The form was last updated on June 16, 2021, and is required under Texas state law for all vendors negotiating contracts or responding to RFPs and bids advertised by the local governmental entity. Vendors must certify that they do not have employment or other business relationships with members of the Harris County Juvenile Board, Deputy Directors or similar individuals of HCJPD, or family members of these individuals that would result in taxable income exceeding $2,500 during a 12-month period.
Additionally, vendors must certify that they have not provided Juvenile Board members, Deputy Directors or similar individuals, or their family members with one or more gifts with an aggregate value exceeding $100 during a 12-month period. Vendors who can certify these statements must sign and return the completed form; those unable to certify must instead complete the Conflict of Interest Questionnaire and follow its accompanying instructions. The certification requirements are based on sections 176.003 and 176.006 of the Texas Local Government Code.
