Statement of Work / Performance Work Statement

Procurement Request #: 000HCCHD-2026-98161 Title: Coal Workers' Health Surveillance Program - RAMAKRISHNAN

Period of performance of this contract is: 9/1/2026 to 8/31/2027 Option Year 1: 9/1/2027 to 8/31/2028 Option Year 2: 9/1/2028 to8/31/2029 Option Year 3: 9/1/2029 to 8/31/2030 Option Year 4: 9/1/2030 to 8/31/2031

SECTION 1 – BACKGROUND

The Coal Workers' Health Surveillance Program (CWHSP) was initially established in 1970 by the Federal Coal Mine Health and Safety Act of 1969. In accordance with the Mine Safety and Health Administration’s recently published rule on respirable coal mine dust exposure, the CWHSP provides U.S. coal miners with the opportunity to undergo health screening for respiratory disease with respiratory symptom questionnaires, chest radiography, and spirometry (a type of lung function test) at first entry into coal mining and at intervals thereafter throughout their coal mining careers. Medical facilities must be approved by NIOSH for participation in CWHSP before they can provide services to coal miners under the program.

SECTION 2 – PURPOSE/OBJECTIVE

The primary objective of the CWHSP is to screen miners for coal workers’ pneumoconiosis (CWP). The vendor will perform required classifications of chest radiographs taken for all components of the CWHSP (includes Enhanced, Expanded, and Special Studies). These chest radiographs must be interpreted by physicians/radiologists who are certified by NIOSH as being proficient in the use of the International Labour Office (ILO) system for classifying radiographs of the pneumoconioses (B Readers). Classifications for electronic images (analog no longer used) will be submitted electronically using NIOSH’s ILO Classification form. Digital radiographs will be loaded in the Picture Archiving Communications System (PACS) for the vendor to retrieve, classify, and return – all electronically.

SECTION 3 – SCOPE OF WORK

B Readers are used in the medical field to classify chest radiographic images of workers participating in health surveillance due to jobs involving exposure to mineral dusts such as coal mine dust, crystalline silica and asbestos. B Readers also perform classifications in a range of other settings, including research, legal, and evaluation for benefits.

Vendor agrees to classify all radiographs (both analog and digital) sent to them not to exceed 2,000 radiographs for the period of performance for $27,500. This figure is based on past history regarding the number of radiographs received, and also the anticipated increase from the additional components of the CWHSP. A two-week turnaround time is required.

CWHSP contracted B Readers will have access to personally identifiable information (PII) only to the extent necessary to perform their assigned duties related to the review and classification of radiographic images (coal miners name and date of birth). Access to PII will be limited and governed by applicable federal privacy laws, HHS/CDC policies, and data security requirements. All contracted B Readers are required to safeguard sensitive information and use it solely for authorized program purposes only. B Readers must retain their certification, they must sit and pass the B Reader Examination every 5 years.

B Readers for the CWHSP are selected using the following criteria:

1) Checked "YES" on the Interpreting Physician Certification Document (CDC/NIOSH(M)2.12) under, "Would you be interested in participating as a B Reader for the CWHSP?";

2) Overall score on the most recent B Reader Exam of 75 or above;

https://www.federalregister.gov/articles/2014/05/01/2014-09084/lowering-miners-exposure-to-respirable-coal-mine-dust-including-continuous-personal-dust-monitors

3) Active in reading at least 50 x-rays per month as indicated on the CDC/NIOSH(M)2.12 form .;

4) Small opacity score on the most recent B Reader Exam of 12 or above;

5) Inconsistency index score on the most recent B Reader Exam of 20 or above;

6) Board certification in radiology (RA), pulmonary (PU) or occupational medicine (OM);

7) Holds an academic appointment. In order to achieve confidence and integrity of the classifications provided by the B Readers chosen to participate in the CWHSP, they are required to meet a set of stringent criteria prior to their participation. These criteria include making a higher-than-average score on the B Reader Examination, as well as achieving a designated score on the small opacity and inconsistency indices sections on the Examination. They also must be board certified in their specialty. This physician meets all of the required criteria so we can be confident of continuing a high standard of film classifications and thus preserving the integrity of the Program.
