About this file
This is a Request for Proposal (RFP) issued by the Montana Department of Public Health and Human Services, Public Health and Safety Division, seeking contractors to implement the Montana Diabetes Prevention Program (MDPP). The RFP was released on 12/28/2018 with a proposal submission deadline of 1/31/2019, and aims to provide initial start-up funding for a two-year project period to implement a diabetes prevention program (DPP) that will be independently sustained thereafter. The solicitation is classified under multiple commodity codes including educational services, health-related services, and professional services, with the primary goal of establishing a program to prevent diabetes.
The RFP requires potential offerors to complete several mandatory components, including submitting company details, acknowledging the scope of services, addressing offeror qualifications, and completing a cost proposal. Additional requirements include compliance with an Equal Pay for Women attachment and submission of Dark Money Declaration forms. While specific funding amounts are not explicitly stated in the documents, the RFP indicates that contractors will receive initial start-up funding for a two-year period. Potential bidders must carefully review all prerequisites, including the Single Point of Contact, proposal submission guidelines, and the evaluation scoring matrix, which will determine how proposals are assessed and ranked.
