CV’s/Resumes for the proposed ED Board Certified Physicians shall be submitted for evaluation and meet the minimum requirements of 6.4.1, 6.4.2, 6.4.3, 6.4.4, 6.4.5, 6.4.6, 6.4.7, 6.4.8
Provide a copy of mal practice insurance covering the company and proposed physicians.
