About this file

This document contains a statement of work template and a related broad agency announcement solicitation. The statement of work template outlines various sections for a procurement request including background, purpose, scope of work, tasks, deliverables, period of performance, and evaluation factors. However, it does not include any specific details about required products or services.

The broad agency announcement solicitation seeks proposals for underground mine evacuation technologies and human factors research. Proposals must be submitted by April 29, 2020 to the Centers for Disease Control and Prevention for consideration of funding during fiscal year 2020. The solicitation does not specify pricing terms or set asides. The National Institute for Occupational Safety and Health will select some, all, or none of the proposals for award and provides no direct reimbursement of proposal development costs.

SECTION 1 – BACKGROUND

(Note: The background gives information about the evolution of the project as well as the how the project fits into other related projects. This section describes the requirements in general non-technical terms.

This section should explain why the acquisition is being pursued and how it relates to past, current, or future projects. Include a summary of statutory program authority and any regulations that are applicable. Delete this note before submitting the Statement of Work.)

SUBSECTION A – DEFINITIONS

(Note: If applicable, include a list of definitions. Delete this note before submitting the Statement of Work.)

This section should be a succinct statement of the purpose of the acquisition. It should outline the results that the Government expects and may also identify the anticipated benefit to the program. Delete this note before submitting the Statement of Work.)

SECTION 3 – SCOPE OF WORK

(Note: In this section explain the effort in terms of the required work and ‘how’ the work is to be accomplished.

Provide an overall, non-technical description of the work to be performed. Expand on the project purpose and objectives, but do not detail all of the work required. Identify and summarize the requirements of the project, and define its limits in terms of specific objectives, time, special provisions, or limitations. This information must be consistent with the detailed requirements and tasks. Contractor responsibilities are often summarized here, as are the results or supplies expected. Government responsibilities may be considered in organizing the statement of work. CIOs should understand their responsibility to meet any deadlines in order that dependent contractor deadlines may be met without delays in performance. Delete this note before submitting the Statement of Work.)

SECTION 4 – TASKS TO BE PERFORMED

(Note: List all the tasks the vendor will perform to complete the purpose and fulfill the objectives. Delete this note before submitting the Statement of Work.)

(Note: If end supplies or deliverables are required under the contract, they should be clearly and firmly defined and the criteria for acceptance should be given. Delivery or completion schedules are expressed either by calendar date or as a certain number of days from the date of contract award. When using the latter method, specify whether work days or calendar days are intended.

Specify how the contractor will show that it has fulfilled its obligations. Define the mechanism by which the contractor can demonstrate progress and compliance with the requirements, and can present any problems encountered. This is often accomplished through monthly or other periodic progress reports. Discuss what areas the reports are to cover, the report format, number of copies the contractor should submit, and to whom they should be submitted. Clearly identify the criteria to be used by the Government for acceptance.

If applicable, require the preparation and submission of technical and financial progress reports to help document satisfactory progress. Delete this note before submitting the Statement of Work.)

SECTION 8 – REFERENCE MATERIALS

(Note: If applicable, add the reference materials. Provide a description of, or link to, any studies, reports, and other data referred to elsewhere in the statement of work. Each document should be described, cited, and cross-referenced to the applicable part of the work statement. If documents are limited or hard to obtain, and will not be attached to, or a link provided in, the Request for Quotation (RFQ), state where the documents can be obtained, or when and where they will be available for review. Examples of references include: field memoranda, technical reports, scholarly studies, articles, specifications, and standards. Delete this note before submitting the Statement of Work.)

(Note: Technical evaluation factors permit and objective assessment of the merits of individual proposals against standards and the requirement described in the Statement of Work, rather than against other proposals. Technical evaluation factors should be tailored for the specific requirement.

Price evaluation is also required.

See below for an example of basic evaluation factors, both technical and price.

1. Technical Evaluation:

Contractors are to provide a discussion of their technical approach for providing the services required for this Purchase Order. Provide information reflecting experience of assigned staff, including a detailed resume, biography, or curriculum vitae (CV) of each candidate.

This criterion will be evaluated according to the soundness, practicality, and feasibility of the Contractor’s technical approach for providing the services required for this Purchase Order.

2. Price Evaluation:

A price analysis of the quote will be conducted to determine the reasonableness of the Contractor’s price.
