STATEMENT OF WORK FOR FY20 GYM MEMBERSHIP/POOL USAGE

1.0

SCOPE

1.1 Scope of Work: The purpose of this statement of work (SOW) is to describe gym membership/pool usage services provided to members of Coast Guard Air Station San Francisco.

1.2 Period of Performance: The period of performance for this SOW shall extend from 01 OCT 2019 through 30 SEP 2020.

2.0

REQUIREMENTS

2.1 Air Station San Francisco is searching for a gym with a pool facility within 15-20 miles from our location at SFO International Airport that will provide proper training for our members. The crossing of bridges must be excluded to maintain adequate time for members commute. The facility must have a pool with the following requirements: at least 25 yards in length, 12 feet or greater water depth with a shallow end of 4 feet or less, pull up bars in close proximity, allows the use of compressed air cylinders for underwater egress training, allows members to conduct swim tests while wearing a flight helmet, boots, flight suits, and weighted vests, and the ability to conduct underwater training that involves swimming the length of the pool underwater. We will require 19 memberships to cover the unit’s Aviation Survival Technicians on an annual basis. Additionally, the pool will be needed to run all of the Air Station members (around 100 people) through annual underwater egress training twice a year (one day for make-up), which involves a mock aircraft seat that will be tipped in the water with a person strapped to it. The person will attempt to get out of the seat with 2 Rescue Swimmers conducting the evolution. Finally, the pool must be large enough to accommodate parachute disentanglement procedures, which involves a person in an aviator harness attached to a 278 foot ballooned parachute canopy with a rescue swimmer supporting the evolution.

3.0

PAYMENT

3.1 Payment under this contract will be made by the Government monthly in arrears via credit card or electronic funds transfer.

4.0

INSURANCE

4.1 Workman’s Compensation and Employer’s Liability Insurance: The contractor shall comply with all applicable Federal and State Workers Compensation and Occupational Disease statures. Employer’s liability coverage of at least $100,000-000 shall be required.
