About this file

This is Amendment 2 to a Request for Quote (RFQ) issued by the Indian Health Service Great Plains Area Office for solicitation 75H70626Q00214, effective November 4, 2026. The amendment provides answers to 13 questions submitted by potential offerors regarding the requirement for two Full-Time Equivalent (FTE) optometrists.

The requirement is for a Firm-Fixed Price contract (not labor hours) covering two optometrists providing full-time coverage throughout the period of performance, with a contract start date of December 1, 2026 that may be adjusted to allow credentialing and Tier 2S fingerprint processing time. The solicitation uses NAICS code 621320 (Offices of Optometrists) with a $9.0 million size standard. Key qualifications specified include board certification or residency training, DEA registration, and three professional references from the past 12 months with at least two from optometrists who have worked with the candidate directly. Named optometrists and supporting documentation (degree, BLS, malpractice certificate, immunization records) must be submitted after award. Professional liability insurance must meet state minimums for tort claims as determined globally for the company's providers. The evaluation basis is Lowest Price Technically Acceptable (LPTA) as detailed in Section H of the original combined synopsis/solicitation. Government quarters are not available. The award will consist of one Contract Line Item Number (CLIN) rather than ten option periods as originally indicated. For SBA-approved mentor-protégé joint ventures, work performed by joint venture members counts collectively toward Buy Indian subcontracting requirements, provided the JV independently satisfies applicable Indian Small Business Economic Enterprise ownership, earnings, and control requirements.

1. SF 1449, Item 1 - No quantity or unit of issue is stated. Should the unit price be per hour, per 8-hour day, or per FTE-year? What estimated hours or days per optometrist should be priced?

-This will be awarded as a Firm-Fixed Price contract and not a Labor hours contract, so billing will not be hourly, but as a whole for two (2) FTE-year. Please feel free to break down your pricing by hour, day or month but please explain it for us.

2. PWS, Description of Services - Is coverage full-time for both optometrists throughout the period of performance, or ordered as needed “during periods of permanent staffing absences”? Is a minimum guaranteed?

-Full-time coverage for both optometrists throughout the POP.

3. Synopsis G / PWS, Candidate Presentation - Must the named optometrists and their documents (degree, BLS, malpractice certificate, immunization record) be submitted with the quote, or after award upon issuance of the Purchase Order?

-After the award

4. Synopsis H, Factor 4 - Price is evaluated as the sum of items 0001 through 0010 (Option

Periods 1–9), but the SF 1449 lists a single item with a one-year period of performance and no Price Schedule Sheet is attached. Please provide the Price Schedule Sheet and confirm whether option periods apply.

-The award will be for 1 CLIN, not 10, I apologize for the error.

5. PWS, Candidate Presentation & Privileging, Item 10 - Candidate presentation requires three letters of recommendation dated within the past 6 months, while credentialing requires three references within the past year, and the quote requires three professional references. Please confirm which requirement applies at each stage.

-Three professional references are required within the past 12 months with two of those references by other optometrists who have worked with them directly or at the same site and seen their charts.

6. PWS, Privileging, Items 4 & 6 - Are board certification (American Board of Optometry) and a DEA registration required for the optometrists, or only “if applicable”? Neither appears under Qualifications.

-Board-certification and/or residency-trained optometrist is required. DEA registration is required.

7. PWS, Description of Services, Item 4 - What minimum professional liability coverage limits are acceptable to the Contracting Officer (e.g., $1M per occurrence / $3M aggregate)?

- Professional liability is determined as a global with the company for their providers and should meet state minimums for tort claims.

8. SF 1449, Period of Performance - With a 12/01/2026 start and award expected after

October 30, will the start date be adjusted for credentialing and the Tier 2S/fingerprint timeline? Is overlap with the current order anticipated?

-The start date can be adjusted to allow time for credentialing, no overlap of providers will take place.

9. The RFQ lists NAICS 621320 (size standard $9.0M). The predecessor purchase order, 75H70625P00228, was awarded under NAICS 561320 ($34.0M). Please confirm which

NAICS code and size standard apply to this requirement.

- NAICS Code for this buy is 621320 – Offices of Optometrists (size standard $9.0M)

10. Does the Government intend to order one or two optometrists at award? Is there a minimum quantity?

- This solicitation is for Two FTE Optometrists.

11. What is the basis for award (lowest price technically acceptable or best-value tradeoff), and what technical information will be evaluated?

- Basis for Award is listed in section H of the original SAM combined synopsis/solicitation.

It states the basis is LPTA and the evaluation factors follow in detail.

12. Are government quarters available to contract optometrists, and if so, at what rate?

- Government quarters are Not available

13. For an offer submitted by an SBA-approved mentor-protégé joint venture, does work performed by the joint venture’s members count as work performed by the joint venture

(JV) for purposes of the Buy Indian subcontracting limitations (HHSAR 352.226-6)?

-Yes, under the standard clause language, work performed by the JVs members counts collectively as work performed by the JV. The JV must independently satisfy the applicable

ISBEE ownership, earnings, and control requirements. SBA mentor-protégé approval alone does not establish those requirements

B. There will be no other changes by means of this Amendment.
