About this file

This document is Amendment 0001 to Solicitation 75H71026Q00065, issued by Kayenta Health Center Acquisition Department on February 6, 2026, with an effective date of February 6, 2026. The solicitation remains open for quotes until February 23, 2026 at 12:00 PM MST, with a performance period of March 1, 2026 through February 28, 2027. This is a Total Small Business Set-Aside requiring electronic payment submission through www.ipp.gov per clause 352.232-71.

The requirement is for infectious waste collection and disposal services at two healthcare facilities in rural Arizona: Kayenta Health Center (KHC) and Inscription House Health Center (IHHC). KHC requires bi-weekly pickups on Mondays with an estimated annual volume of approximately 450 containers (average 20 per pickup, peak 32), while IHHC requires bi-weekly service with potential reduction to monthly based on infection control/safety approval, with estimated annual volume of approximately 20 containers (average less than 1 per pickup overall). Contractors must provide 31-gallon regulated medical waste containers (35 minimum on-site for KHC, 5 for IHHC), pick up at designated collection points during facility operating hours (Monday–Friday, 8:00 AM–5:00 PM, observing federal holidays), and submit signed manifests with certificates of receipt and destruction. The Government provides red disposal bags and secured storage; contractors select permitted treatment/disposal facilities compliant with federal/state/tribal regulations and obtain necessary permits. Invoices shall be submitted monthly, itemized by CLIN with manifest references, segregated by site. Pricing may include minimum pickup/no-waste fees and fuel/energy surcharges with objective index-based adjustment methodology.

Questions and Answers

1. Can the Government confirm the estimated volume (e.g., pounds or containers) of infectious waste generated per pickup at each facility (Kayenta Health Center and Inscription House Health Center), or provide historical usage data?

Kayenta Health Center (KHC)

• Total containers (9/2024–12/2025): 591

• Average per pickup: ~20 containers

• Peak per pickup: 32 containers

• Estimated annual volume: ~450 containers

Inscription House Health Center (IHHC)

• Total containers (9/2024–11/2025): 19

• Average per pickup (overall): <1 container

• Average per non-zero pickup: ~3 containers

• Peak per pickup: 4 containers

• Estimated annual volume: ~20 containers

Combined estimated annual volume: ~470 containers

2. Is the bi-monthly pickup schedule fixed (e.g., specific weeks/days), or may the Contractor propose pickup days in coordination with the COR?

• KHC: Bi-weekly pickup required.

• IHHC: Bi-weekly schedule currently in place, could limit to monthly schedule. Will be dependent on Safety and infection control standards.

However, the Government reserves the right to adjust IHHC pickup frequency (e.g., monthly or on call) based on historical low volume trends.

IHHC is extremely low volume.

Compared to KHC:

Facility Total Containers Avg Per Pickup Peak

KHC 591 19.7 32

IHHC 19 0.7 4

IHHC volume is ~3% of KHC volume.

Pickups occur on Mondays. If Monday is a federal holiday, pickup will occur the next business day.

3. For the “Minimum Pickup/No Waste” CLINs, should pricing assume a required site visit even when no waste is collected, or is this intended as a standby charge only?

The following fee may be applied: Minimum Pickup/No Waste Fee for both sites

4. Are there any anticipated changes to operating hours or access procedures that could impact pickup scheduling during the period of performance?

The facilities operating hours are Monday through Friday from 8:00 am to 5:00 pm, all federal holidays are observed. Vendor shall comply with the facilities operating hours.

5. Can the Government confirm whether fuel or environmental surcharges are expected to be fixed for the contract year, or adjusted periodically?

IHHC can reduce number of pick ups to 1 pick up per month. On the first Monday of the month. If Monday falls on a federal holiday the pickup will be scheduled for the next business day.

-Are there any facility holidays, restricted hours, or access/security protocols we should be aware of? The facility observes all federal holidays. Operating hours are 8:00 am to 5:00 pm Monday to Friday.

-Please clarify whether there is one designated on-site hazardous waste collection point or multiple collection locations throughout the hospital facility?

There is one designated hazardous waste collection point at both locations

10. Regulatory / Documentation Requirements

-Are there any additional tribal, state, or Joint Commission documentation or reporting requirements beyond the standard signed manifests?
