About this file
This is a UNM Proposed Sole Source Purchase Form submitted by the University of New Mexico's Office for Community Health for services in New Mexico. The request, dated February 12, 2026, seeks to contract with the Guidance Center of Lea County to provide Community Health Worker (CHW) navigation services to assist older adults with accessing benefit programs, Partner HERO support to collaborate with CHWs in addressing systems issues related to services for older adults, and supervision of CHW and Partner HERO staff. The contract term runs from January 1, 2026 through December 31, 2026, with services to be delivered in Lea County to support the Health Equity and Social Connectedness for Older Adults Project (HE+).
The proposed sole source purchase is valued at $83,000.00, exceeding the $60,000.00 threshold requiring posting on the UNM Sunshine Portal and the State of New Mexico Procurement website for 30 days prior to purchase. The justification for sole source procurement is based on the vendor's proprietary capabilities and compatibility requirements, specifically that the Guidance Center of Lea County staff has already been trained through the initial pilot of the HE+ project funded by the New Mexico Aging and Long-Term Services Department and possesses required certifications including HIPAA and Social Determinants Adversity training. The vendor is also experienced with the required reporting tools (Redcap and Smartsheet) and has a Partner HERO on site. Prior due diligence included consultation with community health specialists to identify a non-profit organization in Lea County with the capacity and qualifications to supervise and implement the project, though the form does not list other vendors that were contacted or their specific reasons for disqualification.
