About this file
This is a sworn statement form required by Kentucky Revised Statute 45A.395, mandated for bidders and offerors competing for contracts with the City of Owensboro, Kentucky. The statement serves as a prerequisite for determining bidder responsibility in the procurement for the Moreland Avenue concrete street reconstruction project. The form requires the bidder's authorized representative to certify under penalty of perjury that neither they nor anyone acting on behalf of the bidder has knowingly violated any campaign finance laws of the Commonwealth of Kentucky, and that awarding the contract will not violate such laws. The document provides blank spaces for the company name, authorized officer signature, title, and date of execution, establishing a legal record of compliance with Kentucky campaign finance statutes as part of the bid submission requirements.
The statement defines "knowingly" according to statutory interpretation as awareness or reasonably expected awareness that conduct violates applicable law or that circumstances exist as described in the statute. This certification is a mandatory compliance document that bidders must complete and submit as part of their bid package to be considered responsible bidders. No pricing, project scope, or technical specifications are included in this document; it functions solely as a legal affidavit establishing campaign finance law compliance for bidder qualification purposes.
