Propane Refills As-Needed Services Zia Health Clinic
Refills to be provided at:
Zia Health Clinic 155 Capitol Square Drive Zia Pueblo, NM 87053
Schedule of Services:
Requested services are on an as-needed basis. Both the Contractor and the Clinic site will coordinate a refill schedule. The refill will typically occur once per year (about 1,000 gallons per year).
Changes to the refill schedule must be approved and sent to all necessary parties.
The per gallon cost is inclusive of all necessary labor, materials, equipment, supplies, permits, licenses, and transportation costs to provide mobile on-site propane services.
Delivery of Services:
Site events are scheduled in advance on an as-needed basis. Clear access to the 1000-gallon tank is made available to the contractor on the scheduled refill date. For safety, all refill requests will be attempted prior to the tank reaching 30% of tank capacity. The contractor sells by the gallon and delivers its propane through state-inspected meters that are annually measured for accuracy.
The Invoice shall include the beginning/ending meter read, total quantity delivered and total amount to be paid.
Quality Control:
The contractor must comply with all applicable laws and industry standards concerning use, storage and delivery of propane. The 1000-gallon propane tank to be refilled is the property of the Indian Health Service (IHS). An inspection of the external system for delivery of propane, including external components such as the tank regulators and vents (excluding exhaust vents for products of combustion) shall be done to confirm the system is safe and adequate for the storage of propane and meets safety standards.
This safety check will be completed by the contractor prior to the first refill of the tank and the cost will be added to the first invoice. If during inspections it is determined that additional material and/or labor is needed, incurring additional costs, they must be pre-approved by the COR prior to any charges.
