ATTACHMENT J-6
ADDITIONAL SERVICES
1. Expenses Incurred For Services Included Below. The Contractor is responsible for monitoring the expenses incurred under CLIN 0006, Additional Services, to ensure the funding on the purchase order is adequate to cover any services included in this attachment when requested by the COR. The government is not responsible for the expenditure of funds beyond those obligated under CLIN 0006.
2. Carpet Cleaning.
2.1 The quality standard for providing additional carpet cleaning/extraction services is the same as that described for Carpet and Rugs in Attachment J-1, Janitorial Services, Section 1.2.
2.2 When additional carpet cleaning services are required, the COR will request the Contractor to submit a price quote. The Contractor’s quote shall be based on the per square foot price established in the purchase order for carpet cleaning/extraction times the square footage to be cleaned. The schedule for performing the requested cleaning services will be coordinated between the Contractor and the COR. Under no circumstances is the Contractor to perform these services without written approval from the COR.
3. Interior and Exterior Window Washing/Cleaning.
3.1 When requested by the COR, the Contractor shall clean all exterior windows at the below identified sites. It is anticipated this effort will be required annually.
3850 S. Capitol (two stories) 910 Rhode Island (three stories) 1230 Taylor Street (two stories) 1900 Massachusetts Avenue (RSC) (eight stories)
3.2 When these services are required, the COR will request a quote from the Contractor and coordinate a schedule for completion of the work. A quote identifying a price for each location where services are to be performed. Once the COR agrees to the Contractor’s price, the services may be performed. COR agreement will be provided via email to the Contractor. Under no circumstances is the Contractor to perform these services without written approval from the
COR.
3.3 Windows shall be clean and free of dirt, grime, streaks, fingerprints, spots, paints, sprays, foreign materials, and moisture and shall not be cloudy. Window sashes, sills, woodwork, and other surroundings of glass shall be wiped free of drippings and other watermarks.
3.4 Interior window work shall be scheduled to be performed one floor at a time to keep disruption to building occupants to a minimum.

3.5 The Contractor is required to accomplish window washing work consistent with safety requirements promulgated by the Occupational Safety and Health Administration (OSHA).
Adequate fall protection for window washers is required to be provided by the Contractor.
4. Pressure Washing or Steam Cleaning
4.1 Upon request by the COR, the Contractor shall provide either pressure washing or steam cleaning services to remove all dirt, debris, residue, gum, grease, graffiti, and tar from a designated exterior surface/area of the building(s). Clean-up shall be done in an environmentally sound manner to minimize the amount of waste washed into the storm sewers or onto the ground.
4.2 When these services are required, the COR will request a quote from the Contractor and coordinate a schedule for completion of the work. Once the COR agrees to the Contractor’s price, the services may be performed. COR agreement will be provided via email to the Contractor. Under no circumstances is the Contractor to perform these services without written approval from the COR.
5. Tree Thinning.
5.1 Upon request by the COR, the Contractor shall provide tree thinning services in accordance with the specifications outlined below. When these services are required, the COR will request a quote from the Contractor and coordinate a schedule for completion of the work. Once the COR agrees to the Contractor’s price, the services may be performed. COR agreement will be provided via email to the Contractor. Under no circumstances is the Contractor to perform these services without written approval from the COR.
5.2 Tree thinning shall reduce the density of live branches towards developing the natural branching structure. Thinning shall result in an even distribution of branches on individual limbs and throughout the crown to provide free air circulation through the remaining limbs and branches. Not more than 25 percent of the crown should be removed within an annual growing season.
5.3 Tree thinning shall be performed only by arborists or arborist trainees who, through related training or on-the-job experience, or both, are familiar with the practices and hazards of arboriculture and the equipment used in such operations. This standard shall not take precedence over arboricultural safe work practices. Operations shall comply with applicable Occupational Safety and Health Administration (OSHA) standards, ANSI Z133.1, as well as State and local regulations.
5.4 Climbing spurs shall not be used when climbing and pruning trees. Tree branches shall be removed in such a manner so as not to cause damage to other parts of the tree or to other plants or property. Branches too large to support with one hand shall be precut to avoid splitting of the wood or tearing of the bark. Where necessary, ropes or other equipment shall be used to lower large branches or portions of branches to the ground.
6. Leaf Removal. The Contractor shall remove leaves, as necessary, to maintain a neat and clean appearance. Throughout the year, the Contractor shall remove minor accumulations due to isolated leaf drop and shall check all storm drain openings on the premises and remove any leaves or debris that have accumulated. Care shall be taken to remove leaves in existing mulched areas to maintain a neat and clean appearance without substantial reduction in mulch depth or damage to herbaceous or woody plant material. Under no circumstances is the Contractor to perform these services without written approval from the COR.
7. Pest Control.
7.1 Any eradication of pests not included in the services provided under Attachment J-3, Pest Control Services, are considered additional services. For example, eradication of birds, bats, snakes, and termites will be provided on an as needed basis.
7.2 If such services are needed, the COR will request a quote from the Contractor and coordinate a schedule for completion of the work. Once the COR agrees to the Contractor’s price, the services may be performed. COR agreement will be provided via email to the Contractor. Under no circumstances is the Contractor to perform these services without written approval from the COR.
8. Removal of Blood Borne Pathogens.
8.1 The Contractor shall provide all equipment and services required to perform clean-up of blood borne pathogens and fully comply with the requirements of the Blood Borne Pathogen Standard, listed in 29 CFR 1910.1030.
8.2 Clean-up of blood borne pathogens are services to be provided on an emergency basis. The Contractor must respond to the site for clean-up one hour of being notified by the COR. Upon completion of the clean-up services, the Contractor shall provide the COR a written statement of the services performed and the price for those services. This statement does not replace the requirement for an invoice. The purpose of the statement is to ensure the COR and Contractor are in agreement regarding the services performed and the final price for those services.
9. Emergency Service Calls for Lab Refrigerators.
9.1 In the event a failure occurs that results in the need for emergency service for any of the lab refrigerators identified in Attachment J-5, Lab Refrigerator Preventative Maintenance Services, the Contractor shall respond as identified in this section.
9.2 Within three hours of notification by the COR or the Contracting Officer, between the hours of 7:00AM and 7:00PM, Eastern Time, the Contractor shall respond to the location of the equipment needing service.
9.3 Upon completion of the on-site inspection, preferably prior to leaving the site, the Contractor shall provide notification to the COR of the required repair (including all required parts and labor) and associated costs. No repairs are to be performed without written authorization from the COR. Once authorized, in writing, by the COR, all repairs are to be completed within two business days of the approval to proceed.
9.4 Reporting. The contractor shall prepare and submit a written report within two business days after the emergency service call. The report shall include the date and time of the service call, the location of the equipment, the repairs performed, associated costs of the repairs, and the name of the technician who performed the repair.
10. Snow and Ice Removal Services.
10.1 Scope of Work. The Contractor shall provide snow and ice removal services as outlined in this section to fulfill the government’s intermittent need for snow and ice removal.
10.2 Notification to the COR before Beginning Snow and Ice Removal Services. The Contractor shall contact the COR at the following times when snow removal/de-icing services are required:
10.2.1 At least one hour prior to the start of any snow removal/de-icing services, including pre-treatment, at any CSOSA location;
10.2.2 At the point when no additional accumulation is to occur, the Contractor shall contact the COR upon completion of snow removal/de-icing at each CSOSA site.
10.3 Written Statement of Services Performed. Within two business days of completion of any snow and ice removal services (i.e., after snow event), the Contractor shall provide the COR a written statement of the services performed and the price for those services. The price shall be based on the item prices for snow and ice removal included in the purchase order. This statement does not replace the requirement for an invoice. The purpose of the statement is to ensure the COR and Contractor are in agreement regarding the services performed and the final price for those services.
10.4 Locations of Service.
10.4.1 The Contractor shall perform all snow and ice removal services outlined in this section at the following CSOSA locations:
• 1900 Massachusetts Avenue (RSC) (operational 24 hours a day/ 7 days a week)
• 1230 Taylor Street, NW
• 910 Rhode Island Ave, NE
• 3850 South Capitol Street, SE
10.4.2 1900 Massachusetts Ave., SE (RSC). The RSC is a 24-hour a day operational facility.
Therefore, snow/ice removal and de-icing shall occur seven days a week in accordance with the snow plowing and snow shoveling requirements outlined in this section.
10.4.3 For all other locations, snow/ice removal and de-icing shall occur such that the requirements identified in in this section are met between the hours of 6:00am through 7:00pm Monday through Friday.
10.5 Pre-treatment. Pretreatment of entrances, steps, sidewalks, driveways, plaza areas, and parking lots shall be provided any time snow is anticipated. Optimally this should be completed 4 to 6 hours prior to snow accumulation.
10.6 Snow and Ice Removal.
10.6.1 Snow and ice shall be removed from areas as shown on site drawings (Exhibit 1 to this attachment). Clear snow/ice from:
• Entrances
• Steps
• Sidewalks
• Approaches
• Fire hydrants;
• Traffic signs;
• Driveways;
• Parking lots;
• Catch basins; and
• Plaza areas
10.6.2 The Contractor is responsible for monitoring weather conditions as needed to ensure the above areas are maintained in a clear and safe condition.
10.6.3 After removal of snow and ice, the Contractor shall apply ice control (except on traffic signs, fire hydrants, and catch basins) anytime there is a chance of refreeze. The Contractor shall use chemicals and sand to reduce safety hazards due to ice and snow. Only chemicals included in the Contractor’s snow plan are authorized for use. The Contractor shall ensure there is an adequate supply of chemicals and sand on site or readily available to cover unexpected snow and ice occurrences.
10.7 Snow Shoveling Requirements. All locations that are shoveled, i.e., that cannot be plowed, are to be free of snow/ice at all times.
10.8 Snow Plowing Requirements.
10.8.1 Snow plowing and/or de-icing coverage of plowed areas begins when approximately one and one half inches of snow has fallen. Snow plowing and/or de-icing of plowed areas and will continue at every one and half inches of snow accumulation thereafter At the point when no additional accumulation is to occur, regardless of the amount of snow remaining, all remaining snow shall be cleared.
10.8.2 It is understood that depending on the length and severity of the ice or snowstorm that it may take the contractor varying amounts of time to fulfill all work required.
10.8.3 When snow plowing, the Contractor shall:
10.8.3.1 Stack excess snow such that it does not to impede traffic flow or pedestrians and will allow for use of all parking spaces.
10.8.3.2 Clear driveway entrances and exits to their full width with no snow piles blocking the visibility to incoming or outgoing traffic. Any snow from the city plowing the streets shall be cleared from the entrances of driveways.
10.8.3.3 At all times, areas shall be clear of snow and ice to allow for free direct vehicular and pedestrian traffic wherever required. Storing of materials or dumping of snow or ice on sidewalks or in planted areas is not acceptable.
10.9 Snow Plowing Areas Requiring Heavy Equipment. If requested by the COR, the Contractor shall provide snow and ice removal services using heavy equipment and associated labor and materials, e.g., bobcat, dump truck, etc. If these services are required, the COR will request the Contractor to submit a price quote based on the level-of-effort identified by the COR.
The Contractor’s quote shall be based on the unit price for heavy equipment snow removal established in the purchase order. There will be a maximum of one hour allowed for equipment loading, destination charges, equipment unloading, and returning equipment. The schedule for performing the requested services will be coordinated between the Contractor and the COR.
Under no circumstances is the Contractor to perform these services without written approval from the COR.
10.10 Contractor Responsibility.
10.10.1 The Contractor shall use caution when snow and ice removal services are in progress to prevent any damage to the buildings, grounds, vegetation, landscape areas, sidewalks, roads, fire hydrants, shrubs, signs, other protrusions, asphalt, concrete, or stone surfaces. The Contractor shall be held liable for any damage incurred to government property during the performance of work.
10.10.2 The Contractor is responsible for damages to vehicles parked or in areas to be plowed. The Contractor must notify the COR within 24 hours of any damages to any vehicles during the performance of snow and ice removal services. Contractor will be liable for any damages resulting from the services performed. The Contractor will be required to take pictures of any damages that occur as a result of snow plowing (e.g. damages to asphalt, concrete, vehicles, etc.) and provide copies to the COR via email within two hours from the time the damage occurs.
10.10.3 The Contractor shall ensure that all grass areas, concrete, and asphalt are protected from damage caused by the application of de-icing material. Any outdoor steps shall be given first priority, followed in turn by sidewalks, driveways and parking lots. De-icing material shall be applied using other than by hand methods.
10.10.4 All locally prescribed safety regulations, laws, and practices shall be carefully observed in performance of the work.
11. Office/Cubicle Move In/Out Cleaning.
11.1 The Contractor shall provide detail cleaning of offices and cubicles for move ins and outs.
If such services are needed, the COR will request a quote from the Contractor and coordinate a schedule for completion of the work. Once the COR agrees to the Contractor’s price, the services may be performed. COR agreement will be provided via email to the Contractor. Under no circumstances is the Contractor to perform these services without written approval from the
COR.
11.2 Services for this element of work include:
11.2.1 Furniture Cleaning. Furniture surfaces shall be cleaned and sanitized, including inside of drawers. Any papers and trash found in drawers shall be discarded.
11.2.2 Trash. Trash wastebasket shall be cleaned and new liner inserted.
11.2.3 Floor Vacuum. Floors shall be vacuumed.
11.2.4 Carpet Cleaning. Carpets shall be cleaned and spots removed.
11.2.5 Window Blinds. Window blinds shall be dusted.
11.2.6 Glass surfaces. Any glass surface, except computer screens, shall be clean and streak free.
