About this file
This document is an employee separation checklist for the Office of Civil Rights within the Department of Health and Human Services. It outlines the process for departing employees to return any office equipment, technology assets, keys, or identification badges assigned to them. Items listed include laptops, monitors, docking stations, power supplies, mice, keyboards, phones, headsets, printers, and PIV cards. The checklist provides fields to specify the employee name and last date, as well as the location of each physical asset. Departing staff must coordinate returning OCR-owned items to their supervisor, while OCIO equipment can be mailed back to the specified address. Contact information is also required for OCIO to retrieve laptops and devices located off-site.
The related federal contract opportunity is to provide executive consulting, strategic planning, program management, and administrative support services to the Office of Civil Rights at HHS. The pre-solicitation notice did not include pricing terms, response dates, or any other procurement specifics beyond the purpose and agency of the upcoming contract award.
