the procurement made before this one, a summary of the contents of paragraph 7 of the justification for that procurement and an explanation of the results. – Prior Sole Source document was not available to this contracting office from the previous contracting office. A search of beta.SAM could not locate the posted copy of the approved justification and approval.

(5) If any prior contract for this requirement was accomplished using full and open competition, include a detailed explanation of the changed circumstances causing this action to now limit the sources. Not Applicable
(6) An explanation of any unusual patterns that may be revealed by the history, e.g., several consecutive, urgent buys. Not Applicable
(7) If a justification was prepared to support the procurement made before this one, briefly describe the circumstances justifying the procurement and whether there have been any significant changes. Information from the previous justification is not available as noted in 4 above. This justification is to take the Phase 1 effort under the previous action to Phase 2. The developed Phase 1 VR has proprietary data developed by the contractor that no other source has access in the development for Phase 2.
