ATTACHMENT J-5
LAB REFRIGERATOR PREVENTATIVE MAINTENANCE SERVICES
1. Scope of Work. The Contractor shall provide all necessary management, supervision, personnel, labor, materials, parts and equipment necessary to perform preventive maintenance, calibration, certification, and repairs on the Court Services and Offender Supervision Agency (CSOSA) Medical Lab refrigerators.
2. Equipment Covered. The below Table 1 provides the location of the equipment covered under this Attachment J-5 as well as the equipment make, model, and serial number.

3. Quarterly Inspection and Preventive Maintenance Services.
3.1 The contractor shall perform quarterly inspections as well as preventive maintenance on the CSOSA equipment identified in Table 1 above. All work shall be performed so that all equipment is maintained in operating condition as prescribed by the original equipment manufacturer recommended guidelines to include all tasks identified in this Section C.
3.2 The quarterly inspection and preventive maintenance shall include a thorough inspection of the mechanical operation, the unit, cleaning of coils and a complete calibration, utilizing National Institute of Standards and Technology (NIST) certified instruments.
3.3 Reporting.
3.3.1 Within one business day of completing each quarterly inspection and preventive maintenance service, the Contractor shall provide a written report to detailing the following:
3.3.3.1 The equipment location, make, model, and serial number serviced;
3.3.3.2 Date of service;
3.3.3.3 For each equipment identified, provide the preventive maintenance service conducted;
3.3.3.4 Whether additional repairs are needed in order to maintain reliable, continuous, and safe operation and if so, identify the repairs required.
3.3.2 The report shall be provided via email to the Contracting Officer’s Representative
(COR).

4. Repairs Noted During Quarterly Inspection and Preventive Maintenance.
4.1 If an issue arises during the quarterly inspection and preventive maintenance which may result in a future unit failure, or hamper its operational status, or limit the unit in maintaining regulated temperature set points, the Contractor shall provide a detailed cost estimate for the required repairs. The cost estimate shall include the following:
4.1.1 The part(s) to be purchased and associated price for each part listed;
4.1.2 The amount of labor required to complete the repair and the associated price for the labor;
4.1.3 Any additional cost elements that make up the total price of the repair.
4.2 The repair estimate shall be submitted to the COR within two business days of conducting the quarterly inspection and preventive maintenance.
4.3 No repairs are to be performed without written authorization from the COR. Once authorized, in writing, by the COR, all repairs are to be completed within two business days of the approval to proceed.
5. Emergency Service Calls. Emergency service calls for all lab refrigerators identified in this attachment shall be performed in accordance with Attachment J-6, Additional Services, Section 10, Emergency Service Calls for Lab Refrigerators.
6. Hours of Operation.
6.1 Any work outlined above shall only be performed at CSOSA between the hours of 7:00AM and 7:00PM, Eastern Time, Monday through Friday, except federal holidays. Any work outside the above identified hours of operation is not authorized unless written approval is provided by the COR.
6.2 The Contractor shall coordinate any work at CSOSA with the Contracting Officer’s Representative (COR). The Contractor shall not perform any work at CSOSA on federal holidays or other non-business days without prior approval of the COR.
