Q&A

RFQ No. 75H70726Q00107

Nurse Practitioner – Ute Mountain Ute Health Center, Towaoc, CO

1. Q: We would like to know if this is a brand new contract OR if there is (was) an incumbent performing these services. If not brand new, could you please provide the current / previous contract number?

A: This is an existing requirement. Services are currently being provided under emergency contract No. 75H70725P00111. This solicitation is for continued Nurse Practitioner services

2. Q: Can you confirm if there is a current incumbent supporting this requirement?

A. Yes

3. Q: Is there a current incumbent contractor for this requirement? If so, provide the contract number.

A. Yes, 75H70725P00111

4. Q: The Statement of Work references BLS certification and, depending on service unit needs, ACLS and PALS certifications. Could the Government confirm whether BLS alone is sufficient with the remaining certifications required post-award?

A. Current BLS certification is required, and documentation shall be submitted with the quotation. ACLS certification is also required but may be obtained after award at the Contractor’s expense. PALS certification is recommended but is not required.

5. Q: Are we allowed to submit the OF 306 (Declaration for Federal Employment) forms for candidates after the contract award, or are these required during the pre-award stage?

A. The OF-306 shall be completed after award as part of the Government’s onboarding and background-investigation process. It is not required with the quotation. All required background-investigation and suitability approvals must be completed before performance begins. See Section B.9(a) of the RFQ.

6. Q: Is on-site or Government-furnished housing available for the awarded provider at the facility, or is the position expected to be all-inclusive with no Government-furnished housing or travel?

A. Government-furnished housing is not available. The quoted hourly rate shall be fully burdened and include all costs identified in the RFQ, including travel, per diem, and lodging. These costs will not be reimbursed separately.

7. Q: Can the Government please confirm if there are any file size limitations that will affect/restrict its ability to receive and open documents via email?

A. The Government has not specified a maximum email attachment size for this solicitation. Quoters should keep attachment sizes manageable and may submit their quotation in multiple emails, clearly labeled with the solicitation number and “Part X of Y.”

All parts must be received by the quotation deadline. Quoters are encouraged to request acknowledgment of receipt.

8. Q: Can the Government please specify how they would like the "Additional Required Documents" to be submitted by Offerors (SF-18, amendments, IEE Form, etc.)? For example, would the Government like them submiited as part of Volume 3 - Price, or should Offerors submit them within a separate "Volume 4 - Business Information" volume?

A. Submit the Additional Required Documents, including the signed SF-18, acknowledgment of amendments, and signed IEE Representation Form, as a separate attachment labeled “Additional Required Documents” with the quotation email. These documents may be combined into a single PDF or attached separately. A separate Volume 4 is not required.

9. Q: While the Government states that Offerors may submit "up to five (5) recent and relevant" contracts within Volume 2 - Past Performance, can the Government please clarify if there is a minimum amount to be provided?

A. There is no minimum number of past performance references required. Quoters may submit up to five (5) recent and relevant contracts or assignments. As stated in Section F, quoters without relevant past performance, or for whom sufficient performance information is unavailable, will receive a Neutral Confidence assessment and will not be evaluated favorably or unfavorably on that basis.

10. Q: Can the Government confirm whether this opportunity is currently funded? If so, does the Government anticipate any obstacles that could delay or prevent an award from being made?

A. Funding is available for the base period of this requirement. At this time, the Government is not aware of any circumstances expected to prevent award. Award remains subject to completion of the evaluation and applicable acquisition requirements. Funding for option periods is subject to future availability.

11. Q: If housing/lodging is needed, does the government provide or have housing available?

If so, what are the costs?

A. No Government-furnished housing is available. Any required lodging shall be included in the quoted fully burdened hourly rate.

12. Q: Would the Government consider providing an extension to this opportunity to allow respondents adequate time to address the Government's responses to submitted questions?

A. Yes. To provide additional time for quoters to review the Government’s responses and finalize their quotations, the quotation deadline is extended to Monday, November 9, 2026, at 5:00 p.m. MST.
