About this file

This document is a Bid Submission Form for a city procurement process that requires bidding companies to certify their non-discriminatory practices and compliance with the city's contracting requirements. The form outlines a comprehensive set of declarations and agreements, including a commitment to fair solicitation and selection of subcontractors, with specific provisions against discrimination based on various protected characteristics such as race, gender, religion, national origin, sexual orientation, veteran status, and disability. The bid is valid for 120 calendar days from the bid due date, and the city reserves the right to reject any or all bids and make awards according to its best interests.

The form mandates that bidding companies provide documentation demonstrating non-discriminatory practices and disclose any legal complaints filed against them in the past ten years regarding discrimination against subcontractors, vendors, or suppliers. Companies must also affirm that none of their owners, employees, directors, or contractors will violate the city's Conflict of Interest Policy. Additionally, the form requires a detailed contact information section for the bidding company's representative and an optional contact for bid submission questions, ensuring transparency and clear communication throughout the procurement process.
