About this file

This document is Standard Form 30 (Amendment 0001) to Solicitation 75H71026Q00184 issued by Northern Navajo Medical Center in Shiprock, New Mexico on June 16, 2026. The amendment revises the Schedule of Items, provides answers to offeror questions, and extends the solicitation closing date to June 26, 2026 at 3:00 PM Mountain Daylight Time. The solicitation seeks General Surgery Nocturnist services with the Government anticipating multiple awards to satisfy the requirement for two physician positions.

The requirement includes approximately 2,080 hours per six-month performance period, structured across three labor categories: Regular Hours (1,040), Call-Back Hours (1,040), and On-Call Hours (1,412). All hours are invoiced at the same hourly rate. Regular Hours cover nighttime shifts beginning at 5 PM, with weekends covered from 5 PM Friday through Monday morning. Call-Back Hours apply when the contractor is called to the facility for patient care, with expected response times of 10 minutes for pages and 30-minute arrival for assessment. On-Call Hours apply when the contractor carries a pager from the call apartment while remaining available for consultation and inpatient surgical patient care coverage. The position is flexible to accommodate future changes to daytime work if needed. Holiday hours are included at the same rate. Offerors may propose multiple qualified physicians under a single quotation and should price based on the Schedule of Items quantities. The Government prefers one contractor per weekend to minimize hand-offs but allows distribution of coverage among multiple awarded contractors.

Questions for Solicitation 75H71026Q00184

1. Reference: Schedule of Items; PWS Section 5.2.2 The Schedule of Items identifies 1,040 Regular Hours per physician during each six-month performance period. The PWS work schedule describes overnight on-call coverage and call-back services but does not identify regularly scheduled onsite clinical, clinic, operating room, or inpatient coverage hours.

Question: Please clarify the duties and work schedule associated with the Regular Hours CLIN and identify when Regular Hours are expected to be utilized.

This position covers mostly nights but the shift starts at 5PM. Weekends are covered from 5PM Friday through Monday morning. Most hours will be nights and weekends. Occasionally the covering nocturnist may take a patient to the OR in the morning with the day time team either as primary or first assist. The intent is not to have “regular” M-F day time work scheduled. However the contract is flexible in case that change is needed in the future.

2. Reference: Schedule of Items; PWS Sections 5.1.4.4 and 5.2.2 The Schedule of Items identifies 1,040 Call-Back Hours per physician during each six-month performance period.

Question: Please clarify how Call-Back Hours are defined, tracked, and invoiced.

Specifically, does call-back time begin when the provider is contacted, when the provider arrives at the facility, or when patient care activities begin?

The contractor is paid a flat rate for all the hours worked regardless of context. So for example if the nocturnist is in the call apartment, not called in, they still are paid the same as if they came in to do a surgery or were rounding in the admitted patients, The contract is designed this way because the volume is relatively low but the services are greatly needed.

3. Reference: Schedule of Items; PWS Section 5.2.2 Question: Please confirm that the On-Call Hours CLIN applies to periods during which the contractor is carrying the pager and available for consultation but is not actively engaged in patient care activities requiring call-back to the facility.

The contractor is theoretically always engaged in care activities because if there are patients on the general surgery service the contractor is covering their care while on duty. In addition to covering inpatient surgical patients as well as any care support needed for general surgery patients during their duty hours (say questions on an outpatient antibiotic being picked up over the weekend by our pharmacy) they are also available for consultation. The difference is the contractor is able to take phone calls/cross cover from the call apartment and come in as needed.

4. Reference: Schedule of Items Question: Are the estimated quantities for Regular Hours (1,040), Call-Back Hours (1,040), and On-Call Hours (1,412) based on historical utilization, projected utilization, or estimated maximum requirements?

We anticipate that contractors will need to cover about 2080 hours during each 6 month period of the contract. This is based on known needs at the facility. We are designing the call coverage schedule around the nocturnist.

5. Reference: Schedule of Items Question: Please provide the Government's expected invoicing methodology for the three labor categories. Should contractors invoice actual hours worked within each category, or will invoicing be based on a predetermined allocation of hours?

Contractors shall invoice for actual hours worked

6. Reference: PWS Section 5.1.4.4 and 5.2.2 Question: Please identify any required response times for telephone consultations and any required arrival times when called back to the facility for patient care activities.

The expectation is to return pages within 10 minutes (or by proxy if in the OR) and arrival for assessment within 30 minutes.

7. Reference: PWS Section 5.2.2 Question: The PWS states that the contractor will cover one to two full weekends per month. Please confirm whether this requirement is assigned per individual physician or whether weekend coverage may be distributed among multiple awarded contractors.

Weekend coverage could be distributed but we would prefer one contractor per weekend to minimize hand offs. But each weekend could be covered by a different individual.

8. Reference: Schedule of Items Note The solicitation states that the Government may issue multiple awards to satisfy the requirement for two General Surgery Nocturnists.

Question: If multiple awards are made, how does the Government anticipate distributing shifts, call coverage, and estimated hours among awarded contractors?

Coverage will be based on the Government's need for two General Surgery Nocturnist positions.

9. Reference: Evaluation Factors and Key Personnel Requirements Question: May offerors propose multiple qualified physicians under a single quotation, and if so, will the Government evaluate staffing depth and continuity of coverage as a strength?

Yes. Offerors may propose multiple qualified physicians under a single quotation.

Quotations will be evaluated in accordance with the evaluation factors identified in the solicitation.

10. Reference: PWS Section 5.1.4.4 Question: For planning and pricing purposes, can the Government provide historical workload information from the most recent contract period, including:

a. Average number of overnight calls per month;

b. Average number of call-back events per month;

c. Average number of emergency surgical procedures performed during overnight coverage;

d. Average number of weekend call-back events per month?

This is very variable and depends on if patients or admitted or present to the ED.

An estimate would be 0-4 ED calls a night and 0-1 overnight cases. The service contains 0-4 patients.

I would estimate 1-3 cases done overnight per week, 0-2 cases sent out after stabilization, 0-5 cases scheduled overnight but done the next day per week.

In a 24 hour weekend day would anticipate 1-4 ED consults. An average weekend would estimate 4-10 ED consults, so 16-40 a month. Some consults are uncomplicated, some require admission, some require procedures and still other stabilization or more urgent intervention.

11. Reference: PWS Section 5.2.2

Question: Please confirm whether holiday coverage is included within the estimated CLIN quantities identified in the Schedule of Items. Can the Schedule of Items be modified to include a CLIN for holiday coverage hours?

Holiday hours are included and are at the same rate.

12. Are the quantities listed for Regular Hours, Call-Back Hours, and On-Call Hours estimated billing quantities only, or are they intended to represent the actual anticipated workload?

Anticipated workload is 2080 hours for 6 months.

13. How does the Government define and distinguish "Regular Hours" versus "Call-Back Hours" for invoicing and payment purposes?

They are invoiced at the same rate. The only distinction is time of day.

14. Should offerors price strictly against the quantities listed in the pricing schedule, regardless of the actual coverage hours described in the PWS?

Offerors shall provide pricing based on the quantities identified in the Schedule of Items.

15. Is the Government seeking coverage from a single physician, or does it anticipate making multiple awards and distributing the schedule among more than one physician?

The solicitation states that the Government may make multiple awards to satisfy the requirement for two General Surgery Nocturnist positions.

16. Is the provider required to remain physically present in the hospital during the overnight shifts, or is this considered home-call coverage with return to the facility only when paged?

Call apartment coverage with return to the facility when paged.

17. Could you please confirm if this date is correct and if there have been any updates or amendments that may not have been posted yet?

The dates on the solicitation notice are correct. Any changes or amendments will also be posted on SAM.gov.

18. What hours constitute Regular Hours?

All hours are billed at the same rate.

19. Is the General Surgery Nocturnist required to remain in-house during the overnight shift?

No, they can stay in the call apartment and return as needed

20. When does billable Call-Back time begin and end?

The entire time the surgeon is working, holding the pager etc is billed at the same hourly rate.

21. Is the contractor compensated for all scheduled overnight hours as Regular Hours in addition to On-Call Hours?

All hours worked receive the same rate.
