About this file

This is a Quality Assurance Surveillance Plan (QASP) for Solicitation Number 75F40126Q134475, a Firm-Fixed-Price contract (Base + 4 Option Years) for Bulk Carbon Dioxide Refills & Telemetry System services. The QASP outlines the monitoring framework and performance standards that the Contracting Officer's Representative (COR) will use to ensure contractor compliance with delivery and system availability requirements.

The contractor is responsible for remotely monitoring CO2 levels, providing bulk carbon dioxide with delivery documentation, quality and safety data sheets, and timely invoices. The COR will execute continuous surveillance through weekly telemetry portal logins to verify the bulk tank inventory remains above a mandatory 10-day reserve supply floor and never drops below the 30% (458-gallon) emergency threshold. At each delivery, the COR will verify that International Society of Beverage Technologists (ISBT) lot numbers and fluid quality certificates demonstrating greater than 99.9% gas purity are provided. Monthly invoice audits will reconcile invoiced quantities against delivery tickets and review performance records for service failures. Performance remedies include: (1) Tank drops below 30% floor trigger invoice withholding and mandatory Corrective Action Plan submission within 5 business days, with negative CPARS entries; (2) Emergency response delays exceeding 2 hours result in negative CPARS entries and potential termination for cause upon continued failure; (3) Telemetry downtime exceeding 24 cumulative hours per billing cycle is documented as negative performance entries; and (4) Non-conforming gas quality results in immediate rejection at contractor's expense with mandatory re-delivery of certified product within 24 hours at no additional government cost.

QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

Bulk Carbon Dioxide Refills & Telemetry System

Requirement: Bulk Carbon Dioxide Refills & Telemetry System

Solicitation Number: 75F40126Q134475

Contract Type: Firm-Fixed-Price (Base + 4 Option Years)

Primary Oversight: Contracting Officer’s Representative (COR): (to be identified upon contract award)

1. Purpose

This QASP provides an effective, streamlined method for the COR to monitor contractor performance. It ensures the contractor maintains continuous bulk gas availability and telemetry uptime, shifting operational and subcontractor supply chain risk entirely onto the vendor.

2. Roles and Responsibilities

• The Contractor: Remotely monitor CO2 level, provide CO2 as described in the requirements section with applicable delivery documentation and quality and safety data sheets, invoices submitted after work performance in a timely manner.

• The COR: Performs biweekly remote dashboard tracking, reviews delivery tickets on a monthly basis, formally documents any performance deficiencies, and monitors compliance against the PRS thresholds. This individual forwards all documented deficiency findings and contractual remedy recommendations to the Contracting Officer for formal action and enters the initial annual CPARS ratings.

• The designated POCs (one of which is the COR): Perform telemetry monitoring of the CO2 level, collect delivery documents, reconciles delivery documents against invoices, assists the COR in other functions, as necessary.

• The Contracting Officer (CO): Issues formal contractual remedies (including Letters of Concern and Cure Notices), validates performance-driven CPARS actions upon COR notification, and validates and submits final annual CPARS rating.

3. Surveillance Methods & Invoice Verification Workflow

The COR will execute surveillance using a non-burdensome workflow tied directly to monthly invoice approvals:

• Standard Outcome Monitoring (Continuous): The COR or designated POC will log into the secure telemetry web portal weekly to verify that the bulk tank inventory has remained safely above the mandatory 10-day reserve supply floor and has never dropped below the 30% (458-gallon) emergency threshold.

• Delivery Inspection (Per Occurrence): At every delivery, the COR or designated POC will verify that the contractor provided the required International Society of Beverage Technologists (ISBT) lot numbers and fluid quality certificates demonstrating a gas purity level greater than 99.9%. The contractor’s delivery technician is solely responsible for ensuring the bulk storage tank infrastructure is left securely configured in its proper, baseline operating position upon completion of the fill.

• Monthly Invoice Audit (Pre-Approval): Prior to approving the contractor’s monthly invoice, the COR will verify that the invoiced quantities align with the delivery tickets received during that billing cycle. The COR will review their performance records for that month to confirm whether any service failures occurred (such as a late delivery or an un-remediated low-supply alert). Any verified service failures will be formally documented as performance deficiencies in the contract file prior to invoice approval.

4. Performance Remedies & Contractual Consequences

If the contractor fails to meet a performance metric in the PRS matrix, the COR will issue a Performance Assessment Report (PAR) documenting the specific failure. The Contracting Officer will save the PAR to the contract file and apply corresponding remedy.:

1. Tank Drops Below Floor (<30%): The COR will withhold approval of the monthly invoice, and the Contracting Officer will mandate that the contractor submit a formal Corrective Action Plan (CAP) within 5 business days detailing their supply chain remediation strategy. Invoice payment will remain suspended until a compliant CAP is approved by the Government. A negative entry will also be recorded in the annual CPARS under the “Schedule” and “Management” categories.

2. Emergency Response Late (>2 Hours): The contractor will receive a negative entry in the annual CPARS under the “Quality of Service” category. Continued failure to meet emergency response windows shall result in a mandatory corporate Corrective Action Plan (CAP) review and may be grounds for a Termination for Cause under RFO commercial terms.

3. Telemetry Interface Downtime: The contractor shall perform immediate troubleshooting and complete all necessary system re-performance, repairs, or software updates at their sole expense. Telemetry monitoring downtime exceeding twenty-four (24) cumulative hours within a billing cycle shall be documented as a negative performance entry in the annual CPARS evaluation under the “Quality” and “Management” categories.

4. Non-Conforming Gas Quality: Immediate rejection of the entire delivery load at the contractor’s sole expense with zero payment for non-conforming product. The contractor shall re-deliver certified conforming product within 24 hours at no additional cost to the Government.
