About this file
This is a Drug-Free Workplace Certification form required by the State of Florida under Section 287.087 of the Florida Statutes for the Re-Bid Claim Field Investigative Services contract opportunity. The certification applies to firms competing for field investigative and adjusting services, including reporting and processing from local offices or resident adjusters, with most work conducted during normal City business hours (Monday-Friday, 7am-7pm). The resultant contract will have a 5-year base period, and firms must demonstrate sufficient resources and qualified staff to investigate liability claims for the City.
To qualify for drug-free workplace program preference in the award process when bids are equal in price, quality, and service, bidding firms must certify compliance with six specific requirements: publishing a workplace statement prohibiting unlawful manufacture, distribution, dispensing, possession, or use of controlled substances; informing employees of drug abuse dangers and available assistance programs; providing each employee with a copy of the workplace policy statement; requiring employee notification of any drug-related convictions occurring in the workplace within five days; imposing sanctions or requiring participation in rehabilitation programs for convicted employees; and maintaining good faith efforts to sustain a drug-free workplace. Respondent firms must have an authorized representative sign and return the completed certification form to validate their drug-free workplace program compliance.
