however, resource information such as data concerning labor hours and categories, materials, subcontracts, etc., must be contained in the technical quote so that your understanding of the

The technical quote shall be single spaced and shall be printed on 8.5 x 11 inch paper, times new roman, Font 12 and be a maximum length of thirty (30) pages. Each page shall be numbered and each volume shall be tabbed to correspond to the table of contents. Offerors may include 11 x 17 inch foldouts only to display graphics, flow charts, organizational charts, or drawings. Any additional pages will be removed and not evaluated.

Volume II - Price Quote.

The price quote shall propose labor categories, labor hours, any unit pricing used, any discounts offered and any other relevant pricing information that adequately fulfill the requirements set forth in the SOW. The Offeror shall also furnish a price breakdown to accomplish the SOW including a total firm-fixed price associated to the tasks listed in the SOW.

Two factors will be evaluated. Factor 1 -

Technical will be evaluated using Acceptable or

Unacceptable. Factor 2 - Price will be evaluated on the total price being determined to be fair and reasonable.

The Government reserves the right to make a single award, multiple awards, or no award as a result of this solicitation. In addition, the solicitation may be amended if determined to be in the best interest of the Government. The

Government reserves the right to make award without discussions.

The quote will be evaluated using the following criteria and weights as they relate to the content of the corresponding volumes.

The proposal shall provide a statement of the purpose, scope, and problems of the requirements in the Offeror's own words to demonstrate his/her complete understanding of the intent and requirements of the project. The discussion shall include anticipated major problem areas, together with potential approaches for their solution.

2. Key Personnel.

Offeror shall describe its approach in creating a schedule that meets the requirements of the SOW and delineate how absences will be back filled in seen or unforeseen circumstance.

The Key Personnel identified to work under this task order shall have the following minimum experience, education, and license:

Relevant Experience: At least 6 months experience of driving non-emergency medical patients.

Education: High school diploma or GED is required.

License: Key Personnel must have a chauffeur's license.

Contractor must provide proof that the key personnel assigned has successfully passed a drug test from a certified medical/drug testing facility and has no at-fault accidents on their driving record.

Factor 2 - Price.

Price will be evaluated on the total price being determined to be fair and reasonable.

Statement of Work (SOW) Title: Transportation Services

Date: July 18, 2019

I. Background

The Department of Health and Human Services (HHS), Health Resources and Services Administrations (HRSA), Healthcare Systems Bureau (HSB) provides for care, treatment, education, and research on Hansen’s Disease (HD) and Leprosy through the National Hansen’s Disease Program (NHDP), centered in Baton Rouge, Louisiana. The Operations Branch provides all ancillary support to the program which includes facilities management, transportation, and supplies.

II. Purpose / General Description

The purpose of this requirement is to provide transportation services for patients with HD.

Transportation is an integral component of the program’s operation and without these required services, patient care would not exist.

III. Period of Performance / Place of Performance

The period of performance shall be a base period of six (6) months plus one 6-month option period. The estimated start date is August 26, 2019.

The Place of Performance is specified per the task (See Section IV. Task 3).

Core Days and Hours of Operation

Monday 7:00am - 4:30pm Tuesday 7:00am - 4:30pm Wednesday 7:00am - 4:30pm Thursday 7:00am - 4:30pm Friday 7:00am – 4:30pm

For days worked that exceed 6 hours, a 30-minute uncompensated lunch break shall be included.

The contractor (and/or subcontractor) shall ensure that all employees having access to (1) Federal information or a Federal information system, or (2) personally identifiable information (PII), complete the HRSA Records Management Training before performing work under this contract, and thereafter completing the annual refresher course during the life of the contract.

The training can be requested by emailing the records management team at recordsmanagement3@hrsa.gov. The listing of completed training shall be included in the first progress report. Any revisions to this listing as a result of staffing changes shall be submitted with next required progress report.

Regardless of format, all digital content or communications materials produced as a deliverable under this contract must conform to applicable Section 508 standards to allow federal employees and members of the public with disabilities to access information that is comparable to information provided to persons without disabilities. The contractor is responsible for remediating all deliverables that do not comply with the applicable requirements as set forth below.

Task 3: Transportation

The Contractor shall:

A. Morning Shuttle:

1. Arrive at 1770 Physicians Park Drive by 7:00am.

2. Pick up the keys for the government van.

3. Fill out the Motor Vehicle Operators Trip ticket (Attachment B)

4. Depart 1770 Physicians Park Drive and drive to 5445 Point Clair Road, Carville, Louisiana, 70721 (30 Miles). Drive to building 29 where the patients are currently housed overnight. Arrival should be around 8:00am.

5. Pick-up one (1) to five (5) patients that are housed overnight.

6. Drive back to 17050 Medical Center Drive, Baton Rouge, Louisiana, 70816, Plaza II

(clinical branch of NHDP) to drop off patients off for medical treatment.

7. Walk the patients into Plaza II, onto the elevators, then to the second floor.

8. Every Friday, pick up the clean laundry from Room 1 and transport in the back of the van.

9. Drive the government van to 1770 Physicians Park Drive.

10. Complete the Motor Vehicle Operators Trip Ticket (Attachment B). Place the form in the driver box.

11. Drop the keys off in the “Key Box”.

B. Afternoon Drop-Off

1. Arrive at 1770 Physicians Park Drive by 2:15pm.

5. Once all the patients are in the van, drive to 5445 Point Clair Road, building 29. Walk the patients into the building. If an individual requires a wheel chair drop off, that individual must be in front of the recreation building.

6. After all the patients have been dropped off, return back to 1770 Physicians Park Drive.

7. Every Thursday, pick the laundry up from Plaza II and drop in Building 29, Room 1.

8. Complete the Motor Vehicle Trip Ticket (Attachment B), and drop the completed ticket off in driver box.

9. Shut and lock the gate.

10. Place the key in the “Keys Box”.

C. Airport Pick-up/Drop off

1. All airport pick-ups will be requested 48 hours prior to the patients’ arrival.

2. Key personnel will be notified via email that there will be a patient pick-up at the airport. The airport this program uses for patient travel is Baton Rouge Metropolitan (BTR, 9430 Jackie Cochran Drive, Baton Rouge, LA 70811).

3. Patient pick-up can be done during the morning or afternoon shuttle. For morning shuttle, the driver shall pick the patients up in Carville then arrive to the airport. In the afternoon, patients will leave Plaza II for the airport to accommodate the arrival/departure.

4. Patient drop off can be done during the morning or afternoon shuttle. For morning shuttle, the driver shall pick the patients up in Carville and proceed to the airport. In the afternoon, patients will leave Plaza II for the airport to accommodate the departure.

5. At the beginning and conclusion of any trip, drivers must fill out the Motor Vehicle Operators Ticket (Attachment B) to document mileage.

D. Bus station Pick-up/Drop-off

1. All bus station pick-ups/drop-offs will be requested 48 hours prior to the patient’s arrival.

2. Key personnel will be notified via email that there will be a patient pick-up at the bus station. The bus station for patient travel is Baton Rouge Bus Station (1253 Florida Blvd, Baton Rouge, Louisiana, 70802).

3. Patient pick-up shall be done during the morning or afternoon shuttle. For morning shuttle, the driver shall pick the patients up in Carville then arrive to the bus station. In the afternoon, patients will leave Plaza II for the bus station to accommodate the arrival/departure.

4. At the beginning and conclusion of any trip, drivers must fill out the Motor Vehicle

Operators Ticket (Attachment B) to document mileage.

Task 4: Ad-Hoc Shuttle Transportation:

If the arrival or departure time cannot be accommodated during the morning shuttle (9:00am – 10:30am) or afternoon shuttle (1:30pm – 3:00pm), there may be a request for a driver outside these timeframes. These request will be either for a departing flight or departing bus and arriving flight or arriving bus.

Note: In the past 6 months there have been 8 cases where the arrival or departing flights were outside this timeframe.

Task 5: Baseline Security Requirements.

Applicability. The requirements herein apply whether the entire contract or order (hereafter “contract”), or portion thereof, includes either or both of the following:

1. Access (Physical or Logical) to Government Information: A contractor (and/or and subcontractor) employee will have or will be given the ability to have, routine physical (entry) or logical (electronic) access to Government information.

2. Operate a Federal System Containing Information: A contractor (and/or any subcontractor) will operate a federal system and information technology containing data that supports the HHS mission. In addition to the Federal Acquisition Regulation (FAR) Subpart 2.1 definition of “information technology” (IT), the term as used in this section includes computers, ancillary equipment (including imaging peripherals, input, output, and storage devices necessary for security and surveillance), peripheral equipment designed to be controlled by the central processing unit of a computer, software, firmware and similar procedures, services (including support services), and related resources.

Task 6: Safeguarding Information and Information Systems.

In accordance with the Federal Information Processing Standards Publication (FIPS) 199, Standards for Security Categorization of Federal Information and Information Systems, the contractor (and/or any subcontractor) shall:

1. Protect government information and information systems in order to ensure:

• Confidentiality, which means preserving authorized restrictions on access and disclosure, based on the security terms found in this contract, including means for protecting personal privacy and proprietary information;

• Integrity, which means guarding against improper information modification or destruction, and ensuring information non-repudiation and authenticity; and

• Availability, which means ensuring timely and reliable access to and use of information.

2. Provide security for any contractor systems, and information contained therein, connected to an HHS network or operated by the contractor on behalf of HHS regardless of location. In addition, if new or unanticipated threats or hazards are discovered by either the agency or contractor, or if existing safeguards have ceased to function, the discoverer shall immediately, within one (1) hour or less, bring the situation to the attention of the other party.

3. Adopt and implement the policies, procedures, controls, and standards required by the HHS Information Security Program to ensure the confidentiality, integrity, and availability of government information and government information systems for which the contractor is responsible under this contract or to which the contractor may otherwise have access under this contract. Obtain the HHS Information Security Program security requirements, outlined in the HHS Information Security and Privacy Policy (IS2P), by contacting the CO/COR or emailing fisma@hhs.gov.

4. Comply with the Privacy Act requirements and tailor FAR clauses as needed.

Task 7: Protection of Sensitive Information.

For security purposes, information is or may be sensitive because it requires security to protect its confidentiality, integrity, and/or availability. The contractor (and/or any subcontractor) shall protect all government information that is or may be sensitive in accordance with OMB Memorandum M-06-16, Protection of Sensitive Agency Information by securing it with a FIPS 140-2 validated solution.

Task 8: Confidentiality and Nondisclosure of Information.

Any information provided to the contractor (and/or any subcontractor) by HHS or collected by the contractor on behalf of HHS shall be used only for the purpose of carrying out the provisions of this contract and shall not be disclosed or made known in any manner to any persons except as may be necessary in the performance of the contract. The contractor assumes responsibility for protection of the confidentiality of Government records and shall ensure that all work performed by its employees and subcontractors shall be under the supervision of the contractor. Each contractor or employee or any of its subcontractors to whom any HHS records may be made available or disclosed shall be notified in writing by the contractor that information disclosed to such officer or employee can be used only for that purpose and to the extent authorized herein.

The confidentiality, integrity, and availability of such information shall be protected in accordance with HHS and HRSA policies. Unauthorized disclosure of information will be subject to the HHS/HRSA sanction policies and/or governed by the following laws and regulations:

1. 18 U.S.C. 641 (Criminal Code: Public Money, Property or Records);

2. 18 U.S.C. 1905 (Criminal Code: Disclosure of Confidential Information); and

3. 44 U.S.C. Chapter 35, Subchapter I (Paperwork Reduction Act).

Task 9: Contractor Non-Disclosure Agreement (NDA).

Each contractor (and/or any subcontractor) employee having access to non-public government information under this contract shall complete the HRSA non-disclosure agreement (See Attachment C – non-disclosure agreement). A copy of each signed and witnessed NDA shall be submitted to the Contracting Officer (CO) and/or CO Representative (COR) prior to performing any work under this acquisition.

Task 10: Incident Response.

FISMA defines an incident as “an occurrence that (1) actually or imminently jeopardizes, without lawful authority, the integrity, confidentiality, or availability of information or an information system; or (2) constitutes a violation or imminent threat of violation of law, security policies, security procedures, or acceptable use policies. The HHS Policy for IT Security and Privacy Incident Reporting and Response further defines incidents as events involving cybersecurity and privacy threats, such as viruses, malicious user activity, loss of, unauthorized disclosure or destruction of data, and so on.

A privacy breach is a type of incident and is defined by Federal Information Security Modernization Act (FISMA) as the loss of control, compromise, unauthorized disclosure, unauthorized acquisition, or any similar occurrence where (1) a person other than an authorized user accesses or potentially accesses personally identifiable information or (2) an authorized user accesses or potentially accesses personally identifiable information for an other than authorized purpose. The HHS Policy for IT Security and Privacy Incident Reporting and Response further defines a breach as “a suspected or confirmed incident involving PII”.

In the event of a suspected or confirmed incident or breach, the contractor shall:

1. Protect all sensitive information, including any PII created, stored, or transmitted in the performance of this contract so as to avoid a secondary sensitive information incident with FIPS 140-2 validated encryption.

2. NOT notify affected individuals unless so instructed by the Contracting Officer or designated representative. If so instructed by the Contracting Officer or representative, the contractor shall send notifications to affected individuals following specific instructions from the HHS Privacy Incident Response Team (PIRT).

3. Report all suspected and confirmed information security and privacy incidents and breaches to the HRSA Security Operations Center…
