About this file
This document is a Bid Rate Sheet for a Portable Diagnostic Sleep Testing Services contract (Bid Number SD23-00080) issued by the California Department of Corrections and Rehabilitation (CDCR) and the California Correctional Health Care Services (CCHCS). The contract requires the selected vendor to provide portable diagnostic sleep testing services to CDCR/CCHCS institutions for a 3-year term. The State does not guarantee any specific business volume, as services are subject to changes in the patient population. Bidders must submit a single bid rate per service, which cannot exceed the $300 per service cap. The lowest responsible and responsive bidder will be awarded the contract, with the contract amount determined using a cost allocation formula factoring in the bidder's rank and the number of contracts awarded.
Regarding pricing, the approved bid rate will be used to reimburse the contractor for services provided. The bid rate must include all training, equipment, materials, and supplies required to provide the portable diagnostic sleep testing services as specified in the Scope of Work. The bid rate submitted will remain in effect for the full term of the agreement, including any extensions. Subcontracting is prohibited, and will result in termination of the agreement.

BID RATE SHEET
Bid Number: SD23-00080 Exhibit B-1
Estimated # of Services Per
Year x
Bid Rate Per Service (Per Patient) x 3 Year Term = Bid Amount x $ - x 3 = $ -
Basis of Award
Failure to Provide the Bid Rate shall result in the rejection of bid.
BID RATE CAP: $300.00 Per Service (Per Patient)
The Bidder must be able to provide services to all Institutions listed on Exhibit H. The State does not expressly or by implication guarantee business, as services are subject to change depending on CDCR/CCHCS fluctuation in the patient population.
BID RATES MUST BE AT OR LOWER THAN THE BID RATE CAP IDENTIFIED BELOW. ANY BID
RECEIVED THAT EXCEEDS THE BID RATE CAP WILL BE REJECTED.
PORTABLE DIAGNOSTIC SLEEP TESTING SERVICES
In the case of discrepancy between the Bid Rate Per Service (Per Patient) and the Bid Amount, the Bid Rate Service shall prevail.
Bidder shall submit one (1) Bid Rate in clear legible figures in the space provided below. The Bid Rate will include the all-inclusive training and equipment/materials/supplies that are required to provide Portable Diagnostic Sleep Testing (PDST) Services as specified in the Exhibit A, Scope of Work (SOW). The approved Bid Rate will be used to reimburse Contractor for services provided to the Institution listed on Exhibit H.
The lowest responsible and responsive Bidder will be the Rank 1 Contractor on the Bid Matrix. The contract amount will be determined using a cost allocation formula that includes the Contractor's rank and the number of contracts awarded.
Contractors with multiple Agreements for the same service at the same Institution shall be obligated to provide service at the rate specified in the Contractor’s primary Agreement (i.e., the Agreement resulting from the first bid) until all obligations under that Agreement (e.g., number of hours) are satisfied before the rate in any subsequent Agreements can be used. The only exception to this provision occurs when the rate in a subsequent Agreement is lower than those of the primary Agreement; the State then has the sole right to determine which rate will be applied. If awarded an Agreement, the rate submitted below shall remain in effect for the term of the Agreement, including any extension of the term through a formal amendment. Under no circumstances can any portion of this service be subcontracted. Subcontracting shall be cause for termination of this Agreement.
