About this file
This is a Questions and Answers document for Indian Health Service solicitation 75H71326Q00007, a medical supply procurement issued by the Department of Health & Human Services, Public Health Indian Health Service, Portland Area Office. The document, dated June 30, 2026, provides clarifications on three key issues raised by potential quoters.
The first clarification addresses units of measure (UOM) for pricing purposes. The Government amended Attachment 1 (Schedule of Items) to include a separate Unit of Measure column on each pricing tab, with package indicators moved from item descriptions to plain language designations such as Box of 50, Box of 200, Pack of 20, Case of 6, Pair, or Each. Quoters must price each line item using the stated UOM, and the estimated annual quantity applies to that specified UOM. The second clarification confirms that no separate SF 1449 or alternate form will be issued; instead, the Government is using the RFO FAR Part 12 combined synopsis/solicitation streamlined procedure, with the combined document, attachments, and amendments posted to SAM.gov constituting the RFQ. The third clarification establishes that firm-fixed unit pricing is required for the base period and each option period with no separate economic price adjustment mechanism included. Quoters must propose unit prices for each line item and period in Attachment 1, incorporating anticipated manufacturer cost increases, shipping, handling, overhead, profit, and discounts into their initial quotations. Option-period prices accepted at award are firm, and manufacturer price increases alone do not justify awarded price increases unless approved through written contract modification by the Contracting Officer.

MEMORANDUM
Date: June 30, 2026
From: Jacob Blalock, Purchasing Agent
Subject: Questions and Answers - 75H71326Q00007 Solicitation
To: All Concerned Parties, Contract File
The following is a list of questions received regarding subject solicitation with responses:
1. Units of Measure. We are reviewing your Medical Supply solicitation #75H71326Q00007 and have a quick question. What are the UOMs of the products as related to the annual quantity given? Are they all eaches or does it vary by product? For example, CLIN #0018 says 50/bx in the description, so the assumption is that means the annual quantity is 200 boxes; but CLIN #0019 does not give any packaging, so would this be 16,000 boxes or eaches? Since the sheet sums the extended price, we would like to make sure the unit prices quoted are apples to apples.
Response: The units of measure vary by line item. The Government has amended Attachment 1 - Schedule of Items to add a separate Unit of Measure column on each pricing tab. Quoters shall price each line item using the Unit of Measure stated in the amended schedule. Package indicators previously embedded in the item description have been moved to the Unit of Measure column in plain language, such as Box of 50, Box of 200, Box of 170, Pack of 20, Case of 6, Pair, or Each. The estimated annual quantity applies to the stated Unit of Measure. For example, if CLIN #0018 identifies the Unit of Measure as Box of 50 and the estimated annual quantity as 200, quote per box and treat the annual quantity as 200 boxes. If CLIN #0019 identifies the Unit of Measure as Each and the estimated annual quantity as 16,000, quote per each and treat the annual quantity as 16,000 eaches.
2. Solicitation Form. Will there be an SF 1449 or alternate form issued on SAM.gov for this solicitation?
Response: No separate SF 1449 or alternate form will be issued. The Government is using the RFO FAR Part 12 combined synopsis/solicitation streamlined procedure. The combined synopsis/solicitation document, attachments, and any amendments posted to SAM.gov constitute the RFQ for this requirement. Quoters shall submit their quotation in accordance with the solicitation instructions.

3. Economic Price Adjustments. In response to Solicitation 75H71326Q00007, can the Government clarify whether annual price adjustments are permissible if manufacturer cost increases occur during the option periods? Specifically, may pricing be adjusted annually as costs increase, or must anticipated increases be built into the initial quotation pricing?
Response: The solicitation requests firm-fixed unit pricing for the base period and each option period. No separate economic price adjustment mechanism is included in the solicitation.
Quoters shall propose unit prices for each line item for the base period and each option period in Attachment 1, including any anticipated manufacturer cost increases, shipping, handling, overhead, profit, and discounts. Option-period prices, if the options are exercised, will be the prices accepted at award. Manufacturer price increases alone do not entitle the contractor to increase awarded prices. Backorders, discontinued items, and substitutions shall be handled in accordance with the Specifications Sheet and do not authorize a price increase unless approved by the Contracting Officer through a written contract modification.
