PAST PERFORMANCE QUESTIONNAIRE

Past Performance Questionnaire

The United States Securities and Exchange Commission, Office of Acquisitions, intends to acquire Supplemental Retirement Plan (SRP) Services. As part of the procurement process, an increased emphasis is being placed on past performance as a source selection factor.

In order to accurately evaluate the quoter’s past performance your assistance is requested in obtaining and verifying the past performance history of this quoter. It is important to note that names of individuals providing reference information about a quoter’s past performance will not be released to the contractor.

Two forms are provided to complete the evaluation: 1) Performance Rating Form; and 2) Additional Comment Form. Upon completion of this form, please submit it as an e-mailed attachment to Patrick Sisk at SiskP@sec.gov. Please make the subject line of the e-mail read “50310219Q0165 - Past Performance Questionnaire – Insert Evaluated Company’s Name.” Submissions must be received by Monday, October 7.

The following standards shall be used in arriving at the rating.

Exceptional Contractor’s performance exceeded customer expectations or contract requirements, demonstrated an above satisfactory understanding of the requirements and provided significant features or benefits.

Acceptable Contractor met customer expectations or contract requirements and demonstrated a satisfactory understanding of the requirements.

Marginal Contractor’s barely met customer expectations or contract requirements and demonstrated a less than satisfactory understanding of the requirements.

Unacceptable Contractor did not meet customer expectations or contract requirements and demonstrated an unsatisfactory understanding of the requirements.

Other If the element is not applicable, indicate with “N/A.” If no data has been obtained or additional comments are provided, please note in this column.
