About this file
This is a Buy American Certificate form (FAR 52.225-2, October 2022 version) that offerors must complete as part of their proposal submission for the Sonosite ST Ultrasound System solicitation (36C26226Q1377) issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 22.
The certificate requires offerors to certify the domestic or foreign origin status of all end products being offered. Offerors must identify each end product as either domestic or foreign, list any foreign end products manufactured in the United States that do not qualify as domestic products, and indicate whether foreign end products (excluding commercially available off-the-shelf items) exceed 55 percent domestic content. Additionally, offerors must separately identify line-item numbers for any domestic end products containing critical components as defined in FAR Part 25. The form includes designated spaces for offerors to list foreign end products by line-item number and country of origin, along with their domestic content percentages, and to list domestic end products containing critical components. Government evaluation of proposals will proceed in accordance with the policies and procedures outlined in FAR Part 25, and the form must be signed by an authorized representative.
