About this file

This is a Trench Safety Act Requirement form for the Wiles Road Westbound Departure Lane Transition Improvements project in Florida, administered by a county highway construction and engineering division. The fixed-price contract requires vendors to furnish all labor, materials, equipment, tools, and services necessary for roadway improvements on Wiles Road from University Drive to Coral Hills Drive, including miscellaneous signing and pavement marking enhancements extending to Riverside Drive. Vendors must submit their solicitation responses electronically through BPRO and obtain a submission receipt by the specified closing date and time. Vendors are required to ask questions and seek clarification through BPRO by the designated questions due date, and the county will respond through the BPRO messages section. All submittal documents, including both required and optional items, should be provided at the time of submission to enable timely and comprehensive review.

The form requires vendors to demonstrate compliance with the Florida Trench Safety Act (Sections 553.60-553.64, Florida Statutes) and OSHA excavation safety standards (29 C.F.R. Section 1926.650) for any project trenches exceeding 5 feet in depth. Vendors must submit a completed Trench Safety Act form with their solicitation response or within three business days of the county's request, with non-compliance potentially resulting in a nonresponsive determination. The form requires vendors to identify both the anticipated method and cost of compliance with applicable trench safety standards, including special shoring requirements if applicable, with costs structured either by linear feet or square feet depending on whether the contract is open-end or fixed. Additionally, vendors must hold an FDOT Certificate of Qualification, meet bonding requirements, and address County Business Enterprise goal participation as outlined in the solicitation documents.
