About this file

This document is an Acknowledgment of Non-Negotiable Contract Terms form from Inland Empire Health Plan (IEHP) Procurement, designed to be executed by bidders and submitted with a quotation or proposal. The form requires bidders to acknowledge and accept two critical conditions: first, that all standard contract terms and conditions provided in RFP Exhibit B (IEHP Standard Contract Terms) are non-negotiable and must be accepted as-is; and second, that any exceptions, deviations, or comments not provided at the time of bid submission will not be subject to negotiation upon contract award.

The document is structured as a legally binding declaration, with the bidder's authorized signatory confirming understanding and agreement to the terms under penalty of perjury under California law. By signing, the representative affirms they have the authority to bind their company to these non-negotiable terms, and must complete the document with the date, city, and state of execution, along with their signature. The form is revision dated 09-19-2024, indicating it is a current procurement document for potential vendors seeking to do business with Inland Empire Health Plan.
