About this file
This document is a Proposed Sole Source Purchase Form from the University of New Mexico (UNM) for a contract opportunity with The Guidance Center of Lea County. The contractor will provide staffing and supervision for one Community Health Worker and one Partner HERO to serve older adults in Lea County, New Mexico. The project aims to address social drivers of health and increase access to benefit programs for older adults in the region. The contract term is from September 1, 2024 to June 30, 2025 at a total cost of $126,141.60.
The justification for sole source procurement is that The Guidance Center has the necessary experience, staffing, organizational capacity, and alignment with the project's goals to successfully provide the required services. Other potential vendors were considered, but the Guidance Center was determined to be the only organization with the specific capabilities and community connections needed to effectively serve the target population in Lea County.

Buyer Team - See Commodity list at http://purchase.unm.edu/department-information/comm-code-listing.pdf Provide a basic description of goods/services to be provided:
Contractor will staff and supervise one Community Health Worker and one Partner HERO to serve Older Adults in McKinley County. They will conduct community outreach in Lea County, New Mexico and provide assistance to Older Adults who need help accessing services and benefit programs. This effort will address social drivers of health that impact adults who are 60 years of age and older.
Why is this purchase needed?
There are many Older Adults in New Mexico who are eligible for benefit programs but are not accessing these services, due to stigma, structural barriers, lack of clarity about eligibility, need for support with application process, etc. There is a high level of need in Lea County, creating higher numbers of health disparities and greater need for support services. The funder chose to focus on serving the northwest area of the state in hopes of improving health disparities in tribal communities. This project seeks to address these issues by providing culturally sensitive support to access benefit programs.
