In compliance with Homeland Security Presidential Directive 12 (HSPD12) Coast Guard Training Center, (TRACEN) Cape May has a standardized entry protocol called the RAPIDGate Program for all vendors, suppliers, and contractors who require access to the Training Center to perform unclassified work for 5 days or more. The RAPIDGate Program, from Eid Passport, provides a standardized background check, identification badge and entry procedures for vendors, suppliers, and contractors that require access to TRACEN Cape May. Companies and individuals can register for the program at the TRACEN Security Office using the self-service RAPIDGate Registration Station. Based on the information collected the RAPIDGate Program runs a 10-year felony background screen, other criminal screens, validates the individual's social security number, verifies that the individual is either a U.S. Citizen or is legally eligible to work in the United States. Upon passing the screening process, participants will be issued a personalized RAPIDGate identification badge that when verified by a security officer will allow the participant to enter TRACEN Cape May unescorted. To enroll see the guidelines below. RAPIDGate Program Enrollment:

Enroll your company by calling Eid Passport at 877-RAPIDGATE (877-727-4342) and provide a TRACEN Cape May sponsor point of contact that includes a name, phone number, and email address. Once your request is received, final authorization to enroll in the RAPIDGate Program will be granted by the TRACEN Cape May Security Office.

Once your company has been approved for enrollment and paid the enrollment fee, instruct your employees who need access to TRACEN Cape May to register for the RAPIDGate Program using the self-service Registration Station in the TRACEN Security Office. Each employee should be ready to provide your company's RAPIDGate company code, his or her address, phone number, date of birth, and Social Security number for proof of identification and background screening. The Registration Station will capture the employees photograph and fingerprints for identity verification and badging during the application process. Sub contractor employees may register under the prime contractor company enrollment.

Once your company has approved each employee for participation, and paid the registration fee, the employee will undergo a background screen. Upon passing the screen, your employees will be contacted by TRACEN Cape May Security Office to pick up their personalized RAPIDGate Badge. The employees can show one form of identification from List A, or two forms of identification from List B when picking up their RAPIDGate Badge.

List A (One required if using this list)

Once a RAPIDGate Badge is issued, employees will be required to present their badge to gain entry to TRACEN Cape May facilities, and must wear and display the badge at all times while on the base. Questions about the CG TRACEN Cape May program should be addressed to rapidgate@tracencm.uscg.mil with the subject line re: RAPIDGate Program.

PERFORMANCE REQUIREMENT SUMMARY

(PRS)

FOR

THE COLLECTION, HANDLING,

TRANSPORTATION, AND DISPOSAL OF SOLID

WASTE, INDUSTRIAL REFUSE, GREEN WASTE,

AND RECYCLING SERVICES

U.S. COAST GUARD

TRAINING CENTER CAPE MAY, NJ

(PRS) PERFORMANCE REQUIREMENT SUMMARY

1. The Government expects the Contractor to perform all services required in the

Performance Work Statement (PWS) at an acceptable level. Whenever the Government learns of substandard performance, either through direct observation, customer complaints, or any other mechanism, the preferred course of action will be discussions with the Contractor to inform the Contractor of a problem. Discussions should effectively prevent recurrences of substandard performance. However, this Performance

Requirement Summary (PRS) provides an alternative mechanism by which the

Government’s payment to the Contractor may be reduced in the event of substandard performance.

2. Criteria for acceptable and unacceptable levels of performance are as follows: Certain tasks will be considered critical to the Government’s performance of its duty to its personnel as well as for safety, well being and health and welfare. Because of this, the

Government has listed certain tasks as critical and must be completed properly every time as scheduled. Coast Guard Training Center Cape May has identified the below listed items as critical contract requirements and will be considered as defects if not met per contract requirements:

a. Trash, Garbage, Single Stream Recycling containers not emptied on scheduled date as required by PWS para 5.1 and 5.2.

b. Trash, Garbage, Yard Waste, Single Stream Recycling containers not emptied on scheduled date as required by PWS para 5.3.

c. Scrap Metal, Bulk Items, Wood Wastes and Wood Pallets not emptied as required by PWS para 5.4.

The standards for the above items must not exceed 3 defects per month because of this criticality. However, any complaints received thereafter will cause the services to be unacceptable for that month.

3. The Government’s primary quality assurance procedures are based on scheduled inspections and validated customer complaints by the Customer Representative/Unit

Point of Contact (CR/POC). One complaint will be counted for each reported and verified unacceptable performance of service (as listed in the PWS). Thus, if one person complains that an 8 cubic yard front load trash container was not emptied during a scheduled pick up period and a 6 cubic yard single stream recycle container was not emptied for the same period, this would count as two complaints, one for each unsatisfactory performance of service. However, if two different people complain about the same 8 cubic yard front load container not emptied during a scheduled pickup period;

this would only count as one complaint because both complaints stemmed from the same unsatisfactory performance.

4. When the number of defects in the Contractor’s performance exceeds the allowable number of defects, the CR/POC shall initiate a Contract Discrepancy Report (CDR). In response to the CDR the Contractor will be required to explain, in writing, why performance was unsatisfactory, how performance will be returned to satisfactory levels, and how recurrence of the problem will be prevented. The Contractor’s payment shall be reduced by 25% of one days pay. If the Contractor is required to rework services found by the Government to be unsatisfactory, it will be at the Contractor’s expense.

5. The Contractor shall be paid for all work performed to the contract standards. Monthly payment may be reduced for unsatisfactory performance, using the following computations:

Example: The Contractor performs required services during the month. The CR/POC receives 4 complaints in a non critical area. These 4 errors compute as unacceptable. The

CR/POC takes the Contract Price (Total invoiced amount for the month) and divides it by the number of days worked that month; then multiplies that number by the deduction rate (25%) which becomes the deduction for that month.

Contract Price: $801.75/ 5 times in the month = $160.35 X 25% = $40.09.

6. A defect will not be counted when the service is re-performed within 2 hours after notification by the CR/POC and receives an acceptable rating by the CR/POC.

7. Deductions shall not be deemed to waive or limit any right of the Government under the clauses entitled “Default” and “Inspection of Services”.

8. If more than 4 deductions are taken per fiscal year, Contractor Performance Evaluations may reflect poor performance ratings.

Quality Assurance Surveillance Plan

1. Purpose: This Quality Assurance Surveillance Plan (QASP) describes methods that the

Government will use to monitor whether the Contractor’s performance of the above contract is acceptable. The QASP is the Government’s written plan for administering the Contract and monitoring major aspects of the Contractor's performance. The QASP is not part of the

Contract. It is provided to the Contractor solely for informational purposes and is subject to change by the Government at any time during the contract performance period. Therefore, the Government reserves the right to perform more or less surveillance than is provided in this QASP.

2. Introduction: The QASP establishes a method to evaluate the services the Contractor is required to furnish. It is designed to provide an effective method of monitoring Contractor performance for each listed objective on the Performance Requirements Summary (PRS) in the contract. The Government is concerned with the quality of the services provided, and not with the method that the Contractor uses to provide these services. If performance of any required service is unsatisfactory as a result of the Contractor's actions or omissions, then the

Government may reduce the contract payments to reflect the decreased value of the services received in accordance with the PRS and/or applicable FAR clauses.

3. Responsibility for Quality Control: The Contractor, not the Government, is responsible for quality control and management actions to meet the terms of the Contract. The Government performs quality assurance to monitor whether Contract standards are achieved.

4. Surveillance Methods: The Government will evaluate the Contractor's performance under this contract. Government surveillance may include, but is not limited to, the following methods:

a. Inspections.

b. Customer complaints with COR verification.

5. Deduction Methods: The Government will record all instances of substandard performance,…
