STATEMENT OF WORK (SOW)
Replacement of carpet and carpet padding in Coast Guard Housing Valdez
GENERAL.
Scope: The purpose of this project is to have the carpet and padding replaced in all upstairs bedrooms and hallways and downstairs living rooms in Coast Guard Housing, Valdez, Alaska 99686.
Objective: The Government’s objective is to meet the standards of providing clean and affordable housing for Coast Guard members stationed in Valdez, Alaska.
Short Title: Replacement of carpet and carpet padding in Coast Guard Housing Units.
1.0 DESCRIPTION OF WORK – All available houses: Contractor shall furnish all labor, tools, materials, transportation, equipment, supervision and personnel to perform and complete the removal and disposal of all old carpet and carpet padding in 5 units. After removal of all old carpet and carpet padding, the contractor shall complete all required floor preparation to the subfloor prior to the installation of the new carpet and carpet padding.
The contractor shall provide new carpet and carpet padding for all spaces requiring new flooring in all available units. The square footage of carpet and carpet padding to be replaced per unit is approximately 1100 SQFT. The new carpet will be (BRAND NAME OR EQUAL) to 24oz Timeout Khaki Purecolor carpet and 8lb Ultra Magic carpet padding or equivalent. Contractor will also provide new rubber transition inserts between linoleum and carpet, at the top of the stairs and the upstairs bathroom. The lengths of all the transitions that will need to be provided are approximately 150 FT.
2.0 DESCRIPTION OF WORK – All Available houses:

3.0 CONTINUITY OF FACILITIES OPERATION: Schedule work to minimize interference with the facilities normal operations. Notify the Government Project Officer 24 hours in advance of any shutdowns. Perform all on-site work between the hours of 8:00 AM and 5:00 PM, Monday through Friday (Federal Holidays excluded), unless otherwise approved by the Contracting Officer.
4.0 PERFORMANCE STANDARDS: Proper adherence to all safety procedures is required prior, during and after work being performed. The contractor shall furnish all personal protective equipment (PPE) as required.
The contractor shall safeguard the public and Government personnel, property, materials, supplies, and equipment exposed to contractor operations and activities, and avoids interruptions of Government operations in the performance of services.
5.0 TASK ORDER MANAGEMENT DESIGNATION: The Government Project Officer for this order is DCC Mike Ackiss at Coast Guard Housing in Valdez, Alaska who can be reached at (757) 617-3799, or via email to carlton.m.ackiss@uscg.mil. Site visits if requested may be scheduled with DCC Ackiss.
The Government Purchasing Agent for this order is Ms. Martha Bailey at U.S. Coast Guard Base Kodiak, Alaska who can be reached at (907) 487-5180 X6614 or via email to Martha.l.bailey@uscg.mil.

All contracting questions shall be routed through Ms. Martha Bailey and/or Mr. Timothy Cowley during the performance period of this order.
6.0 DELIVERABLES: Upon completion of the job, the Government Project Officer shall do a full inspection of all finished work for quality assurance.
7.0 LIABILITY OF DAMAGES: The contractor shall be liable for all damages to Government equipment and property when such damage is due to the fault or negligence of the contractor. The contractor shall be responsible for all damages or injury to persons that occur as a result of the contractor’s fault or negligence. The contractor shall take proper safety and health precautions to protect the job site, their workers, the public and the property of others.
