STATEMENT OF WORK
Title: Canine Rabies Surveillance Data Collection
SECTION 1 – BACKGROUND
The Poxvirus and Rabies Branch (PRB), Centers for Disease Control and Prevention (CDC) serves as a World
Health Organization (WHO) and World Organization for Animal Health (OIE) reference center for rabies and collaborates with international governments and partners. In March 2015, the city of Arequipa, Peru had its first confirmed canine rabies case after 15 years of rabies free status. CDC has developed several models for evaluating the impact of a rabies outbreak in an urban community, but to-date has not had the opportunity to evaluate this model in the setting of an ongoing outbreak. Furthermore, CDC has developed tools to conduct improved rabies surveillance. The ongoing rabies outbreak in urban Arequipa presents an opportunity to improve rabies surveillance infrastructure, while providing data critical for the evaluation of CDC-developed rabies implementation tools and collect data to evaluate and inform CDC-developed rabies transmission models. In FY19 CDC coordinated trainings with local veterinary staff on these models and adopted generic CDC guidance into local context. Early data collection has begun to inform and improve the CDC models. Additional data collection will be used to further enhance CDC-developed tools to improve their utility in enacting future CDC-led rabies control activities domestic and abroad.

SECTION 2 – PURPOSE
The purpose of this project is to collect high-quality data from the field to evaluate three CDC-developed tools related to rabies risk and transmission dynamics to allow for better implementation and management of rabies surveillance programs. The purpose of this Purchase Order is to procure evaluation reports on three CDC-developed tools and collect at least 300 rabies case-management reports to improve functionality of CDC-developed rabies transmission models. CDC will analyze the data collected through this contract to revise national and international standards and best practices for rabies surveillance programs. The results will allow for a globally applicable tool to expand to all CDC country office projects
SECTION 3 – SCOPE OF WORK
The Contractor will assist in the procurement of required supplies necessary to perform tasks. The Contractor will ensure that a cadre of trained surveillance officers are available to perform data collection and operate standardized electronic data collection tools. The Contractor will utilize CDC-developed rabies investigation tools for investigation of suspected human rabies cases, leading to collection and provision of high-quality human rabies surveillance data. Additionally, the Contractor will complete program and tool evaluation reports for the following:
REaCT Tool, Verbal Autopsy for Human Rabies Investigation Tool, Dog Population Enumeration Tool, Dog
Vaccination Tracking Tool.
SECTION 4 – TASKS TO BE PERFORMED
The Contractor shall perform the following:
1) Assist in the procurement of required supplies necessary to perform tasks; a. THE CONTRACTOR will be responsible for procuring CDC-approved tools for data collection and animal surveillance.
2) Ensure surveillance officers are properly trained on data collection methods and standardized electronic data collection tools; a. THE CONTRACTOR will conduct 3 regional trainings on electronic data collection
3) Conduct up to five human rabies investigations utilizing the CDC-developed Verbal Autopsy data collection tool;
a. The Contractor will identify suspected human rabies cases and report them to CDC, by phone and email, within 1 day of notification;
b. The Contractor will use a CDC-developed verbal autopsy data collection tool to collect information on the suspected human rabies cases;
c. The Contractor will send the completed data forms to CDC within 1 day of completion
(electronically);
d. The Contractor will analyze data to produce better national, regional, and global estimates on human rabies burden with the assistance of CDC.
4) Collect data on animal bite events.
a. The Contractor will investigate and report on animal bite events;
b. A CDC-approved, electronic bite investigation form;
c. Bite forms should be sent to CDC by the 15th of each month.
5) Ensure timely reporting of animal rabies surveillance data from the city of Arequipa surveillance activities to CDC;
a. The Contractor will ensure timely and accurate reporting of animal rabies quarantine and testing results to CDC (all results, with completed investigations forms to be delivered by the 15th of the month);
b. The Contractor will ensure timely processing and reporting of rabies samples to the Universidad
Peruana Cayetano Heredia for diagnostic testing.
i. Estimated 30 rabies samples to be tested per month, with results reported to CDC by the
15th of every month.
SECTION 5 – GOVERNMENT FURNISHED MATERIALS
The Government will provide a Data Collection Tool and Bite Investigation Form.

SECTION 9 – TRAVEL
The contractors may be required to travel domestically and internationally (approximately 2 trips as indicated below and as approved and directed by OAS for the CDC. Travel related documents generated or developed under the travel aspects of this task are not deliverables, but are to be considered as inherent to the performance of the
Purchase Order.

In order to facilitate the Government's determination whether proposed EIT supplies meet applicable Section 508 accessibility standards, offerors must submit an HHS Section 508 Product Assessment Template, in accordance with its completion instructions. The purpose of the template is to assist HHS acquisition and program officials in determining whether proposed EIT supplies conform to applicable Section 508 accessibility standards. The template allows offerors or developers to self-evaluate their supplies and documentation detail - whether they conform to a specific Section 508 accessibility standard, and any underway remediation efforts addressing conformance issues.
Instructions for preparing the HHS Section 508 Evaluation Template are available under Section 508 policy on the
HHS Web site http://hhs.gov/web/508.
In order to facilitate the Government's determination whether proposed EIT services meet applicable Section 508 accessibility standards, offerors must provide enough information to assist the Government in determining that the
EIT services conform to Section 508 accessibility standards, including any underway remediation efforts addressing conformance issues.
(d) Respondents to this solicitation must identify any exception to Section 508 requirements. If a offeror claims its supplies or services meet applicable Section 508 accessibility standards, and it is later determined by the

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-
2, Audit and Records-Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
