SECTION B – SUPPLIES OR SERVICES AND PRICE/COSTS
B.1 Services
Contractor shall provide services in accordance with the Statement of Work (Section C).
B.2 Contract Type
This is a Firm Fixed Price purchase order with unit rates; quantities are based on anticipated estimates and the Contractor is Not-to-Exceed the allotted quantities and cost associated with each Contract Line Item Number (CLIN). The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical or professional aspects of services rendered (e.g. professional judgments). There are no materials or travel included in this purchase order.
B.4 Contract Funding
This purchase order is incrementally funded in the amount of $TBD. Additional funding will be added by a purchase order modification. (To be removed upon award if fully funded.) The Contractor is not authorized to exceed the obligated amount on this purchase order.

SECTION C – STATEMENT OF WORK
Re-entry and Sanctions Center (RSC) Group Intervention Services
C.1 Agency Background
C.1.1 The Court Services and Offender Supervision Agency (CSOSA) was established under Section 11232 of the National Capital Revitalization and Self-Government Improvement Act of 1997 to effectuate the reorganization and transition of functions relating to pretrial services, parole, adult probation and offender supervision in the District of Columbia to a Federal Executive Branch agency. CSOSA’s mission is to increase public safety, prevent crime, reduce recidivism, and support the fair administration of justice in close collaboration with the community through the supervision of all probationers, parolees and supervised releases held under the authority of any United States or District of Columbia statute, or any other lawful authority entrusted to the District.
C.1.2 The Re-entry and Sanctions Center (RSC) is located at 1900 Massachusetts Avenue, SE, Washington DC. CSOSA's 102-bed RSC provides high-risk offenders and defendants (hereinafter referred to as residents) with a 28 day (men) and 42 day (women) intensive assessment and reintegration program. The RSC also allows CSOSA to impose prompt and meaningful residential sanctions for residents who have violated the conditions of their release, improving the likelihood of successful supervision.
C.1.3 The RSC Group Intervention Services program will meet the assessment and evaluative needs of female residents with extensive substance abuse histories. The female residents (up to 15) assigned to the RSC participate in a 42-day holistic and multidisciplinary program, during which they do not leave the facility or receive visitors. Treatment readiness and motivation is the focus of each of the interventions offered at the RSC. These interventions are structured to address one or more of the factors identified as particular challenges to a female resident’s successful reentry and long-term recovery from substance abuse. These factors may include psychiatric distress, substance abuse, cognitive impairments, protracted withdrawal, poor attachment/social bonding, and extensive criminality.
C.1.4 Female residents receive counseling, a complete physical, psychological, and behavioral assessment, as well as referral to an inpatient, residential, or daily outpatient substance abuse treatment program. Upon completion of the program, female residents are equipped with the tools needed to prevent relapse, succeed in a treatment modality, improve familial relationships, and modify deviant behaviors. Because an extensive discharge summary is prepared for, and shared with, each female resident, the female resident leaves the RSC program with a better understanding of her relapse triggers and the specific strategies that she can use to counteract them. After being introduced to these concepts, the female resident is more likely to enter treatment with a positive attitude and a commitment to change. In addition, the in-depth RSC assessment is used to develop the female resident's supervision plan so that they are held accountable for engaging in programming that addresses their needs.
C.2 Objectives
C.2.1 The Contractor shall provide all necessary labor, supervision, materials, and supplies to provide female group intervention services at the CSOSA RSC, in accordance with the requirements specified herein.
C.2.2 To effectively address re-entry barriers that are common to all female RSC residents, the Contractor shall facilitate group interventions, which are designed to identify the barriers and equip the female residents with strategies for approaching and overcoming such barriers. Examples of common barriers include anger management, denial, relapse prevention, etc.
C.2.3 The Contractor shall provide the necessary staff and labor to facilitate one or a combination of group, and/or individual therapy sessions, for RSC female residents, and upon request, attend Interdisciplinary Team Meetings (IDTs) to present/discuss resident findings.
C.2.4 The Contractor shall be available to provide on-site group and individual therapy sessions of up to one (1) hour per session, any time, during the hours of 9:00 am – 9:00 pm Monday to Friday. An agreed upon staffing schedule will be developed between CSOSA and the Contractor at the post-award meeting to be held within ten (10) days following award.
C.2.5 The sessions will consist of female residents with extensive substance abuse histories that are either in a re-entry or sanctioned status and have been sent to the RSC for stabilization. Some may also have co-occurring disorders.
C.2.6 The Contractor staff conducting each group/individual session shall arrive at the RSC at least five (5) minutes prior to the scheduled session in order to meet with the RSC Unit Manager, or their designee, for a pre-intervention briefing. At the beginning of each group/individual session the Contractor shall provide the resident(s) with a sign-in sheet to ensure credit for participation in the group sessions.

C.3 Group Trauma and Individual Trauma Therapy Sessions Performance Requirements
C.3.1 The Contractor shall facilitate direct trauma groups and ad hoc individual counseling sessions for female residents and present findings in IDT meetings. The Contractor shall facilitate groups using a cognitive-behavioral evidence-based curriculum that demonstrates effectiveness in achieving the following:
C.3.1.1 Group Trauma Therapy Session.
C.3.1.1.1 This group should focus on the forms of trauma and abuse, the connection between trauma and substance abuse, the impact of trauma, and the process of healing from trauma. Because some of the residents served may suffer from Post-Traumatic Stress Disorder (PTSD), this group session will help residents develop the appropriate coping skills that address the traumatic events.
C.3.1.1.2 The Contractor shall ensure that each group session has a clearly defined topic, objective, and goals that are designed to assist residents in identifying their barriers to successful reintegration and equip residents with strategies for approaching and overcoming barriers.
C.3.1.2 Individual Trauma Therapy Session. Individual Therapy Services shall be requested by the COR for residents who may benefit from individual therapy in addition to group therapy, to improve their capacity to respond positively to group therapy.
C.4 Group Violence and Victimization Therapy Session Performance Requirement. The Contractor shall facilitate violence and victimization groups using a cognitive-behavioral evidence-based curriculum that possesses demonstrated effectiveness in focusing on the forms of abuse and assault (physical, sexual, and mental) that women experience, how substance abuse leads to a woman’s victimization, and identifying the perpetrators of these acts. It will also address the impact of violence and victimization on the woman’s recovery process.
C.5 Group Self-Awareness/Character Building Therapy Session Performance Requirement. The Contractor shall facilitate self-awareness/character building groups. The Contractor shall facilitate groups using a cognitive-behavioral evidence-based curriculum that possesses demonstrated effectiveness in focusing on the factors that diminish a woman’s self-worth and image. This group will help women understand how interactions with others, and the messages received from others, have shaped their self-concept and affected their self-esteem.
The group will use cognitive restructuring to help women increase their self-awareness, improve their self-esteem, and shape the choices a woman makes throughout her recovery. The group will also focus on helping women to identify unhealthy relationships and co-dependent behaviors, grieve and reconcile loss, and create supportive and mutually satisfying relationships
C.6 Group Psychotherapy Session Performance Requirement.
C.6.1 Psychotherapy Group Sessions. The Contractor shall facilitate direct psychotherapy groups and ad hoc individual counseling sessions to residents, and present findings in IDT meetings. The Contractor shall facilitate groups using a cognitive-behavioral evidence-based curriculum that possesses demonstrated effectiveness in using psychological techniques to encourage women to communicate their innermost conflicts and problems. Through the disclosure process the woman’s mental and emotional disorders will be addressed, the symptoms will be relieved, and social functioning should improve.
C.6.2 The Contractor shall ensure that each group session has a clearly defined topic, objective, and goals that are designed to assist female residents in identifying their barriers to successful reintegration and equip residents with strategies for approaching and overcoming barriers.
C.7 Presentation of Findings in IDT Meetings. IDT meetings, consisting of CSOSA supervision and intervention staff meeting to discuss a resident’s intervention plan or her progress in relation to an established intervention plan, are held once per week, and are one (1) to two (2) hours in duration. The Contractor can expect to attend each weekly meeting to present/discuss, at a minimum, the findings of individual and/or group interventions for the Trauma group. The Contractor may periodically, one (1) or two (2) times per month, also be invited to present/discuss the findings of individual and/or group interventions for the Violence and Victimization, Self-Awareness/Character Building, and Psychotherapy sessions.
C.8 Reporting Requirements. At the conclusion of each group/individual session, the Contractor shall enter a narrative summary relevant to the resident’s participation in the CSOSA Supervision Management Automated Record Tracking (SMART) case management system. At a minimum, the summary shall include the group topic, objectives and each participant’s responsiveness to the session. The Contractor must enter the narrative summary in the CSOSA SMART case management system no later than one (1) business day following the delivery of the group and/or individual therapy session. The Contractor shall not remove any progress notes, (handwritten, recorded, or otherwise) from the RSC which contain the personally identifiable information (PII) of any group participants/attendees.
C.8.1 At a minimum, the following information must be included in the SMART case management system:
• Resident Identification Information (Name, Police Department Identification (PDID) Number, etc.);
• Group Topic, Session Date, and Time;
• List of the group participants/attendees
• Summary of the Group/Individual Experience; and
• Narrative Summary of any significant information relevant to the resident’s participation in the group to include clinical observations of issues, steps necessary to stabilize participant, and encourage progress toward constructive self-expression.
C.8.2 The group/individual sign-in sheets shall be submitted to the designated staff person at the end of each group/individual session.
C.9 Coverage
C.9.1 The Contractor shall facilitate the groups in accordance with the agreed upon schedule with fully qualified facilitators. In order to ensure that group facilitation takes place in accordance with the agreed upon schedule, the Contractor is strongly encouraged to not only have a primary facilitator for each group, but one or more qualified individuals available who can facilitate in the absence of the primary facilitator. As such, the Contractor may submit security clearance paperwork for “back up” facilitators to ensure no lack of coverage occurs due to the unavailability of a primary facilitator.
C.9.3 If the Contractor is unable to provide the requisite schedule coverage, the Contractor shall notify the COR via email and telephone at least 24 hours in advance.
C.10 Contractor Personnel Qualifications
C.10.1 All of the Contractor’s personnel shall:
C.10.1.1 Be a citizen of the United States of America, or lawfully permitted to reside in the United States and possess a valid work permit.
C.10.1.2 Be at least 21 years of age and able to withstand the physical demands of the job and be capable of responding to emergency situations.
C.10.1.3 Be able to clear the Government’s background investigation.
C.10.1.4 Be able to speak, read, and write the English language fluently.
C.10.1.5 Be a graduate of an accredited institution.
C.10.1.6 Have the requisite educational and professional experience specified for the positions and services that they will provide.
C.10.1.7 Follow federal, state, county and local requirements for licensing, certification, and credentialing and all other applicable regulations regarding the provision of services described in this purchase order.
C.10.1.8 Be deemed drug free by the Contractor before being hired to perform services under this purchase order.
C.10.2 Group and Individual Trauma Therapy Personnel Qualifications. This group must be led by a Licensed Psychologist or Licensed Independent Social Worker (LICSW).
C.10.2.1 Psychologist. The Psychologist shall possess:
C.10.2.1.1 A PhD. or Psy.D in clinical or counseling psychology from a graduate program in psychology accredited by the American Psychological Association, or a Masters of Social Work (MSW) from a graduate program accredited by the Council on Social Work Education.
C.10.2.1.2 A full, current, and unrestricted license to practice clinical or counseling psychology at the doctoral level in the District of Columbia or a state, territory or Commonwealth of the United States.
C.10.2.1.3 At least three (3) years of verifiable work experience as a licensed clinical or counseling psychologist specializing in providing trauma therapy.
C.10.2.2 Licensed Independent Social Worker. The LICSW shall possess:
C.10.2.2.1 A MSW from a graduate program accredited by the Council on Social Work Education.
C.10.2.2.2 A full, current and unrestricted licensed to practice as an LICSW or as a Licensed Professional Counselor (LPC) in the District of Columbia or a state, territory or Commonwealth of the United States.
C.10.2.2.3 At least three (3) years of verifiable work experience as a LICSW or LPC specializing in providing trauma therapy.
C.10.3 Group Psychotherapy Personnel Qualifications. This group must be led by a Licensed Psychologist or Licensed Clinical Social Worker (LICSW).
C.10.3.1 Psychologist. The Psychologist shall possess:
C.10.3.1.1 A PhD. or Psy.D in clinical or counseling psychology from a graduate program in psychology accredited by the American Psychological Association.
C.10.3.1.2 A full, current, and unrestricted license to practice clinical or counseling psychology at the doctoral level in the District of Columbia or a state, territory or Commonwealth of the United States.
C.10.3.1.3 At least three (3) years of verifiable work experience as a licensed clinical or counseling psychologist specializing in providing psychotherapy services.
C.10.3.2 Licensed Clinical Social Worker. The LICSW shall possess:
C.10.3.2.1 A MSW from a graduate program accredited by the Council on Social
C.10.3.2.2 A full, current and unrestricted licensed to practice as an LICSW in the District of Columbia or a state, territory or Commonwealth of the United States; and
C.10.3.2.3 At least three (3) years of verifiable work experience as a LICSW specializing in providing psychotherapy services.
C.10.4 Group Violence and Victimization Therapy Personnel Qualifications. This group must be led by a Licensed Psychologist, LICSW or LPC.
C.10.4.1 Psychologist. The Psychologist shall possess:
C.10.4.1.1 A PhD. or Psy.D in clinical or counseling psychology from a graduate program in psychology accredited by the American Psychological Association.
C.10.4.1.2 A full, current, and unrestricted license to practice clinical or counseling psychology at the doctoral level in the District of Columbia or a state, territory or Commonwealth of the United States.
C.10.4.1.3 At least three (3) years of verifiable work experience as a licensed clinical or counseling psychologist specializing in providing violence and victimization services.
C.10.4.2 Social Worker. The Social Worker shall possess:
C.10.4.2.1 A MSW from a graduate program accredited by the Council on Social
C.10.4.2.2 A full, current and unrestricted licensed to practice as an LICSW or as a LPC in the District of Columbia or a state, territory or Commonwealth of the United States.
C.10.4.2.3 At least three (3) years of verifiable work experience as a LICSW or LPC specializing in providing violence and victimization services.
C.10.5 Group Self-Awareness/Character Building Therapy Personnel Qualifications.
This group must be led by a Licensed Graduate Social Worker (LGSW), LPC or Substance Abuse Counselor.
C.10.5.1 LGSW/LPC. The LGSW/LPC shall possess:
C.10.5.1.1 A MSW from a graduate program accredited by the Council on Social
C.10.5.1.2 A full, current and unrestricted licensed to practice as a LGSW or as a LPC in the District of Columbia or a state, territory or Commonwealth of the United States.
C.10.5.1.3. At least three (3) years of verifiable work experience as a LGSW or LPC who worked with female offenders/defendants in an adult residential treatment, community corrections, re-entry, or correctional facility.
C.10.5.2 Substance Abuse Counselor. The Substance Abuse Counselor shall possess:
C.10.5.2.1 At least five (5) calendar years of verifiable experience as a certified and/or licensed substance abuse counselor who facilitated character building/self-awareness groups for female offenders/defendants.
C.10.5.2.2 A minimum of five (5) years of verifiable work experience as a certified or licensed substance abuse counselor who worked with female offenders/defendants in an adult residential treatment, community corrections, re-entry, or correctional facility.
C.10.5.2.3 A full, current, and unrestricted license, or certification, to provide substance abuse counseling services in the District of Columbia or a State, Territory or Commonwealth of the United States.
C.11 Licensure and Certification
C.11.1 Within ten (10) business days following purchase order award, the Contractor shall provide the COR with copies of current licensure, certification and comprehensive credentials of all personnel who will be assigned to perform services under this purchase order.
C.11.2 Thereafter, the Contractor and Contractor’s staff shall maintain current licensure and certification required to perform the services outlined within the purchase order. Upon request from the COR, the Contractor shall provide copies of the required licenses and certifications. The failure to maintain current licensure and/or certification shall result in the immediate removal of unqualified personnel, and may result in suspension of Contractor services until appropriate licensure or certification is obtained.
C.12 Communicable Disease/TB Testing. The Contractor shall ensure that all purchase order staff assigned to perform under this purchase order are tested annually for Tuberculosis using the Purified Protein Derivative (PPD) tuberculin test. In the event that any Contractor staff are not able to take a tuberculin test, the Contractor shall submit annual chest x-ray results demonstrating asymptomatic clearance by a medical doctor. Results of all tests/x-rays shall be provided to CSOSA within ten (10) business days and annually thereafter. CSOSA strongly encourages the Contractor to require its staff to maintain current immunizations (i.e., influenza, hepatitis, tetanus, etc.). The Contractor shall remove from performance under this purchase order any staff members who are found or suspected to be suffering from a communicable disease, disorder, or respiratory problem, and who, after taking/exercising universal precautions set forth by the Centers for Disease control or other reputable health care organizations, pose a risk to the health and safety of residents or others in the work place.
C.13 Drug Testing. Due to the mission of the CSOSA RSC, the Contractor must comply with all conditions of the Drug-Free Workplace Act of 1988.
C.13.1 Drug Testing. All individuals assigned to this purchase order who facilitate group intervention services will be subject to pre-employment and random drug testing by the Contractor. The Contractor shall conduct the testing within fourteen (14) calendar days prior to the individual staff member beginning work under this purchase order. The Contractor will supply CSOSA with a copy of test results. Drug test collections will be conducted by urinalysis and will be in accordance with HHS Mandatory Guidelines for Workplace Drug Testing Programs published in Federal Register on April 13, 2004 (69 FR19644) and the HHS Urine Specimen Collection Handbook, effective date November 1, 2004.
C.13.2 Tested Drugs. Drugs that the Contractor shall test for are Marijuana, Cocaine, Amphetamines, Opiates and Phencyclidine (PCP).
C.13.3 Laboratory Testing. The laboratories chosen to perform the drug testing for individuals assigned to this purchase order must be certified by the HHS, and laboratory procedures will be in accordance to HHS Mandatory Guidelines for Federal Workplace Drug Testing Programs.
C.13.4 Refusal to undergo Drug Test. Any Contractor personnel who are assigned to this purchase order who refuse to be tested, attempt to alter or substitute the specimen, fail to appear for testing without an approved deferral, which constitutes refusal to test, or refuse to provide consent to release information of the test results, shall be removed from the purchase order immediately with notification to the COR.
C.14 Compliance with Federal, State and Local Regulations
C.14.1 The Contractor shall ensure that all staff assigned to the RSC adhere to the applicable CSOSA Policies and the RSC operational instructions. Copies of the CSOSA Policies and the RSC operational instructions will be made available to the Contractor upon award.
C.14.2 The Contractor shall provide services in compliance with all federal, state and local laws, regulations and professional standards that relate to the provision of psychological services in a residential facility. The Contractor and Contractor staff shall comply with all the requirements of the Health Insurance Portability and Accountability Act of 1996 as amended (HIPAA) regulations and the requirements of 42 CFR part 2 (Confidentiality of Alcohol and Drug Abuse Patient Records), DC Office Code §§ 7-1201.01 eq. seq (D.C.
Mental Health Information Act), and D.C. Official Code §§ 7-302 and §§ 7-1605 (HIV/AIDS) Cancer Confidentiality.
C.14.3 The Contractor shall also comply with confidentiality and training standards defined by the American Correctional Association (Adult Community Residential Services, 4th edition), the Commission on Accreditation of Rehabilitation Facilities (CARF), and the American Society of Addiction Medicine (ASAM).
C.15 Personnel Records. The Contractor shall maintain documentation that verifies that Contractor personnel have received adequate training to perform the duties for which they are assigned and meet all applicable requirements for certification and/or licensure.
C.16 Emergency Reporting
C.16.1 The Contractor shall immediately notify the COR of any actions, events or deeds that appear to pose a safety concern, or an imminent danger to the RSC residents, contractors, or staff.
C.16.2 The types of events that are deemed an imminent danger are: building/physical plant concerns, discovery of weapons or items that may be used as such, as well as, the resident’s suicidal ideations/ actions, verbal or physical threats, physical altercations, or any other action that suggests that the resident poses a threat to self or others.
C.16.3 In the event the COR is not available, the Contractor shall notify the Unit Manager, Shift Supervisor, or RSC Program Director.
C.17 Contractor Performance Standards. A minimum standard of performance is set forth below to provide guidance for contractors in maintaining suitability to perform services within the RSC.
C.17.1 Responsibilities.
C.17.1.1 The Contractor cannot subcontract any of the purchase order requirements specified herein without the express, written consent of the Contracting Officer.
C.17.1.2 The Contractor shall ensure that all its staff adhere to the Standards of Conduct (set forth below) and meet all applicable health requirements.
C.17.1.3 The Contractor shall also be responsible for taking any and all such disciplinary action as necessary when Contractor staff fail to meet purchase order standards or performance requirements.
C.17.2 Contractor’s and Contractor’s Personnel Standards of Conduct.
C.17.2.1 CSOSA is a law enforcement Agency and therefore requires a degree of public trust and confidence. Although CSOSA contractors and contractor personnel are not employees of the Federal Government, they shall be required follow and comply with these standards of conduct. All Contractor personnel shall follow the minimum standards of conduct set forth in 5 C.F.R. PART 2635.101 and CSOSA Policy Directive PS 1106.
Both documents are incorporated into this purchase order by reference and shall be made available to the Contractor upon request.
C.17.2.2 In 5 C.F.R. PART 2635.101, there is guidance on developing a standard for the Contractor and Contractor employees. Also, a minimum code of conduct is set forth below to provide guidance in achieving a greater individual standard. The Contractor and Contractor personnel assigned to this purchase order shall:
1. Be courteous and demonstrate good manners toward all CSOSA employees, RSC residents, and the public.
2. Maintain a respectful and helpful attitude in all endeavors.
3. Maintain a neat, clean, and professional appearance.
4. Report to work physically fit and mentally alert. If feeling otherwise, make appropriate notification to the Contractor and request necessary relief or instructions.
5. Report any circumstances that may adversely affect performance on a particular assignment to the Contractor, prior to the assignment.
6. If a Contractor employee should be detained or become aware of being under investigation, by any federal, state or local agency, for any legal or ethical violation, the Contractor employee must report this to the Contractor, no later than the next working day. The Contractor shall immediately report the incident to the
COR.
7. Not engage in discussions concerning CSOSA internal matters, policies, grievances, or personalities
8. Not engage in discussions of financial, personal, or family matters with CSOSA employees, family members, the public, or any known associate of the above.
9. Not entertain, socialize, or enter into business arrangements with, nor give legal advice or grant special favors to, CSOSA employees, RSC residents, family members, and friends of the above.
10. Except in an officially authorized capacity, not possess narcotics, dangerous drugs controlled substances, or marijuana, either on or off duty.
11. Abstain from the consumption and possession of alcoholic beverages while on duty.
12. Not report for duty or work under the influence of intoxicants or drugs.
13. Not report for duty or work under any condition that impairs the ability to perform as expected.
14. Not accept or solicit gifts, favors, or bribes in connection with official duties.
15. Not allow CSOSA employees, RSC residents, or their family members and friends into their home or living quarters.
16. Not visit the RSC during non-duty hours or allow family members and friends to visit the RSC or other operational areas. An exception must be requested in writing and approved by the COR.
17. Not gamble or enter into games of chance with CSOSA employees or RSC residents. Not gamble or unlawfully bet or promote gambling on CSOSA owned or leased premises.
18. Not disclose any official information, except to the COR or an authorized senior official, or make any news or press releases related to this purchase order. Press inquiries must be brought to the attention of the COR. This does not prohibit protected whistle blowing activities or protected union activities.
19. Refrain from discussions concerning duty assignment, particularly manpower, security precautions, or procedures, except with those persons having a need to know.
20. Comply with applicable laws both on and off duty.
21. Not knowingly give false or misleading statements or conceal material facts in connection with employment, promotion, travel voucher, any record, investigation, or other proper proceeding.
22. Not discriminate against or sexually harass CSOSA employees, RSC residents, members of the public, the judiciary, or engage in any prohibited personnel practices.
23. Ensure that financial obligations are met.
24. Not use official identification for other than official purposes.
25. Abide by all ethical standards of CSOSA applicable to agency employees regarding conflict of interest, outside activities, gifts and use of federal property.
26. Not accept or solicit gifts, favors, or bribes in connection with official duties.
27. Not bid on, or purchase in any manner, directly or through an agent, any property being offered for sale by CSOSA or by others serving on behalf of CSOSA.
28. Refrain from any activity that would adversely affect the reputation of CSOSA.
29. Avoid personal and business associations with persons known to be convicted felons or persons known to be connected with criminal activities. This does not apply to immediate family members so long as notification is made to the COR.
It is the responsibility of the Contractor to report any relationship (including friendships, family relationships (i.e. relatives), spouses/cohabitants, parental relationships, etc.) with a known offender/defendant immediately to the
COR.
30. Avoid any criminal, infamous, dishonest, immoral, or notoriously disgraceful conduct, to include the habitual use of intoxicants or non-prescription drugs.
31. Not operate a CSOSA vehicle, or any other vehicle, while on CSOSA business, in an improper manner or under the influence of intoxicants or drugs.
32. Not misuse official authority, credentials, communications equipment, or weapon(s).
33. Not make statements about agency employees or officials, or other contractors or contract personnel with knowledge of the falseness of the statement or with reckless disregard of the truth.
34. Report violations of prescribed rules, regulations and any violations of statute or law to the Contractor and/or the COR.
35. Not violate security procedures or regulations.
36. Not close or desert the group/client meeting prior to scheduled closure unless directed to do so, or permission is received from the COR, RSC Deputy Director, or RSC Director.
37. Always perform assignments in accordance with prescribed regulations to the best of personal ability and in accordance with safe and secure working procedures and practices.
38. On occasion, the Contractor or Contractor personnel may be required to follow specific directions or guidance from the COR, the RSC Deputy Director, RSC Director, or other CSOSA staff personnel. The Contractor or Contractor personnel shall, to the best of their ability, carry out the direction or guidance given. Any specific direction or guidance receive that the Contractor considers outside the scope of this purchase order shall be reported to the Contracting Officer immediately.
39. Do not possess, use, lose, damage, or otherwise take CSOSA property or the property of others without authorization of the COR.
40. Conduct only official business on CSOSA property.
41. Refrain from neglecting duties. This includes sleeping on duty, unreasonable delays or failures to carry out assigned tasks, conducting personal affairs during duty hours, and refusing to render assistance or cooperate in upholding the integrity of the therapeutic program services.
42. Refrain from use of abusive or offensive language, quarreling, intimidation by words, actions, fighting and participation in disruptive activities that interfere with normal and efficient CSOSA operations.
43. Answer and make only official business calls on CSOSA telephones.
C.18 Other Services. The Contractor shall provide only the services in the amount and quantity identified in this purchase order. If services are required that are not in the scope of this purchase order, the contractor shall obtain prior approval from the Contracting Officer before providing any services.
C.19 Work Restrictions. The Contractor shall maintain satisfactory standards of staff competence, conduct, appearance, and integrity, and shall take such disciplinary action against its staff as may be necessary. The Government reserves the right to direct the Contractor to remove a staff member from the work site for failure to comply with the performance standards defined in section C.16.2 of this purchase order and the Contractor shall initiate immediate action to replace that staff member to maintain continuity of services at no additional cost to CSOSA.
All backup or replacement staff must be approved by CSOSA.
C.20 Re-entry and Sanctions Center Security.
C.20.1 The Contractor agrees to adhere to all regulations prescribed by CSOSA with regard to the safety and security of the RSC facility, staff, and the residents. The RSC Safety and Security policy will be provided to the Contractor upon purchase order award.
C.20.2 The Government requires all Contractor personnel to sign in and out of the RSC for security reasons. Additionally, the Contractor and Contractor personnel shall submit to a background investigation and drug testing, and attend a training orientation program, prior to providing services in the RSC. All Contractor personnel that provide services within the confines of the RSC shall have a complete background investigation conducted. The following investigative procedures shall be applied and the appropriate forms completed before an individual is permitted inside the RSC:

Law Enforcement Agency Checks Credit Check Financial Disclosure Forms Vouchering of employers over the past five years Resume/Personal Qualifications OMP -329-A (Authority for Release of Information) Completed Contractor Pre-Employment Form National Agency Check and Inquiries (NACI) check (if applicable); and Urinalysis (if the test is positive the contractor shall be disqualified from performing under the purchase order).
C.20.3 The Contractor and Contractor personnel agrees to complete the required documents and undergo the above listed procedures. Contractor personnel who do not pass the security clearances will be unable to perform services under this purchase order. The final determination and completion of the security investigation procedures will be made at the sole discretion of the Government. All persons are advised that a urinalysis test for the detection of illegal drug usage may be performed. Any Contractor personnel testing positive shall be disqualified from performing under this purchase order. Contractor personnel deemed not suitable will not be granted access to perform services under the purchase order.
C.21 Government Mandatory Training Requirements.
C.21.1 Contractor personnel shall attend Government mandated training. The Government will require Contractor personnel to complete training in a classroom or an online setting.
The training topics may include safety/security, IT Security/Privacy Act, RSC Program Regulations.
C.22.2 The Government estimated need for Contractor attendance in mandatory training is approximately eight (8) hours per year, which may be completed in one session, or may be completed over the course of a number of small sessions.
C.23 Special Language Requirements. The Contractor shall be able to provide bilingual services (Spanish or other languages), as needed, to include services for the hearing impaired through the use of a sign-language interpreter. The Contractor may satisfy this requirement through the use of volunteer interpreters or contracted service providers.
C.24 Government’s Quality Assurance Program (QASP). The Contractor shall provide individual and group intervention services, exactly as specified in this Section C SOW and other applicable Sections of this purchase order. The CSOSA Contracting Officer (CO) or COR, with the support of other CSOSA components, shall conduct scheduled and unscheduled QASP inspections and audits of the Contractor’s performance against the requirements of this Section C and other applicable Sections of this purchase order. The CSOSA CO or COR will provide two
(2) business days advance notification for unscheduled QASP inspections and audits. The Contractor’s performance assessment will be based upon the expectation that the Contractor’s performance will be 100% in compliance with all purchase order requirements. When an inspection or audit determines that the Contractor is deficient and not fulfilling one or more of the purchase order’s performance requirements, the CSOSA CO will issue a corrective action report (CAR), which identifies the deficiency and the Contractor shall develop a plan of action with milestones (POAM) to correct the deficiency in the CAR within the period of time designated by the CO and submit the POAM to the CO for approval. When the deficiency has been corrected, the Contractor shall be inspected or audited to ensure the Contractor’s corrective action is sufficient to prevent reoccurrence of the deficiency.
C.25 Contractor Quality Control (QC) Plan. The Contractor shall develop, and provide to the CO within 30 days of purchase order award, a QC Plan that identifies what actions, processes, procedures, inspections, reviews, and responsibility assignments the Contractor will utilize to ensure that the Contractor’s performance complies with all of the requirements of this purchase order. The CO shall have ten (10) business days to review the QC Plan and provide requested or required changes to the Contractor. The Contractor will then have five (5) business days to incorporate the CO’s required/requested changes and return a final QC Plan to the CO. The Contractor’s QC Plan shall be reviewed and updated as required, but no less than annually.
C.26 Post-Award Meeting. The COR will schedule a post-award meeting between the Contractor and CSOSA within ten (10) business days following purchase order award. The purpose of this meeting will be to review the performance requirements of the purchase order, agree on the roles and responsibilities of CSOSA and the Contractor under the purchase order, and discuss any other issues proposed by either party. The Contractor shall prepare a meeting report that details who was in attendance at the meeting, what discussions took place, and what agreements were reached. A copy of the report shall be provided to the COR within ten (10) business days following the meeting.

SECTION D - PACKAGING AND MARKING
D.1 Deliverables
Deliverables provided electronically shall be formatted with read/write capability using Microsoft WORD 2010 format, or other than text files any other application compatible with Microsoft Office Suite applications. Font type of Times New Roman with a font size of 12 shall be used throughout the document. The Contractor shall minimize additional formatting as much as possible (e.g. avoid use of columns). Avoid using any design/layout elements.

SECTION E - INSPECTION AND ACCEPTANCE
E.1 Inspection of services furnished hereunder shall be performed by the CO and/or the COR, in accordance with FAR 52.212-4(a).

SECTION F - DELIVERIES AND PERFORMANCE
F.1 Period of Performance
The period of performance of this purchase order is as follows:

F.2 Place of Performance
The Contractor will perform the work under this purchase order at the Re-entry and Sanctions Center, 1900 Massachusetts Avenue, SE, Building 17, Washington, DC 20003.
F.3 Observance of Legal Holidays
F.3.1 CSOSA personnel observe the following days as holidays:

F.3.2 If a holiday falls on Sunday, the following Monday will be observed as the legal holiday.
F.3.3 When a holiday falls on a Saturday, the preceding Friday is observed as a legal holiday by U.S. Government agencies.
F.3.4 In addition to the days designated as holidays, the Government observes the following days:
(1) Any other day designated by Federal Statute
(2) Any other day designated by Executive Order
(3) Any other day designated by the President’s Proclamation
F.3.5 It is understood and agreed between the Government and the Contractor that observance of such days by Government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation except as set forth within the purchase order.
F.4 Deviations
Any deviation from the delivery or performance requirements by the Contractor may be allowed only upon advance written agreement from the CO.
F.5 Notice to Government of Delays
In the event the Contractor encounters, or anticipates encountering, difficulty in meeting performance requirements, or in complying with the purchase order delivery schedule or completion date, or whether the Contractor has knowledge that any actual or potential situation is delaying, or threatens to delay, the timely performance of this purchase order, the Contractor shall immediately, within one (1) business day, notify the CO and the COR, in writing, giving pertinent details; provided, however, that this notification shall be informational only in character, and that this provision shall not be construed as a waiver by CSOSA of any delivery schedule or date, or any rights or remedies provided by law or under this purchase order.
F.6 Travel. No travel reimbursement is allowed under this purchase order.

G.1.2 The COR is responsible for the technical aspects of the purchase order and serves as the liaison with the Contractor. The COR is responsible for the final inspection and acceptance of all work performed and such other responsibilities as may be specified in this purchase order.
G.1.3 The COR is not authorized to make any commitments or otherwise obligate the Government or authorize any changes which affect the purchase order price, terms or conditions.
