About this file

This is a Sole-Source Justification for an acquisition above the Simplified Acquisition Threshold submitted by the Department of Health and Human Services, Indian Health Service (IHS).

The IHS Portland Area is requesting continued services for a Youth Residential Treatment Center (YRTC) serving eligible American Indian and Alaska Native (AI/AN) male and female youth. The requirement includes 24-hour residential substance use disorder treatment, clinical services, case management, culturally responsive programming, referrals, and aftercare coordination in accordance with IHS standards. The recommended sole-source contractor is Native American Rehabilitation Association of the Northwest, Inc. (NARA), located at 1776 SW Madison Street, Portland, OR 97205. The estimated procurement value is $478,739.50 with a tentative performance period of March 28, 2026 through September 30, 2026. The justification for sole-source selection is based on unique capabilities, as market research identified no other responsible sources capable of meeting the Government's minimum requirements within the Portland metropolitan area. The contractor must currently operate an established, state-licensed youth residential treatment facility with organizational infrastructure, clinical staffing, accreditation, and operational capacity to provide immediate and uninterrupted services, plus demonstrated capability to deliver culturally specific, trauma-informed programming with established relationships with Tribal communities. The Government is concurrently planning a long-term competitive contract anticipated for award at fiscal year-end, and this interim award is necessary to ensure continued service without disruption, which would otherwise present significant clinical and safety risks to current patients. The justification has been certified by the Project Officer, Project Officer's Immediate Supervisor, and Contracting Officer as accurate and complete.

SOLE-SOURCE JUSTIFICATION

FOR ACQUISITIONS ABOVE SIMPLIFIED ACQUISITION THRESHOLD

Activity: The Department of Health and Human Services, Indian Health Service (IHS) proposes to enter into a purchase order on Sole Source under the following acquisition as implemented by FAR Subpart 6.103-1.

1. Requisition#: TBD

2. Description of Supplies or Services: Portland Area Indian Health Services is requesting for Youth Residential Treatment Center (YRTC) continued services for eligible American Indian and Alaska Native (AI/AN) male and female youth, including 24-hour residential substance use disorder treatment, clinical services, case management, culturally responsive programming, referrals, and aftercare coordination, in accordance with Indian Health Service standards.

3. Period of Performance: Tentative: 3/28/2026-9/30/2026

4. Estimated Value of Procurement: $478,739.50

5. Recommended Sources:

Native American Rehabilitation Association of the Northwest, Inc. (NARA) 1776 SW Madison Street Portland, OR 97205

6. Reason for Sole Source/Limited Sources/Expedited Processing:

[ x] Only one source (unique capabilities) [ ] Proprietary drawings, specifications, and/or engineering data, patents, copyrights.

[ ] Brand Name essential to the Government’s needs.

[ ] Standardization Program supported by Agency Head Determination and Findings (D&F).

[ ] Unusual and Compelling Urgency [ ] Authorized or required by statute

a. Justification Narrative:

The Indian Health Service requires Youth Residential Treatment Center (YRTC) services for eligible American Indian and Alaska Native (AI/AN) youth in the Portland, Oregon metropolitan area. The requirement remains unchanged from prior acquisitions and consists of 24 hours per day, 7 days per week residential substance use disorder treatment delivered in a licensed facility providing culturally specific, trauma-informed care.

To meet the Government’s minimum needs, the contractor must currently operate an established, state-licensed youth residential treatment facility within the required geographic area at the time of award. The contractor must possess the organizational infrastructure, clinical staffing, accreditation, and operational capacity necessary to provide immediate and uninterrupted residential behavioral health services. Additionally, the contractor must demonstrate the capability to deliver culturally responsive programming specifically designed to address the unique social, cultural, and behavioral health needs of AI/AN adolescents, including established relationships with Tribal communities and referral networks.

Market research conducted for this requirement identified no other responsible sources capable of meeting these minimum requirements within the Portland metropolitan area. No additional qualified sources have been identified that operate a comparable licensed youth residential treatment facility capable of providing immediate continuity of culturally specific residential services within the required timeframe.

The Government is concurrently conducting acquisition planning for a long-term competitive contract anticipated for award at the end of the fiscal year. This action is necessary to ensure continued performance of required services during the period needed to complete that acquisition.

Accordingly, only one responsible source is reasonably available to meet the Government’s minimum requirements for geographic presence, licensure, operational readiness, and culturally specific residential treatment capability during this interim period.

b. Impact to the Government if not awarded in accordance with the above:

Failure to award this action would result in a disruption of residential behavioral health services for AI/AN youth currently receiving treatment. Any interruption in 24-hour residential substance use disorder services would present significant clinical and safety risks, including treatment regression, increased risk of relapse, and potential harm to patients.

In addition, a lapse in services would undermine continuity of care, disrupt established therapeutic relationships, and negatively affect coordination with Tribal communities and referral partners. Such disruption would impair the Indian Health Service’s ability to carry out its mission to raise the physical, mental, social, and spiritual health of American Indians and Alaska Natives to the highest level.

Because residential treatment services must be delivered without interruption and no alternate source can meet the Government’s minimum needs within the required timeframe and geographic location, failure to award would result in unacceptable programmatic and patient-care risk.

7. Program Office Certification This is to certify that the portions of this justification that have been developed by the undersigned program office personnel, including supporting information and/or data verifying the Government’s minimum needs or requirements, or other rationale for limited sources, are accurate and complete.
