Questions and Answers.

1. Is there an incumbent for this work or is this a new requirement?

No incumbent, and this isn’t a new requirement.

2. Are you anticipating that study personnel will only be qualified to receive 1 gift card throughout the study process or should we anticipate that personnel will be able to complete more than one survey and receive multiple gift cards for their effort?

We are anticipating that each study participant would receive 1 gift card.

3. Will each survey generate a unique study ID?

Yes, each survey will generate a unique ID.

4. Will contractor be required to create to track IP traffic or cookies to ensure all gift card recipients are unique?

No. But the vendor should track to make sure the same survey participant does not submit for multiple gift cards.

5. The RFQ refers to a “choice of a $15 gift card.” What is meant by this specifically? Does CDC envision participants being able to select from a variety of gift cards? For example, would participants have the choice between “any store gift cards (i.e., Visa or Mastercard)”, “single store gift cards (i.e., Home Depot, Apple, Starbucks)”, or “family of stores gift cards (i.e., Darden family of restaurants gift card or Gap Options gift cards usable at Gap, babyGap, GapKids, GapMaternity, Athleta, Old Navy and Banana Republic)”?

If the vendor can provide access to multiple stores, then yes.

6. If so, will CDC determine which choices will be available to participants prior to awarding the contract?

No.

7. Since unused fund balances will be de-obligated from the contract, is it correct to assume that the price to fund the gift cards should be included in the quote?

Yes.

8. Who is the incumbent for the existing/previous contract?

None.

9. What was the previous/existing contract # and award amount?

We have not assigned one

10. Is the incumbent eligible to bid?

Not applicable

11. Are the services satisfactory from the incumbent?

12. Are you open to changing the contractor?

13. I understand that this is LPTA, what is the budget ceiling for this contract? i.e. ROM (rough order of magnitude), NTE (not to exceed), etc. – This action is being competed under FAR Part 13. A budget or ROM is not being provided due to competitive procedures.

14. Can this contract be fulfilled from the contractor's location? If not, what percentage of it must be fulfilled at the place of performance specified in the RFP?

Yes, the contract can be fulfilled remotely, online.

15. In case of travel, What's the budgetary limit on daily travel/lodging expense?

Not Applicable

16. Is security clearance required? If so, will you help with the contractor in getting the security clearance?

Yes, it's currently undergoing IT and security review in Resource Planning.

17. Is FedRamp required?

Yes

18. Are you looking for COTS or custom developed solution?

Yes, we want a platform/website where the vendor can award a gift card after the participants complete a survey.

19. Is there a specific requirement on which gift card service provider we're supposed to use?

20. Is there a specific requirement on which specific retailers we're supposed to generate giftcards for? (ex. walmart, home depot, etc.)

21. "Invoice for gift card reimbursements in $2,500 increments." - So our bid does not include the cost of the gift cards, is this correct?

The cost of the gift cards should be included in your proposed costs.

22. Is breakdown of costs required?

Yes, please see RFQ for details regarding cost breakdown information.

> Automatically (using platform) generate and maintain a log of date, time and contact information used for respondent, with a study ID generated to provide documentation to NIOSH with no PII included (no email or phone numbers included).

23. Who generates the study ID?

The survey software generates the participant ID, we want to provide a link to a vendor where participants can obtain their gift card after completing the survey.

24. Is the study ID supposed to be asked & collected on the survey compensation platform?

The non-identified participant ID will need to be captured to track how many gift cards are distributed.
