VHAPM Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

VHAPM Part 813.106 Page 1 of 1 Original Date: 08/30/17 Revision 01 Eff Date: 04/23/2018

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1

For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C246-18-AP-0786

1. Contracting Activity: Department of Veterans Affairs, VISN 06, Hampton VA Medical Center, 100

Emancipation Drive, Hampton VA 23667.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

This is an emergency requirement to supply the Asheville VAMC with a Steris 4085 Surgical Table.

The Steris 4085 General Surgical Table is a highly versatile general surgical table that is electro-hydraulically operated. It is designed to support all general and bariatric surgeries to include;

nephrectomies and cardiac, vascular, endoscopic, gynecologic, urologic, neurologic, ophthalmologic and orthopedic procedures. The estimated amount is; $42,693.16.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

Asheville’s current GU clinic table (Steris CMAX-S) is non-operational. The current surgical table has a broken hand control, diminished foot pump maneuverability, as well as other signs of general wear and tear that is beyond economical repair costs. Replacing the damaged table with an upgraded version from the manufacturer will maintain continuity of care and operations, while significantly reducing the training burden on the staff. In its current state, the surgical table in use at Asheville’s

GU clinic poses a safety risk to patients and staff alike. The item in need is proprietary to Steris

Corporation (Large Business, DUNS: 177360039). Steris Corporation has identified Buffalo Supply

Inc. (Small Business, DUNS: 109070714) as their exclusive reseller/distributor for the Department of

Veteran Affairs.

4. Description of market research conducted and results or statement why it was not conducted:

Due to the urgent nature of the acquisition, market research efforts were limited. Efforts included contacting the suggested source (Buffalo Supply Inc.) to confirm that they were authorized resellers for the manufacturer. A search of the FPDS-NG website was also conducted in an effort to review previous purchases for the item in need for this acquisition. The Contracting Officer discovered that there were awards made directly to Steris Corporation as well as to Buffalo Supply Inc.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
