This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
75H1225Q-011KN is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2025-03.
The Phoenix Area Indian Health Services (IHS) has a requirement for Air Charter Services for transportation of clinical, administrative, and other staff of the Phoenix Indian Medical Center (PIMC) located in Phoenix, Arizona. This procurement is conducted pursuant to the authority of Federal Acquisition Regulation (FAR) part 12, Commercial Products & Commercial Services in conjunction with FAR 13.5, Simplified Procedures for Certain Commercial Products.
Set-Aside 100% Small Business NAICS 481111 – Scheduled Passenger Air Transportation SB Size Standard $40.0 million
The Government intends to award to a responsible party whose offer meets the solicitation requirements and provides “Best Value to the Government” based on price and other factors. Offerors must be registered and active in the System for Award Management (SAM) at the time of quotation submission. Award announcement will be made through electronic public notice, via SAM.gov.

SECTION B – CONTINUATION
1. CONTRACT TYPE
The Phoenix Area Indian Health Services, anticipates awarding a single Firm Fixed Price award.
2. PERIOD OF PERFORMANCE
The initial Period of Performance is intended to be performed 12-months from award date. Contract performance shall be for a base year and four (4) option years. Option years will be exercised pursuant to FAR 52.217-9, Option to Extend the Term of the Contract.

(d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 1 day of contract expiring.

(a) The Government may extend the term of this contract by written notice to the Contractor within 15 days of the contract expiring; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.

Submitting the Quote: Submit your company’s quote via email to kenneth.nicholson@ihs.gov. Quotes must be received via email no later than: May 19, 2025 at 2:00 pm MDT. Include the following information beginning on the second page of your quote. Have the items sectioned and clearly marked:
1. Pricing: Your total price and any discount terms.

2. Technical: Narrative (no more than three (3) pages) detailing the approach to execute the project.
Provide details, including any product or service literature, and capabilities of your company.
Technical must contain a description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. Include a potential start date as this will be given consideration to best value determination.
3. Past Performance: List of jobs/contracts completed or ongoing in the past five (5) years in which you acted as the Prime Contractor or Sub-Contractor responsible for the performance of related services. Provide at least 3 and no more than 5 past performance records.
Records can be from federal, state, and local government, and private industry. Each record must contain the following information:
• Project Title
• Start and End dates
• Brief Description
• Name of Customer and their contact information
• Total dollar value
System for Award Management (SAM): prospective contractors are required to be registered in the System for Award Management (SAM) at the time an offer or quotation is submitted in order to comply with the annual representations and certifications requirements. Once registered, you must remain registered throughout performance until final payment. Go tohttps://www.acquisition.gov for information on SAM registration and annual confirmation.
Issuance of Purchase Order: Your quote should contain your best terms. The Contracting Officer may reject any or all quotes. After evaluation of quotes, the Contracting Officer may negotiate final terms with one or more quoters before issuing the award. The Contracting Officer will not negotiate with any quoters other than those of the Government’s choice and will not use the formal source selection procedures described in FAR Part 15.

52.212-2 -- Evaluation -- Commercial Items (Oct 2014) [Addenda]
Evaluation procedures will be IAW FAR 13.106. The Government will select the quote that represents the best value to the Government. The Government will perform a comparative evaluation of price, technical solution, and past performance. The evaluation factors Technical and Past Performance, when combined, are approximately equal to cost or price. The following factors shall be used to evaluate offers:
(i). PRICE:
Total evaluated price, including options, will be calculated for evaluation purposes and used to determine the best value to the Government.
(ii). TECHNICAL:
The contractor will be evaluated based upon their technical narrative and capabilities as they relate to the requirement, as well as their overall understanding of the project.
(iii). PAST PERFORMANCE The offeror will be evaluated based upon the relevance and quality of past contracts/jobs performed. More recent and more relevant performance has a greater impact in the Past Performance assessment than less recent and less relevant performance.
NOTE: The Government reserves the right to contact any reference provided by the offeror in evaluating the offeror’s past performance. Furthermore, the Government reserves the right to use any information that comes to the attention of the Government in evaluating the offeror.
The Government reserves the right to make an award to other than the lowest priced quote if another superior if another superior technical submission, or a submission indicating a reduced performance risk, warrants paying a premium. As non-price factors are evaluated closer to one another between quotes, price will become more significant.
FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (Feb 2021)
Provision must be completed in the offeror’s SAM.Gov profile.
System updates may lag policy updates. The System for Award Management (SAM) may continue to require entities to complete representations based on provisions that are not included in agency solicitations. Examples include 52.222-25, Affirmative Action Compliance, and paragraph (d) of 52.212-3, Offeror Representations and Certifications— Commercial Products and Commercial Services. Contracting officers will not consider these representations when making award decisions or enforce requirements. Entities are not required to, nor are they able to, update their entity registration to remove these representations in SAM.
