Statement of Work Title: GeneXpert instruments Service Maintenance Agreement

SECTION 1 – BACKGROUND

The mission of Division of Healthcare Quality Promotion (DHQP) is to protect patients; protect healthcare personnel; and promote safety, quality, and value in both national and international healthcare delivery systems. DHQP is a part of the National Center for Emerging and Zoonotic Infectious Diseases (NCEZID), at the Centers for Disease Control and Prevention (CDC). DHQP is composed of nine (9) branches/office/programs which include the Office of the Director (OD), the Surveillance Branch (SB), the Prevention and Response Branch (PRB), the Immunization Safety Office (ISO), the International Infection Control Branch (IICB), the Epidemiology Research and Innovations Branch (ERIB), the Health Systems Strengthening, Resilience and Training Branch (HSSRB), the Medical Product Safety Branch (MPSB), and the Clinical and Environmental Microbiology Branch (CEMB).

To support this mission, the Clinical and Environmental Microbiology Branch (CEMB) provides reference antimicrobial susceptibility testing; provides reference identification services for anaerobes, staphylococci and related genera, and the non-tuberculous mycobacteria (excluding M. tb complex, M. leprae, and M. ulcerans); provides environmental hygiene consultations, environmental sampling and laboratory assistance for the investigation of outbreaks and other adverse events in healthcare settings; maintains surveillance programs for important, healthcare-associated bacterial pathogens, such as MRSA, C. diff, and antibiotic resistant Gram-negative bacteria, including carbapenem-resistant Enterobacteriaceae (CRE); and develops and optimizes methods to detect and decontaminate pathogenic bacteria on healthcare surfaces, water, and devices. In addition, CEMB supports and assists with coordination of CDC’s Antimicrobial Resistance Laboratory Network (AR Lab Network) which provides the infrastructure and lab capacity for public health laboratories across the country to detect and support response to resistant organisms recovered from human samples. CEMB manages the AR Isolate Bank which is a centralized repository of microbial pathogens with well-characterized resistance profiles that are assembled by CDC in collaboration with the Food and Drug Administration (FDA). The AR Bank is available to support and advance development of diagnostic devices and antimicrobial drug products. CEMB also utilizes new advanced molecular detection methods (AMD) such as whole genome sequencing to detect novel and emerging AR microorganisms and to research the microbiome.

In order complete the mission of CEMB we are committed to a continuous quality improvement process that ensures the data we generate, the research we provide and all the critical functions we perform are reliable and accurate and done with the best interest of the public. CEMB operates several laboratories that adhere to the standards set forth in the Clinical Laboratory Improvement Act (CLIA) that governs quality standards for laboratory testing performed on specimens from humans, for diagnostic, prevention or treatment and/or assessment of health. We also follow our established Quality Management Systems policies to ensure all of our equipment and methods are functioning within alignment of the agency-wide CDC Laboratory Quality Plan which requires use of an eQMS (electronic Quality & Compliance Management Solution).

CEMB utilizes Cepheid GeneXpert Instruments within our Healthcare Wastewater activitiy to evaluate the use of the Cepheid Xpert Carba-R assay for the detection of targeted carbapenemase genes from healthcare facility wastewater, and by our National Reference Laboratory (NRL) which screens clinical rectal swab specimens for the presence of bacteria as part of clinical decision making and infection control practices. Our laboratories must meet a CLIA requirement that all major equipment, inclusive of the Cepheid GeneXpert instruments, have manufacturer required maintenance performed and stay under maintenance agreements to ensure reliable reporting of results.

CDC42.0002 Evaluation of Contractor Performance Utilizing CPARS (April 2013)

In accordance with FAR 42.15, the Centers for Disease Control and Prevention (CDC) will review and evaluate contract performance. FAR 42.1502 and 42.1503 requires agencies to prepare evaluations of contractor performance and submit them to the Past Performance Information Retrieval System (PPIRS). The CDC utilizes the

Department of Defense (DOD) web-based Contractor Performance Assessment Reporting System (CPARS) to prepare and report these contractor performance evaluations. All information contained in these assessments may be used by the Government, within the limitations of FAR 42.15, for future source selections in accordance with FAR

15.304 where past performance is an evaluation factor.

The CPARS system requires a contractor representative to be assigned so that the contractor has appropriate input into the performance evaluation process. The CPARS contractor representative will be given access to CPARS and will be given the opportunity to concur or not-concur with performance evaluations before the evaluations are complete. The CPARS contractor representative will also have the opportunity to add comments to performance evaluations.

The assessment is not subject to the Disputes clause of the contract, nor is it subject to appeal beyond the review and comment procedures described in the guides on the CPARS website. Refer to: www.cpars.gov for details and additional information related to CPARS, CPARS user access, how contract performance assessments are conducted, and how Contractors participate. Access and training for all persons responsible for the preparation and review of performance assessments is also available at the CPARS website.

The contractor must provide the CDC contracting office with the name, e-mail address, and phone number of their designated CPARS representative who will be responsible for logging into CPARS and reviewing and commenting on performance evaluations. The contractor must maintain a current representative to serve as the contractor representative in CPARS. It is the contractor’s responsibility to notify the CDC contracting office, in writing (letter or email), when their CPARS representative information needs to be changed or updated. Failure to maintain current CPARS contractor representative information will result in the loss of an opportunity to review and comment on performance evaluations.

[End of Clause]
