6 Combined Synopsis/Solicitation 75P001-19-Q-00238 Section L.3.
states that the Offeror’s proposal must be prepared in two (2) separate volumes:
1. Volume I – “Technical Volume”
2. Volume II – “Price/Business Volume”
Shall be separate and complete.
Additionally, L2 states to submit one (1) electronic version of this completed SF18 along with an exception to the terms and conditions and conditions or assumptions made in pricing the line items in a cover page to the Government:. There is no SF18 form included in the Solicitation, do you want the entire Solicitation document submitted with a cover page addressing exceptions, conditions and assumptions? Should this be submitted with both Volume I and Volume II?
Section L.5. Requires Offerors, other than those certifying themselves as a small business to submit a completed “Small Business Contracting Plan (Attachment 3 of the RFP). Should this be submitted in both Volume I and Volume II?
The SB Subcontracting Plan shall be submitted as part of the Business/Price Volume; reference revised section L.5 of Amendment 1.
M.2.2.1 Factor 1 – Technical Capability Phase 2 paragraph 1 states The Offeror should provide narrative statements that describe the organization’s approach and processes to perform all requirements specified in Section C.3.1 Web-based Respirator Tool Requirements of the PWS. There is no section C.3.1, do you mean section C.4.1?
C.4.1 is the correct reference; reference revised Section M.2.2.1 of Amendment 1
9 M.2.2.2 Factor 2 – Business Price requires offerors to complete, sign and include a complete copy of Standard form 18 with all applicable clauses filled in. Will you be providing a SF 18 form for Offerors to complete?

Reference response to question 10 above.
L.4 (j) states the Offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation “Unique Entity Identifier” followed by the unique entity identifier that identifies the Offeror’s name and address. The section goes on to state that if the offeror does not have a unique entity identifier, it should contact the entity designated at www.sam.gov for unique entity identifier establishment directly to obtain one. SAM.gov utilizes www.fsd.gov for contact/questions. Upon logging into www.fsd.gov and searching Unique Entity Identifier, the Answer received is:
Is the Unique Entity Identifier requested in L.4(j) the DUNS number?
FAR 512.217-9 – Option to Extend the Term of the Contract states:
(a) The Government may extend the term of this contract by written notice to the Contractor within 1 day prior to contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 15 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years (60 months).
Is this stating the government may extend the contract for an additional 60 months after the 4th option year, or that the entire contract is limited to 60 months - including option years and extensions?
FAR 52.217-9 ( c) specifically states: "The total duration of the contract, including the exercise of options, shall not exceed 60 months", and applies to the base IDIQ contract only. This means from the date of contract award, inclusive of options if exercised, the total period of performance of the base IDIQ contract cannot exceed 60 months. However, there are other clauses that may allow extension of this time if exercised, reference FAR 52.217-8.
L.4 (J) your are referencing is part of FAR clause 52.212-1 and is referencing the unique entity identifier that is part of your SAM registration. For the purposes of the cover page for this requirement PSC uses the company's DUNS number.
